목적: 본 연구는 40세 이상 한국 성인 남, 녀를 대상으로 백내장의 수정체 혼탁 부위별 분포를 파악하고, 수정체 혼탁 부위와 관련된 요인을 알아보고자 시행하였다.
방법: 제5기 1, 2차년도(2010, 2011년) 국민건강영양조사(Korean National Health and Nutrition Examination Survey; KNHANES) 자료를 이용하여 40세 이상 총 8,508명을 대상으로 단면연구를 시행하였다. 이 자료를 통해 백내장의 혼탁 부위에 따라 피질백내장, 핵백내장, 기타백내장 (전낭하백내장 + 후낭하백내장 + 혼합백내장)으로 나누어 혼탁 부위별 빈도와 백분율(%)이 산출되었다. 또한 요인들 간의 관련성을 파악하기 위하여 연령, 성별, 당뇨, 고혈압, 고지혈증을 보정한 로지스틱 회귀분석(survey logistic regression)과, 일원배치 분산분석(one-way ANOVA)을 시행하였다.
결과: 본 연구의 결과는 다음과 같았다.
(1) 혼탁 부위별 분포는 핵백내장 (2,048명) > 피질백내장 (707명) > 기타백내장 (642명) 순이었다.
(2) 연령이 높을수록 피질백내장, 핵백내장 그리고 기타백내장의 교차비가 높았으며 통계적으로 유의하였다. 소득 1분위에서 핵백내장의 교차비가 높았으며, 초등학교 이하 졸업에서 피질백내장, 핵백내장 그리고 기타백내장의 위험도가 높았고 통계적으로 유의하였다. 현재 음주자의 경우 기타백내장의 위험도가 감소하였다.
(3) 근시가 있는 경우 기타백내장의 위험도가 높았으며, 당뇨가 있는 경우 피질백내장, 핵백내장 그리고 기타백내장의 위험도가 높았고 통계적으로 유의하였다. BMI의 경우 WHO기준으로는 ‘정상’과 ‘과체중’에서, KSSO기준으로는 ‘과체중’과 ‘비만’에서 핵백내장의 위험도가 낮았으며 통계적으로 유의하였다.
(4) 남성의 경우 비백내장군은 백내장군에 비해 쌀, 콩류, 잡곡은 적게 섭취하였으며, 커피, 돼지고기는 많이 섭취하였다. 여성의 경우 비백내장군은 백내장군에 비해 쌀은 적게 섭취하였으며 김, 달걀, 두부, 버섯류, 요구르트, 빵류는 많이 섭취하였다.
결론: 한국 성인에서 백내장의 혼탁 유형별로 관련 요인이 상이한 것으로 나타났으며, 관련 요인에 대한 교차비 또한 달랐다. 따라서 후속 연구도 혼탁 유형을 구분하여 진행되어야 할 것이며, 환자의 검사와 상담시 한국인 실정에 맞는 기초자료로써 이용될 수 있을 것이다.
Objectives: The purpose of this study is to investigate the factors related with cataract by researching dispersion of cataract subtypes of Korean people over age 40.
Methods: This study proceeds with cross-sectional surveys including the 5th Korean National Health and Nutrition Examination Survey between 2010 and 2011, which covers 8,508 participants over age 40. Based on this survey, the percentage of cataract subtypes is calculated; the cataract subtypes include cortical cataract, nuclear cataract, and other types of cataracts such as anterior (sub) capsular, posterior subcapsular and mixed. Furthermore, this study executes logistic regression analysis and one-way ANOVA that adjust the data on age, sex, diabetes, hypertension, and hyperlipidemia in order to find out the relationship among factors.
Results: The research results are summarized as follows.
(1) The percentage of cataract subtypes is as follows in order: nuclear cataract (N=2,048) > cortical cataract (N=707) > other cataracts (N=642).
(2) The older people are, the greater cross ratio of cortical cataract, nuclear cataract and other types of cataracts is, and it has statistical significance. In case of current drinkers, the risk of being exposed to cataracts decreases.
(3) In case of Myopia, the risk of being exposed to other types of cataracts is higher than usual. In case of diabetes, the risk of being exposed to cortical cataract, nuclear cataract, and other types of cataracts is higher than usual, and it has statistical significance. In case of BMI, WHO standard indicates that those who are normal (18.5∼24.9) or over-weight (25.0∼29.9) have the lower risk of being exposed to the nuclear cataract than usual, and KSSO standard indicates that those who are over-weight (23.0∼24.9) or obese (≥25) have the lower risk of being exposed to the nuclear cataract that usual. This data has statistical significance.
(4) The non-cataract group of males prefers coffee and pork to rice, bean, and barley. On the contrary, the cataract group of males prefers rice, bean, and barley to coffee and pork. The non-cataract group of females intakes laver, egg, tofu, mushroom, yogurt, bread more frequently than the cataract group of females who prefer rice.
Conclusions: In conclusion, this study shows that related factors among Korean adults are various in cataract subtypes and cross ratio of related factors is different. Therefore, a follow-up study should proceed by distinguishing types of cataract subtypes, and this data might be used for consultants and examination on patients in Korea.
