빅데이터를 이용한 대기오염의 건강영향 평가 및 피해비용 추정(Ⅲ) = Assessment of Human Health Effects of Air-Pollution Using Cohort DB and Estimation of Economic Costs in Korea (III)

본 연구는 3년 과제로 기획하였으며, 표본코호트DB를 활용한 대기오염의 건강영향 평가, 건강영향의 피해비용 산정을 위한 단위가치 추정, 건강영향 평가와 단위가치 추정을 연계한 정책평가 분석 틀 구축 및 피해비용 추정 사례연구로 구분하여 진행하였다. 특히 최근 활용이 가능해진 국민보험공단의 표본코호트DB를 활용하여 대기오염으로 인한 만성 건강영향 평가체계를 개선하고, 건강영향(입원, 사망)을 고려한 단위가치를 추정함으로써 건강영향과 경제성 분석의 연결고리를 강화하고자 하였다.
1차 연도(2015)에서는 표본코호트DB를 활용하여 대기오염 장기노출로 인한 호흡기계질환의 신규입원과 반복입원(재입원) 영향을 분석하였으며, 단위가치 추정은 질병영향과 연계 가능한 호흡기계 질환의 에피소드 건당 질병비용(COI)을 추정하였다. 2차 연도(2016)에서는 단기노출로 인한 호흡기계 사망영향을 평가하였으며, 사망영향과 연계 가능한 단위가치 산출을 위하여 대기질 개선으로 인한 사망확률 감소에 대한 지불의사액(WTP)으로부터 통계적생명가치(VSL)를 도출하였다.
3차 연도(2017)의 주요 연구내용은 PM10의 입원, 사망영향 평가결과에 질병비용(COI), 통계적생명가치(VSL)를 연계한 정책평가 분석 틀을 구축하고 해당 피해비용을 추정하는 것이었다. 그러나 최근 PM2.5의 건강위험과 그에 따른 피해비용이 우리 사회의 심각한 이슈로 부상함에 따라 이를 고려하여 본 연구의 통합분석의 범위를 확대하였다. 먼저 농도반응(CR)함수는 PM10뿐만 아니라 PM2.5의 입원영향, 사망영향을 포괄적으로 다룰 수 있도록 선택하였다. 또한 건강영향은 호흡기계로 한정하되 노출 인구집단을 전체연령과 65세이상으로 구분하여 취약집단에 대한 건강영향을 병행하여 살펴보고자 하였다. 더불어 피해 비용 추정 시 입원, 사망영향 이외에 PM10이 호흡기계 질환의 외래방문에 미치는 영향을 추가하여 피해비용 산정의 범위를 확대하였다.
통합분석은 2015년을 기준 시점으로 한 건강영향의 피해비용 추정과 PM10, PM2.5 정책대안 적용을 통한 경제적 편익 산정을 병행하여 진행하였다. 정책대안은 2015년 현재대기 중 PM10 및 PM2.5 농도를 ① WHO 연평균 권고기준(20㎍/㎥, 10㎍/㎥)으로 감소시킬 경우, ② WHO 연평균 잠정목표3(30㎍/㎥, 15㎍/㎥)으로 감소시킬 경우, ③ 30% 감축하였을 경우[정부에서 발표한 『미세먼지 관리 종합대책』(2017.9.26)의 미세먼지 배출량 30% 감축 목표 고려]로 설계하였다.
PM10의 건강영향은 수도권 지역의 호흡기계 사망(전체연령, 65세 이상)과 외래영향을 포함하였고, 입원영향은 분석에 포함하지 않았다. 2015년 기준 PM10, 단기노출, 수도권, 호흡기계, 전체연령을 대상으로 한 호흡기계 기여사망자 수(조기사망자 수)는 747명으로, 그에 따른 피해비용은 7,576억 원에 이르는 것으로 추정되었다. 또한 정책대안1 적용 시조기사망자 수는 2015년 대비 461명 감소하고 그에 따른 경제적 편익은 약 4,675억 원에 이르는 것으로 나타났다. 정책대안2의 경우 해당 수치는 조기사망자 수 317명 감소, 경제적 편익 약 3,215억 원에 달하는 것으로, 정책대안3의 경우 해당 수치는 조기사망자 수 227명 감소, 경제적 편익 약 2,302억 원으로 산출되었다. 동일한 정책대안1, 2, 3 적용 시 PM10농도 감소로 인한 외래 경제적 편익은 2012년 기준 각각 약 2,424억, 1,456억, 1,319억원으로 추정되었다.
한편 PM2.5의 건강영향은 수도권 지역의 호흡기계 사망(전체연령, 65세 이상)과 입원영향(65세 이상)을 포함하였고, 외래영향은 분석에 포함하지 않았다. 2015년 기준 PM2.5 단기노출에 대한 65세 이상 인구집단 대상 호흡기계 조기사망자 수는 1,376명으로, 피해비용은 약 1조 3,955억 원에 이르는 것으로 추정되었다. 또한 정책대안1 적용 시 조기사망자수는 2015년 대비 862명 감소하고 그에 따른 경제적 편익은 약 8,738억 원에 이르는 것으로 나타났다. 정책대안2의 경우 해당 수치는 조기사망자 수 599명 감소, 경제적 편익 약 6,077억 원에 달하는 것으로, 정책대안3의 경우 해당 수치는 조기사망자 수 423명 감소, 경제적 편익 약 4,291억 원으로 산출되었다. 2015년 기준 PM2.5 단기노출에 대한 65세이상 호흡기계 입원 에피소드 건수는 2만 1,721으로, 피해비용은 약 713억 원으로 도출되었다. 동일한 정책대안1, 2, 3 적용 시 PM2.5 농도 감소로 인한 입원영향의 경제적 편익은 각각 약 453억, 315억, 231억 원에 달하는 것으로 추정되었다.
This three-year project aims to assess the long-term effects of air-pollution on human health by employing cohort DB, which is provided by the National Health Insurance Service and to estimate its economic costs in Korea. In particular, the study is designed to formulate an analytical framework of health impact assessment of air-pollution and to strengthen policy-evaluation framework by linking the health end-points such as mortality and morbidity to their unit values.
The first year (2015) of the project focused on assessing the impact on new and repetitive admission of respiratory diseases caused by long-term exposure to air-pollutants such as PM10, NO2, and O3 by employing cohort DB. In addition, the unit values for morbidity were estimated with the cost of illness (COI). The second year (2016) of the project assessed the effect of respiratory disease mortality caused by the short-term exposure and estimated the unit values of mortality, value of statistical life (VSL) which was driven from the willingness-to-pay (WTP) for the decrease of probability due to the improvement of air quality.
The third year (2017) of the study focused on constructing the analytical framework of policy-evaluation by linking the health effects of PM10 concentration on hospital admissions and mortality of respiratory diseases to the estimates of COI and VSL, respectively, in order to estimate its economic costs. Moreover, the health effects of PM2.5 and its damage costs have recently become serious issues in Korean society. Taking into account this, the scope of the integrated assessment has been extended further. Firstly, concentration-response (CR) functions were selected to encompass the health effects of not only PM10 but also PM2.5 on hospital admission and mortality due to respiratory diseases. In addition, the range of health effects was confined to the respiratory diseases yet the exposed populations were differentiated into all-age group and adults over 65 years-old. The differentiation was made to examine the heath effects of the vulnerable population group as well. Lastly, the scope of the morbidity, in addition to the hospital admission, was extended to include out-patient visits for respiratory diseases due to PM10.
The integrated assessment was conducted by estimating the economic costs of health effects in 2015 as a baseline, and air quality (PM10, PM2.5) improvement-policy alternatives were introduced to examine their outcomes. Three air quality improvement-policy alternatives were developed as follows:
(1) WHO Air Quality Guidelines (AQG): Lower the annual mean concentration level for PM10(20㎍/m<sup>3</sup>) and PM2.5(10㎍/m<sup>3</sup>)
(2) WHO AQG Interim target-3 (IT-3): Lower the annual mean concentration level for PM10(30㎍/m<sup>3</sup>) and PM2.5(15㎍/m<sup>3</sup>)
(3) 30% reduction in emissions of particulate matters
The health effects of PM10 took into account deaths of respiratory (all-age group and elderly (≥65)) and the out-patient effects in metropolitan areas, and the respiratory hospital admissions were not taken into account during the analysis. In 2015, the number of people who had short-term exposures to PM10 was up to 747 people with the economic costs of 757.6 billion Won. In the case of policy alternative 1, the number of premature deaths (premature mortality) was expected to decrease by 461 people compared to 2015 with the economic benefits of 467.5 billion Won. As for the second policy alternative, the number of premature deaths was expected to decrease by 317 compared to 2015 with the economic benefits of 321.5 billion Won. For the third policy alternative, the number of premature deaths was expected to decrease by 227 compared to 2015 with the economic benefits of 230.2 billion Won. By applying three policy alternatives, the out-patient economic benefits were estimated at of 242.4, 145.6, and 131.9 billion Won in 2012 respectively as the concentration of PM10 is being reduced.
The assessment of health effects of PM2.5 took into account the mortality of respiratory diseases (all-age group and elderly (≥65)) and the respiratory hospital admissions (elderly (≥65)) in metropolitan areas. However, out-patient effects were not taken into account during the analysis. In 2015, the number of people who had short-term exposure to PM2.5 was up to 1,376 people aged 65 and over with the economic costs of 1.395.5 trillion Won. Moreover, as for the first policy alternative, the under of premature mortality was expected to decrease by 862 people compared to 2015 with the economic benefits of 873.8 billion Won. In the case of the policy alternative 2, the number of premature mortality was expected to decrease by 559 compared to 2015 with the economic benefits of 607.7 billion Won. For the third policy alternative, the number of premature deaths was expected to decrease by 423 by compared to 2015 with the economic benefits of 429.1 billion Won. In 2015, the number of respiratory hospital admission was 21,721 episodes with the economic costs of 71.3 billion Won. By applying three policy alternatives, respiratory hospital admissions economic benefits were estimated at 45.3, 31.5, and 23.1 billion Won in 2012 respectively as the concentration of PM2.5 is being reduced.
