Psittacosis is a systemic infection caused by inhalation of Chlamydia psittaci. The source is usually an infected bird, which may be asymptomatic. Humans can become infected with Chlamydia psittaci through direct contact or inhalation of aerosols from dried feces, feather dust, or respiratory secretions of infected birds. Individuals with recreational exposure to birds like bird fanciers are at greatest risk of infection. A healthy 49-year-old female was admitted with fever, headache, myalgia, dry cough for 7 days in March, 2015. Her husband also presented with fever, headache, dry cough 6 days ago with showing an opacity of left lower field in chest X-ray. Two months ago, they had purchased a parrot and often released outside the cage. On her examination, the body temperature was 38.8°C and others were not remarkable. The oxygen saturation was 96% on room air. Chest examination revealed coarse rale over the left side. Chest X-ray showed consolidation of left lower lobe zone with left minimal pleural effusion. A complete blood cell count and blood chemistry revealed a white blood cell count of 7,320/mm3, alanine aminotransferase of 72 IU/L aspartate aminotransferase of 121 IU/L, alkaline phosphatase of 1,914 IU/L, and C-reactive protein of 13.76 mg/dL. Urine was negative for pneumococcal antigen. Nasopharyngeal aspirate was negative for influenza, Mycoplasma pneumoniae, Chlamydophila pneumoniae by PCR. Tracheal aspirate was negative for C. psittaci by PCR, too. However, microimmunofluorescence serology of paired sera (10 days apart from baseline serum) reported by Korea centers for disease control and prevention showed a rise to 1: 64 of IgM against C. psittaci, while her husband’s was negative. They were treated with clarithromycin for 7 days. This is the first psittacosis case occurred in a family.
