본 연구는 NSAIDs 약물을 대상으로 초음파의 매개변수와 약물의 물리화학적 특성에 따른 경피투과 효과를 비교하고 알아보기 위하여 다음과 같은 실험을 실시하였다.
먼저 NSAIDs 약물인 aspirin, indomethacin, mefenamic acid, diclofenac sodium, ketoprofen, meloxicam을 대상으로 약물의 종류에 따라 초음파 적용방식이 경피투과에 미치는 영향을 알아보기 위해 약물 종류별로 겔을 제조하고, skin permeation tester의 franz diffusion cell과 초음파 변환기를 실험의 목적에 맞게 제작하여 hairless mouse의 피부로 경피투과 실험을 실시하고 약물별 투과를 HPLC로 분석하였다. 이 연구 결과를 요약하면 다음과 같다.
약물별로 초음파를 적용하지 않은 대조군에 비해 초음파 적용군들에서 경피투과가 높게 나타났으며, 특히, 지속초음파에서 경피투과가 현저히 증가하였다. 초음파를 적용하지 않은 대조군 간 비교에서는 측정시간대별 경피투과는 매우 낮았으며, 그 중에서는 meloxicam, indomethacin, ketoprofen 순으로 투과가 높았다. 지속초음파 적용군 간의 비교에서는 ketoprofen이 가장 높았고, 그 다음이 indomethacin과 meloxicam이었다. 맥동초음파 적용군 간의 비교에서는 indomethacin에서 가장 높게 나타났고, meloxicam과 ketoprofen이 비슷한 투과가 나타났다.
Meloxicam을 이용하여 겔을 제조하고 초음파 주파수 및 강도와 meloxicam의 물리화학적 특성에 따른 경피투과를 분석하였다. 이 연구 결과를 요약하면 다음과 같다.
초음파 주파수에 따른 경피투과는 1.0 ㎒에서 더 높았으며, 강도가 증가할수록 더 높게 나타났다. 초음파의 순환주기에 따른 비교에서 지속초음파 1.0 W/㎠와 맥동초음파 3.0 W/㎠에서 비슷한 투과가 나타나 맥동초음파 적용 시에는 고강도를 사용해야 효과적인 것으로 나타났다. 물리화학적 특성에 따른 비교에서 경피투과 촉진제 중 DMI에서 투과가 가장 높게 나타났으며, meloxicam의 함유량이 높을수록 증가하였다. 또한 겔제가 크림제보다 투과가 높게 나타났으며, 비결정형이 결정형보다 더 높게 나타났다. Receptor cell의 온도가 높을수록 투과는 증가하였으며, 겔의 점성도는 높을수록 낮게 나타났다.
결론적으로 본 실험에 사용된 NSAIDs 약물 모두에서 초음파 매개변수가 경피투과에 영향을 미치는 것으로 나타났으며, 지금까지 소염진통을 위한 목적의 음파영동 약물로 가장 많이 사용해오던 aspirin 등 보다는 ketoprofen, indomethacin, meloxicam이 초음파에 의한 경피투과가 높은 것으로 나타났다. 특히, meloxicam은 함유량이 높고, 경피투과 촉진제로 DMI를 사용하며, 점성도가 낮은 비결정형 겔의 형태로 제제화하여 초음파를 적용할 때 경피투과 촉진효과가 가장 큰 것으로 나타났다. 또한 고강도 맥동초음파 적용과 저강도 지속초음파의 적용이 유사한 경피투과를 보여, 지속초음파를 적용할 수 없는 경우에 맥동초음파를 적용할 수 있어 물리치료 분야에서 근 골격계 염증성 병변에 대한 음파영동 약물로 활용성이 매우 높을 것으로 생각된다.
This study conducted the following experiment to examine and compare transdermal permeation effects according to parameters of ultrasound and physiochemical characteristics of drug with NSAIDs drugs.
First, it made gel of each drug to know the influence of parameters of ultrasound on transdermal permeation with NSAIDs drugs such as aspirin, indomethacin, mefenamic acid, diclofenac sodium, ketoprofen and meloxicam, manufactured Franz diffusion cell of skin permeation tester and ultrasound transducer in accordance with the purposes of experiment, conducted transdermal permeation experiment with skin of hairless mouse and analyzed permeation with HPLC.
The results of this study can be summarized as follows.
It was found that ultrasound group showed higher permeation than non-ultrasound group and in particular, transdermal permeation was remarkably increased in continuous ultrasound mode. In comparison of non-ultrasound groups, permeation at each measuring time was very low and it was highest in meloxicam, followed by indomethacin and ketoprofen. In comparison of continuous ultrasound mode groups, ketoprofen was highest, followed by indomethacin and meloxicam. In comparison of pulsed mode groups, indomethacin was highest and meloxicam and ketoprofen showed similar permeation.
Permeation by ultrasound among these experimental drugs was relatively higher and it was involved in COX-2 inhibition unlike other drugs. Recently use of oral agents has been rapidly increased, but it was not generalized to transdermal agent and this study selected meloxicam that transdermal permeation research using ultrasound was not performed and conducted transdermal permeation experiment. It made gel first and analyzed permeation depending on frequency and intensity of ultrasound and permeation depending on physiochemical variables of meloxicam with the same experimental procedures as the above experiment.
The results of this study can be summarized as follows.
Transdermal permeation by ultrasound frequency was higher in 1.0 ㎒ and it was higher as intensity increased. In comparison by parameters of ultrasound, there was similar permeation in 1.0 W/㎠ of continuous mode and 3.0 W/㎠ of pulsed mode and it was effective to high intensity for using pulsed mode. In comparison by physiochemical variables, permeation in DMI of transdermal permeation accelerators was highest and it increased as content of meloxicam was higher. In addition, gel showed higher permeation than cream and amorphism was higher than crystal. As the temperature of receptor cell was higher, permeation was increased and as viscosity of gel was higher, permeation was lower.
In conclusion, it was found that duty cycle of ultrasound affected transdermal permeation in all NSAIDs drugs used in this experiment and transdermal permeation was higher in ketoprfen, indomethacin and meloxicam than in aspirin used frequently as phonophoresis for anti-inflammatory effects. In particular, when using ultrasound in high content of meloxicam, use of DMI as transdermal permeation accelerator and making amorphism having low viscosity to gel, transdermal permeation effect was greated and it is thought that it will have a high availability as phonophoresis drug of muscularskeletal inflammatory disease in the area of physical therapy.
