집단적 이야기치료를 적용한 정신보건 종사자 인권교육의 사례연구 : 정신장애 당사자인 인권강사가 선호하는 인권이야기를 중심으로

본 연구의 목적은 집단적 이야기치료(Collective Narrative Practice)를 적용한 인권교육에 강사로 참여한 정신장애인들의 ‘선호하는 인권이야기(Preffered story)’의 구성 과정 및 내용과 그 이야기에 대한 ‘정신보건종사자들의 반응’을 이해하는 것이다. 또한 분석되어진 당사자와 종사자들의 현장 담론들이 어떻게 현재의 정신보건서비스의 세팅 및 제도와 연결될 수 있을지를 사회적 맥락에서 비판적으로 이해하고자 하였다.
본 연구의 참여자는 2011년 5월부터 2012년 11월까지 인권교육에 강사로 참여한 정신장애인 8명이었으며 자료수집방법은 심층면접, 집단상담, 문서, 참여관찰을 통해 이루어졌다. 연구방법은 인권교육을 하나의 사례로 보고 그 수행을 풍부히 서술하고 이해하기 위해 사례연구를 선택하였으며 구체적 분석과정은 수집된 자료를 전체적으로 기술 및 검토한 후 수정근거이론((Modified Grounded Theory Approach)을 통해 개념(concepts)과 범주(categories)를 발견하고 추상화하였다. 개방코딩을 통하여 23개의 개념과 분석결과에 대한 확인 작업을 거쳐 10개의 범주가 도출되었다.
분석결과 정신장애인의 선호하는 이야기의 재구성 범주들은 ‘커밍아웃의 어려움’, ‘평등한 인간으로 서고 싶음’,‘새로운 역할과 책임의 기회’, ‘선호하는 자기이야기 발견과 인정’, ‘인권침해의 사회적 맥락화’,‘자신과 집단에 대한 긍정적 정체성 형성이었다. 이 중 커밍아웃의 어려움은 장애의 핵심현상으로 의료적 질환으로 중심화 된 정신장애인에 대한 인식에는 치안, 주거, 정서적 공감, 경제능력 등의 다양한 요인이 혼종 되어 상호작용함으로써 사회적인 정신장애인에 대한 지배적 표상이 형성되어졌다.
정신보건종사자의 반응에 대한 범주들은 ‘공감적 이해 반응’,‘조직순응적 직업인 반응’,‘전문가 중심적 반응’,‘당사자중심적 반응’으로 도출되었다. 이러한 반응은 유형분석을 통해 사회적 활동형, 자조적 회복 추구형, 온정적 돌봄형, 권위적 치료과정형으로 나타났다.
본 연구는 해방적 가치에 기반한 실천사례로 한국에서 적용되지 않았던 집단적 이야기치료를 실천의 방법으로 하여 정신장애를 가진 당사자가 자기옹호를 목적으로 참여한 사례 수행과정과 내부자적 관점을 풍부히 이해할 수 있다는 점에서 의의가 있다.
본 연구는 연구결과를 통해 정신장애인들의 ‘선호하는 인권이야기’가 보다 존중되고 확산되기 위해서는 첫째, 정신장애인에게 부여된 부정적인 표상과 정체성의 변화, 둘째, 사회적 역할 수행의 기회 부여와 실질적 권한 부여, 셋째, 정신보건 현장에서의 포스트모던적 내러티브 윤리와 실천방법론 확대, 넷째, 구체적인 자기옹호의 이슈의 개발과 집단적 옹호 활동이라는 네 가지 논의와 제안을 도출하였다.
The purpose of this study is to understand the constructing process and contents of 'preferred story' that people with mental disorder participated as a lecturer in human rights education applying collective narrative practice and the response of the mental health professionals about the story. This study is also try to comprehend how the current system of mental health service may be associated with the discourse of the professionals and the people with mental disorder more critically in social contexts.
Participants in this study were 8 people with mental disorder involved in human rights education as a lecturer from May 2011 to November 2012. Data were obtained by in-depth interview, group counseling, literature and participant observation. To understand and describe the practice of the human rights education abundantly as a case, this study conducted case study. After the data reviewing and overall modifying, it was analyzed by modified grounded theory approach, and through the analysis, many concepts were induced to 23 concepts and 10 categories.
Through the analysis, reconstructed categories of story the people with mental disorder prefer to were 'the difficulty of coming out', 'being treated as equal human beings', 'the opportunity of new roles and responsibilities', 'preferred self-talk detection and self approval', 'social contextualization of human rights violations' and 'forming positive identity for themselves and their group'. Among them, the difficulty of coming out is the core symptom of the disorder, which is induced from the dominant social representation that recognizing the people with mental disorder as having mental disease. It is caused by interacting various factors such as public safety, housing, emotional empathy, economic capacity.
The response categories for the mental healthcare professionals were 'empathic understanding response', 'adaptive response', 'expert-orient response', 'the people involved-orient response'. Through the analysis, these responses were represented as the type of social action, self restoring pursuit, compassionate care and prescriptive process of treatment,
As a practical case based on 'emancipating value', this study contributes to understand abundantly the perspective of the participants who involved in collective narrative therapy in order to develop self efficacy.
The results of the study shows that to make the preferred story of the people with mental disorder being respected and extended, we need to deal with the change of identity and negative representation about the people with mental disorder, the grant of authority and opportunity of performing social roles, the expansion of post modern narrative and practice methodology in the field of mental health, and finally, the development of specific self-efficacy and collective efficacy. This study also made suggestions about those four discussions.
