With the outbreak of Covid-19 in 2020, the world has experienced a deepening of concern on the threat of illness and death, which suddenly became more imminent. While resources on physical caregiving are abundant, those on mental and emotional caregiving are scarce, especially Buddhist resources on end-of-life care. This is in stark contrast to Catholic and Protestant volunteer practices: professional programs that facilitate communication for the recipient are managed extensively. The Buddhist community would be encouraged to come up with its own set of programs that are based on Buddhist principles and provide way for psychological care. Although this is a field in need of a body of research among modern Buddhist societies, it is not difficult to find related material in the two thousand years of word Buddhist history. Brahmajala Sutra introduces 48 minor precepts, the ninth of which is “Look after the sick.” It beckons us to regard the sick as fields of blessings – the same way that we would regard the Buddha – and emphasizes the need to nurse and nurture them back to health. However, it is difficult to find a guideline for modern hospice volunteers to follow when they meet the sick. Although monastics and lay Buddhists may well have both theoretical and practical training, and the mindset to provide appropriate care for the unwell in and outside of their family relations, many of them are at a loss as to how to put those principles into practice.
When hospice volunteers fail to provide care that puts the patient first and offer support that is self-centered, the difference in their background and cultural upbringing reduces the recipients’ satisfaction levels. The biggest obstacles are the volunteers’ anxiousness at meeting palliative patients face to face and the fear of their own deaths. Hospice volunteers and medical staff alike continue to experience emotional suffering.
This provides ample reason to champion a need for a Compassionate-care program for hospice volunteers so that they can better provide suitable hospice care and develop the capability for self-examination. From a Buddhist perspective, this can come from the Four Noble Truths and the Noble Eightfold Path. Anticipating a model of end-of-life care that incorporate communication to accommodate patients’ personal afflictions, meditation on the subject of death and the resulting acceptance of death, this study will look at the theoretical aspect of a compassionate-care program for hospice volunteers that can be utilized in monasteries, social welfare centers, hospitals, schools and homes. Based on the Buddhist Four Noble Truths and the Noble Eightfold Path, the program’s purpose can be summarized into the following:
First, to provide the grounds of a compassionate-care program for hospice volunteers from Buddhist scriptures so that they can be rid of the anxieties of nursing and the fear of death:
Second, to provide a customized program for dying patients, to apply a meditation technique to help them physically, emotionally and spiritually:
Third, to provide a meditation program for lay Buddhists and others in the medical field to prevent exhaustion and promote compassionate-care. This will enable them to avoid social depression and promote positive self-image.
Thus, a compassionate-care program for hospice volunteers with the above purpose can serve as a stepping stone for persons in the hospice and palliative fields to walk the path of the Bodhisattva, ultimately enabling them to attain spiritual enlightenment and to save all sentient beings.
2020년 코로나로 인하여 지구촌은 급작스럽게 다가온 질병과 죽음에 대한 고뇌가 깊어졌다. 그러나 신체적 돌봄에 필요한 자원들만이 풍성하고 정서적, 정신적인 분야로서 특히 임종 돌봄에 관한 불교 관련 자원은 극히 드물다. 그에 반해 일반 기성종교인 가톨릭과 개신교 봉사자들의 임종 관리는 매우 철저하게 이뤄진다. 임종시에 대상자의 관계에 대한 소통의 접근 방법을 돕는 프로그램이 개발되어 매우 전문적으로 관리되고 있다. 그렇다고 하여 타 종교의 프로그램을 그대로 사용하는 것도 문제가 있으며, 불교계에서는 불교적 특성에 바탕 한 정신적 돌봄과 그 구체적 실천행에 대한 프로그램이 마련되어야 한다. 이에 대해서 불교계에는 축적된 연구가 미비하지만, 이미 2천여 년 동안 지속된 불교계에서 그 정신적 돌봄의 근거자료를 마련하는 것은 어렵지 않다.
특히,『범망경』 48경계 중 제9,“병든 이를 보살펴라(不看病戒)”의 내용을 살펴보면 병든 이를 부처님과 동등한 福田으로 간주하며 병든 이를 보거든 간호하고 공양하여 낫게 해야 함을 강조하고 있다. 그러나 현시점에서 구체적으로 병든 이를 대면할 때 호스피스 자원봉사자가 무엇을 어떻게 해야 하는지에 대한 지침은 발견하기 어렵다. 스님 및 불자들이 구체적인 이론과 실습 교육을 받았음에도 병든 가족뿐만 아니라, 주변에 질병으로 고통받는 사람들을 간호하는 데 있어 근본적인 돌봄의 정신은 갖추어져 있으나 구체적 실천에 있어서 어려움을 호소하고 있는 것이 현실이다.
또한 환자 중심이 아닌 호스피스 자원봉사자 자신의 경험적 토대로 자기중심적인 돌봄을 하게 되었을 때 환자의 만족도는 질적으로 떨어진다. 그들은 각각 자라온 배경과 문화가 다르기 때문이다. 돌봄에 있어 가장 큰 걸림돌은 환자들을 대면하는 데 있어 불안감과 각자의 죽어감에 대한 두려움이 가장 크다. 자원봉사자뿐만 아니라 임상 현장에 있는 많은 사람이 감정적 고통을 호소하고 있다.
이에 임상 현장 안에서 호스피스 자원봉사자의 더욱더 적절한 돌봄 제공과 자기를 성찰하는 힘을 길러 수행에 도움이 될 수 있는 호스피스 자원봉사자를 위한 자비 돌봄 프로그램이 절실하게 필요하다고 생각한다. 구체적으로는 불교의 대표적인 교학이며, 실천수행으로 네 가지 성스러운 진리 사성제와 여덟 가지 바른 깨달음의 길 팔정도가 있다. 이를 통하여 환자의 번뇌에 따른 대화 기법과 죽음에 대한 명상을 통하여 벗어남이라고 하는 납득의 길로 이끄는 구체적 방안을 통해 임종 돌봄의 방법이 마련될 것이라고 기대한다. 이와 같은 구체적 실천행의 제시에 따라 사찰 및 복지시설, 병원, 학교 및 가정에서 활용할 수 있는 사성제와 팔정도를 바탕을 둔 이론적인 호스피스 자원봉사자를 위한 자기 돌봄 프로그램을 연구하고자 한다. 이러한 배경하에 본 호스피스 자원봉사자를 위한 자비 돌봄 프로그램의 목적과 취지는 다음과 같이 크게 세 가지로 정리할 수 있다.
첫째, 간병에 대한 불안 및 죽음에 대한 두려움에서 벗어나 효과적으로 적응할 수 있는 호스피스 자원봉사자를 위한 자비 돌봄 프로그램의 근거를 경전에서 제시한다.
둘째, 질병으로 생을 마감하는 사람들의 입장에 맞게 맞춤형 실천 프로그램을 제시하며, 그들을 신체적, 정서적, 영적으로 돕기 위하여 사성제·팔정도 등에 의한 명상법의 구체적인 적용 및 호스피스 자원봉사자를 위한 자비 돌봄 프로그램을 제공한다.
셋째, 임상과 관련된 직종의 불자들이나 일반인들을 위한 소진 예방과 마음 돌봄을 할 수 있는 명상 프로그램을 제공한다. 그에 따라 사회적 우울감에서 벗어나 스스로를 돌보고 관리할 수 있는 긍정적 자기 인식을 할 수 있도록 돕는다.
위와 같은 호스피스 자원봉사자를 위한 자비 돌봄 프로그램의 목적과 취지로 생의 마감과 죽음을 돌보는 관계자들에게 보살행을 실천할 수 있도록 돕고 상구보리 하화중생을 몸소 실천하고 수행의 깊이를 다져가는 계기가 될 수 있다.
