본 연구는 경도인지장애 노인의 인지·신체·정서적 영역을 개선시킬 수 있는 통합 발레 프로그램을 개발하고 그 타당성을 검증하는데 목적이 있다. 치매 발병 고위험군인 경도인지장애는 조기에 진단하면 치매를 예방할 수 있는 단계로 알려져 이를 위한 다양한 프로그램들이 이루어지고 있다. 이러한 프로그램들은 비약물 치료라는 공통점을 가지고 있으나 단일 영역에 치중할 수밖에 없는 한계가 있어 경도인지장애 노인들에게 나타나는 증상들을 통합적으로 다루지 못하는 단점이 있다. 따라서 경도인지장애 노인의 치매 예방 효과를 증진시키기 위해서는 통합 프로그램의 개발이 필요하고 신체적, 시각적, 연극적, 언어적, 음악적 요소 등을 포함하는 발레를 활용하여 경도인지장애 노인에게 적용하였다.
본 연구의 통합 발레 프로그램은 ‘바가노바 히스토리컬 댄스(Vaganova Historical Dance)’에 기반을 두고 프로그램의 성격과 목적 및 목표를 설정하였다. ‘바가노바 히스토리컬 댄스’는 러시아 바가노바 아카데미의 교과과정 중 하나로 중세부터 탄생된 궁정발레를 재구성하여 메소드화 한 것이며, 이를 통해 발레의 기원과 움직임의 기본 원리에 대해 이해하게 되는 학습과정이다. 본 연구의 대상은 경도인지장애 노인으로 참여자의 신체적 특성을 반영하여 바가노바 히스토리컬 댄스를 프로그램 개발 근거로 차용하였다. 개발한 통합 발레 프로그램의 타당성을 검증하기 위해 서울시 소재 치매안심센터에서 경도인지장애 노인 13명을 대상으로 주 2회, 50분, 16회기 실시하였다. 선정된 실험집단을 대상으로 프로그램 적용에 따른 인지적, 신체적, 정서적 영역에 변화가 있는지 살펴보기 위해 반복측정 분산분석과 대응 표본을 실시하였고, 타당화 검증 결과는 다음과 같다.
첫째, 통합 발레 프로그램에 참여한 경도인지장애 노인의 인지적 영역 중 언어기억 검사에서 단어목록기억과 단어목록재인 검사가 프로그램 중재 시기에 따라 유의미하게 개선되었고, 보스턴이름대기 검사는 프로그램 중재 후 통계적으로 유의미한 개선이 있음을 확인하였다. 단어목록회상의 측정값은 통계적으로 유의하지 않았다. 또한, 공간기억 검사인 TMT-A형 검사는 중재 시기에 따라 통계적으로 유의한 개선이 있었으며 동작기억 검사 또한 프로그램 중재 후 유의미한 변화를 보였다. 둘째, 통합 발레 프로그램에 참여한 경도인지장애 노인의 신체적 영역 중 TUG 검사는 통계적으로 유의하지는 않았으나 프로그램 후반에 갈수록 점진적으로 하락하는 것을 확인하였다. BBS 검사는 프로그램 중재 시기에 따라 통계적으로 유의하게 개선됨을 확인할 수 있었다. 셋째, 통합 발레 프로그램에 참여한 경도인지장애 노인의 정서적 영역 중 우울감은 통계적으로 유의하게 개선되었으나 자존감은 통계적으로 유의한 변화가 없었다.
이상과 같이, 본 연구에서 제시한 경도인지장애 노인의 인지·신체·정서 개선을 위한 통합 발레 프로그램은 다양한 측면에서 실효적인 중재 프로그램으로서 가능성이 있음을 확인할 수 있었다. 본 연구의 결과는 경도인지장애 노인의 인지적, 신체적, 정서적 기능의 개선뿐만 아니라 치매 예방을 위한 대안적 치료 활동으로 발레 프로그램을 확장시킬 가능성이 있다는 점에서 의미가 있다. 또한 본 연구에서 개발된 프로그램은 고령자의 실천적인 신체 활동으로 활용 가능하며, 추후 관련 연구를 지속적으로 실행한다면 고령자의 건강한 삶 영위에도 긍정적인 효과를 기대할 것으로 사료된다.
The research aims to develop and validate an integrative ballet program to enhance the cognitive, physical, and emotional aspects of the elderly with a mild cognitive impairment. As dementia is known to be inhibited if the cognitive impairment is diagnosed early, a wide range of programs were developed and proposed for the elderly. Previously presented programs took advantage of non-drug treatments. Still, there exists a limit that they cannot treat various symptoms together as they are specialized at a single aspect of the impairment. To enhance the dementia prevention effect, an integrative approach is significantly necessitated. This has become a motivation of this study. We developed ballet that encompasses physical, visual, linguistic, musical components to develop a program for the elderly with a mild cognitive impairment.
The proposed integrative program is based on ‘Vaganova Historical Dance’, and the content is developed along with participants’ physical characteristics. ‘Vaganova Historical Dance’ is a course in the Russian Vaganova Method, and it has a close relationship with the origin of ballet. The Historical Dance originated from court ballet, and it helps the learner to easily understand the basic principles of movements and the origin of ballet. Considering the ages of the participants, we employed the Historical Dance as a basis of the program, including the basic classic ballet. To validate the program's effectiveness, it is applied to 13elderly patients with a mild cognitive impairment in Seoul Dementia Center. The program was conducted twice a week, 50 minutes per program and, 16 times in total. To scrutinize the difference in the physical, cognitive, and emotional aspects of the patient, Repeated Measures ANOVA and paired sample t-test were taken, and the result is presented below.
First and foremost, in a cognitive aspect of the participants, significant improvements were identified in the Word List Memory and Word List Recognition test during the program, and statistically significant enhancement in the Boston Naming Test was also found after the program. However, there were no statistically significant changes seen in the Word List Recall test. Additionally, statistically significant enhancement during the program was found in a TMT-A test, which is a spatial memory test. A significant improvement in motion memory test was also found after the program. Second, a gradual decrease in a TUG test was identified in a physical aspect of the participants as the program progressed though it was not statistically significant. For the BBS test, statistically significant enhancement was found during the program. Third, in an emotional aspect of the patient, there has been a significant enhancement in alleviating depression, but no significant change was found in self-esteem.
Through the described literature above, we figured out a possibility of the integrative ballet program as an effective treatment to enhance patients' cognitive, physical, and emotional aspects with a mild cognitive impairment. The result suggests a further possibility of extending the ballet program as an alternative treatment program for the prevention of dementia. The program developed in this study also can be used as a practical bodily movement program for the elderly; thus, it can contribute to the overall healthy life for elderly people.
