레즈비언, 게이, 바이섹슈얼 등을 포함하는 성소수자(sexual minority; LGB)는 성적 지향에 대한 낙인과 편견을 지속적으로 경험한다. 선행 연구들은 성소수자가 전체 인구에 비해 건강 격차를 경험하고 있음을 보고해 왔다. 그리고, 성소수자의 소수자 스트레스 과정모형은 역사적으로 주변화 되어온 이 인구 집단이 경험하는 부정적인 사회적 경험이 이들의 건강에 영향을 미치는 요인임을 제시하고 있다. 성소수자에 대한 사회적 인식이 부정적으로 남아있는 한국의 경우, 성소수자의 건강을 이해하기 위한 연구가 부족한 상황이다. 본 학위논문은 한국 성소수자(LGB)가 경험하는 건강 격차를 파악하고, 반동성애 폭력 경험과 정신건강 결과의 연관성을 확인하고, 이 연관성에서 사회적 지지가 조절효과를 가지는 지를 알아보고자 했다. 본 학위논문은 “레인보우 커넥션 프로젝트 I – 한국 성인 레즈비언, 게이, 바이섹슈얼 건강 연구”라는 이름으로 수행된 전국 단위의 단면 조사 자료를 활용했다.
연구의 첫번째 부분에서는 성소수자 참여자의 각 건강 변수 유병률을 전체 인구의 유병률과 비교하여 한국 성인 성소수자가 경험하는 건강 격차를 확인했다. 세 개의 국가 대표성 있는 설문조사 자료에 참여한 일반 인구를 준거집단으로 하여 부정적 자가평가 건강과 우울 증상, 자살 생각 및 시도, 근골격계 통증, 흡연과 위험 음주에 대한 연령 표준화 유병비를 산출하였다. 분석 결과, 한국 성인 성소수자는 전체 인구에 비해 부정적 자가평가 건강, 우울 증상, 자살 생각 및 시도, 근골격계 통증에서 통계적으로 유의하게 높은 유병률을 보였다. 성소수자 여성의 경우 전체 여성 인구에 비해 통계적으로 높은 흡연율을 보고했으며, 레즈비언 여성의 경우 전체 여성 인구에 비해 위험 음주를 할 가능성이 높은 것으로 나타났다. 이러한 연구 결과는 한국 성소수자가 전체 인구에 비해 부정적인 건강 상태를 가지고 있음을 보여준다.
연구의 두번째 부분에서는 한국 성소수자의 반동성애 폭력 경험과 정신건강의 연관성을 분석하고, 이러한 연관성이 가족의 지지 수준에 따라 달라지는지를 확인하고자 했다. 전체 2,152명의 참여자 중 47.7%가 만 19세 이후 언어적 폭력, 신체적 폭력의 위협, 신체적 폭력, 성폭력 등 반동성애 폭력을 한번이라도 경험한 적이 있는 것으로 나타났다. 각각의 반동성애 폭력 경험은 우울 증상과 자살 생각 및 시도와 통계적으로 유의한 연관성을 가지는 것으로 나타났다. 또한 한 가지 유형의 반동성애 폭력을 경험한 이들과 두 가지 이상의 반동성애 폭력을 경험한 이들은 각각 반동성애 폭력 경험이 전혀 없는 이들에 비해 우울 증상과 자살 생각 및 시도를 경험할 가능성이 유의하게 높았다.
이러한 연관성을 가족의 지지 수준으로 층화 한 결과, 반동성애 폭력 경험과 정신건강 결과의 연관성은 중간 수준 및 높은 수준의 가족의 지지를 보고한 집단 내에서만 유의하게 나타났다. 결합 분석(joint analyses) 결과, 경험한 반동성애 폭력 유형의 개수가 많아질수록, 가족의 지지 수준이 낮아질수록, 우울 증상과 자살 생각 및 시도의 유병비가 점진적으로 낮아짐을 확인했다. 연구 결과는 가족의 지지가 한국 성소수자의 긍정적 정신 건강 결과와 연관성이 있는 한편, 가족의 지지가 낮은 집단에 비해 높은 집단에서 반동성애 폭력이 성소수자의 정신 건강에 미치는 부정적 영향이 더욱 클 수 있음을 제시한다.
본 학위논문은 한국 성소수자 인구의 건강 수준을 증진하기 위한 공중보건 개입이 필요함을 보여준다. 본 학위논문의 결과를 바탕으로 국가 대표성 있는 설문조사에서의 성적지향 측정, 정신 건강 개입 전문가들에 대한 문화적 역량 교육, 성소수자에 대한 사회적 인식의 증진 및 반동성애 폭력의 예방과 구제를 위한 정부 차원의 개입 등의 정책적 개입 방안을 제안한다.
The sexual minority population, including lesbian, gay, and bisexual individuals (hereafter LGB), has persistently been a target of stigma and prejudice due to their sexual orientation. Previous literature documented that the LGB population experiences various health disparities compared to the general population, and the minority stress process model of the LGB population was proposed to explain the health impact of negative social experiences among this historically marginalized population. However, there is a paucity of existing studies on the health of the LGB population in South Korea, where the public attitude toward the population remains negative. The dissertation aimed to investigate health disparities of Korean LGB adults, and examine the association of mental health outcomes with experiences of homophobic victimization and potential effect modification by the level of family support in the association, using a nationwide cross-section survey conducted in 2016, “Rainbow Connection Project I - Korean Lesbian, Gay, & Bisexual Adults’ Health Study” (hereafter RCP I).
In the first part of the study, the health disparities of Korean LGB adults were investigated by comparing the prevalence of health outcomes of LGB participants with those of the general population. Age-standardized prevalence ratios (SPRs) were estimated for poor self-rated health, depressive symptoms, suicidal behaviors, musculoskeletal pain, smoking, and hazardous drinking, using the three nationally representative survey data as reference populations. Korean LGB adults consistently exhibited a statistically significantly higher prevalence of poor self-rated health, depressive symptoms, suicidal ideation, suicide attempts, and musculoskeletal pains compared to the general population. Sexual minority women also reported a significantly higher risk of smoking, and lesbian women are more likely to engage in hazardous drinking compared to the general women population. These findings suggest that LGB adults have adverse health conditions compared to the general population in Korea.
The second part of the study aimed to examine the association between homophobic victimization and mental health outcomes among LGB adults in Korea and investigate how this association differs by the level of family support. Among a total of 2,152 participants, 47.7% have ever had the following homophobic victimization experiences from the age of 19: verbal violence, threat of physical violence, physical violence, and sexual harassment. Each type of homophobic victimization was significantly associated with a higher prevalence of depressive symptoms, suicidal ideation, and suicide attempts. Compared to those who never experienced homophobic victimization, those who experienced one type of homophobic victimization and two or more types of victimization showed a significantly higher prevalence of depressive symptoms, suicidal ideation, and suicide attempts.
When stratified by the level of family support, the association between homophobic victimization and mental health indicators was only significant for those with medium and high levels of family support. In the joint analyses, as the number of experienced homophobic victimization increased and the level of family support decreased, a gradient increase in adjusted prevalence ratios of depressive symptoms, suicidal ideation, and suicide attempts was observed. Findings of this study suggest that experiences of homophobic victimization may have a greater impact on mental health among Korean LGB adults with a higher level of family support, even when family support may protect them from adverse mental health.
The overall findings of this dissertation indicate that public health interventions are needed to improve the health of the Korean LGB population. Based on the findings of the dissertation, the following policy implication could be recommended: inclusion of sexual orientation measurement in nationally representative surveys, cultural competency education for mental health care providers, and government-level measures to raise public awareness of the sexual minority population and prevent and take corrective action for homophobic victimization.
