균형 정질액과 알부민을 조합한 수액을 이용한 출혈성 쇼크의 치료 개선에 대한 연구 : 출혈성 쇼크를 유발한 돼지에서 생체전기저항 분석장치를 이용한 임피던스와 혈역학 변화 분석 = Improvement of hypovolemic shock treatment using a combination of balanced crystalloid and albumin

Introduction
The main cause of hypovolemic shock is hemorrhage. In clinical practice, it usually appears in the form of traumatic hemorrhagic shock. In hypovolemic shock, infusion of blood products is the most ideal, but the reality is that the initial resuscitation treatment consists of intravenous fluids. There have been many studies and controversies about what is the best: crystalloid or colloid, and how much supplementation is most effective in hypovolemic shock patients. In the resuscitation of hypovolemic shock, it is important to select which fluid to infuse and how much fluid to infuse and to evaluate the adequacy of resuscitation. There have been no studies on the analysis of impedance change using bioelectrical impedance analysis (BIA) and the change in impedance according to resuscitation fluid in a hypovolemic shock model. In this study, the change in impedance for each type of resuscitation fluid in hypovolemic shock-induced swine models was analyzed by BIA. The resuscitation fluid consists of 1) Balanced crystalloid 2) Balanced crystalloid + 5% Dextrose Water 3) Balanced crystalloid + 20% Albumin. We tried to identify the ideal treatment fluid combination by analyzing the impedance and hemodynamic changes for each fluid.
Methods
A hypovolemic shock model was implemented using 15 female three-way crossbreds (YorkshireⅹLandraceⅹDuroc). The 15 pigs were divided into three groups of 5 pigs each according to the combination of resuscitation fluids. A 7-french three-lumen central venous catheter was inserted into the internal jugular vein, and an arterial line was inserted into the left femoral artery. A 5-french angio-catheter for controlled hemorrhage was inserted right femoral artery. Pulse rate (PR), Mean arterial pressure (MAP), Stroke volume (SV), and Stroke volume variation (SVV) were measured using a minimal invasive hemodynamic monitoring device (Flo Trac™, Edwards Lifesciences, Irvine, CA, USA). Bioelectrical impedance was measured using a segmental multifrequency bioelectrical impedance analyzer, BWA 2.0 (Inbody co., Seoul, Korea). Eight-point electrodes were used, four on each foot and four on each limb. A total of 61 impedance measurements were made for each pig at 6 different frequencies (1kHz, 5kHz, 50kHz, 250kHz, 500kHz, 1000kHz) in 5 segments (right arm, left arm, trunk, right leg, left leg) of the pig. The experiment consists of 3 periods; 1) controlled hemorrhage (a total of 1L bleeding, 60 minutes), 2) resuscitation phase 1 (a total of 1L fluid infusion, 60 minutes), and 3) resuscitation phase 2 (a total of 1L fluid infusion, 60 minutes). The controlled hemorrhage was implemented by connecting a three-way cock to a catheter inserted right femoral artery, and bleeding 100ml every 6 minutes, a total of 10 times for 60 minutes, 1000ml. Resuscitation phase 1 and 2 proceeded consecutively. A total of 2L fluid was infused into each pig for resuscitation.
Results
During controlled hemorrhage, the impedance increased at both low frequency (5k Hz) and high frequency (1M Hz) and decreased at both frequencies after fluid resuscitation. In the time-impedance-impedance ratio 3-dimensional graph, during the common controlled hemorrhage (0 to 60 minutes), the graph showed a tendency to change to a straight line which means the changes of extracellular fluid due to hemorrhage. In the group infused with Balanced crystalloid 2L, the graph showed a tendency to change to a straight line which means the changes of extracellular fluid due to the infusion of fluids. In the group infused with Balanced crystalloid 1L + 5% Dextrose Water 1L and Balanced crystalloid 1.6L + 20% Albumin 400ml, the graph showed a tendency to change to a curved pattern which means the changes of intracellular fluid. The change over time of 1M impedance which reflects total body water, showed an increase in all three groups during the controlled hemorrhage and showed a decrease from the infusion of the fluid to the end of the experiment. The time to recover the 1M impedance value at the beginning of the experiment was 146 minutes for Balanced crystalloid 2L, 128 minutes for Balanced crystalloid 1L + 5% Dextrose Water 1L, and 135 minutes for Balanced crystalloid 1.6L + 20% Albumin 400ml.
In all three groups, PR and SVV showed an increasing trend from the start of the experiment to the end of controlled hemorrhage, while SV and MAP showed a decreasing trend. In addition, from the start to the end of infusion, PR and SVV tended to decrease, while SV and MAP tended to increase. The increase in MAP, decrease in PR, increase in SV, and decrease in SVV due to fluid infusion were clearly observed in the Balanced crystalloid + 20% Albumin fluid combination. It was observed only until about 1.2L of fluid was infused about 70 minutes after the fluid infusion. After 70 minutes, no increase in MAP and SV was observed even after additional fluid infusion. MAP and SV at * point in Figure 6-3 (135 minutes) are 50.2mmHg and 67.6ml/beat, respectively. The average of MAP and SV from this point to the end of the experiment are 50.4mmHg and 68.4ml/beat, respectively. MAP and SV at the end of the experiment are 49.6mmHg and 67.2ml/beat, respectively, and it showed a plateau from 135 minutes to the end of the experiment.
1M impedance, which reflects total body water, increased from the start of the experiment to the end of controlled hemorrhage and then decreased from the infusion of fluid to the end of fluid infusion in all three groups. There was no further increase in MAP and SV from 70 minutes after the fluid resuscitation. It was observed that the 1M impedance reflecting the total amount of water increased after the start of controlled hemorrhage, and then the point of recovery as much as the starting point of the experiment coincided with the 70 minutes after the fluid resuscitation.
As a result of comparing MAP and SV at the start of the experiment, after the end of controlled hemorrhage, at the start of the resuscitation period 2, at the time to recover the 1M impedance value at the beginning of the experiment and after the end of fluid infusion for each resuscitation fluid group, only the Balanced crystalloid + 20% Albumin fluid combination showed a significant recovery of MAP and SV to the level of MAP and SV before controlled hemorrhage (p=0.164, p=0.099). There were no significant differences among the three groups in MAP and SV at the point of recovery of 1M impedance as the initial value and at the point of the end of fluid infusion (p=0.757, p=0.284, p=0.707 / p=0.648, p=0.527, p=0.906).
Therefore, in combination with Balanced crystalloid and 20% Albumin, the effect of increasing MAP and SV is the same when the infusion of fluid is implemented until the point of recovery of 1M impedance as the initial value, not the point of the end of fluid infusion.
Conclusion
When resuscitation using fluids was implemented in hypovolemic shock-induced pigs, it was found that there were movements of extracellular and intracellular water by measuring impedance by BIA. 1M impedance, which reflects total body water, increased with controlled hemorrhage and decreased with fluid infusion. When 1M impedance and hemodynamic parameters were analyzed simultaneously, in the balanced crystalloid + 20% albumin group, there was a point that MAP and SV did not increase even after fluid infusion. In addition, there was another point the 1M impedance reflecting total body water increased after controlled hemorrhage then decreased after fluid infusion and recovered the initial value. The point of recovery of 1M impedance to the initial value coincided with the point that MAP and SV did not increase even after fluid infusion. In addition, among Balanced crystalloid, Balanced crystalloid + 5% Dextrose Water, and Balanced crystalloid + 20% Albumin, only the Balanced crystalloid + 20% Albumin showed significant recovery of MAP and SV at the end of the infusion as much as the start of the experiment. In the Balanced crystalloid + 20% Albumin group, the effect of infused fluids that recover MAP and SV until the time of recovering the initial value of 1M impedance was the same as the time of the end of fluid infusion.
In emergency department, it takes a relatively long time to infuse blood products and it is difficult to implement damage control resuscitation immediately. The results of this study suggest that the clinical use of Balanced crystalloid mixed with Albumin maybe more helpful in securing hemodynamic stability than single Balanced crystalloid infusion therapy. In this study, we used a total of 2L fluid mixed with Balanced crystalloid 800ml and 20% Albumin 200ml. However, for the optimal fluid combination ratio, further follow-up studies are necessary. Since the recovery of vital signs and hemodynamic indicators such as MAP and SV coincides with the change and recovery of 1M impedance reflecting total body water, the possibility of fluid resuscitation using BIA is suggested for critically ill patients.
서론
저혈량성 쇼크의 주된 원인은 출혈로, 임상에서는 주로 외상성 출혈 쇼크의 형태로 나타난다. 저혈량성 쇼크에서는 혈액제제의 투여가 가장 이상적이나 초기 소생 치료는 수액으로 이루어지는 것이 현실이다. 쇼크 환자에서 정질액(crystalloid)와 교질액(colloid) 중 어떤 수액으로 얼마나 보충하는 것이 가장 효과가 있는지에 대해서는 많은 연구와 논란이 있어 왔다. 저혈량성 쇼크의 소생 치료는 어떤 수액을 선택하여 얼마나 줄 것인지와 소생의 적절성 평가가 중요하다. 저혈량성 쇼크 모델에서 생체전기저항 분석을 이용한 임피던스 변화의 분석과 소생 수액에 따른 임피던스 변화에 대한 연구 역시 지금까지 없었다. 본 연구에서는 저혈량성 쇼크가 유발된 돼지에서 소생 수액의 종류별로 임피던스의 변화를 생체전기저항 분석장치를 통해 측정하였다. 소생 수액을 1) 균형 정질액(Balanced crystalloid) 2) 균형 정질액 + 5% 포도당 수액(Dextrose Water) 3) 균형 정질액 + 20% 알부민(Albumin) 세 군으로 나누었다. 각 수액 별 임피던스 변화 및 혈역학적 지표 분석을 통해 이상적인 수액치료 조합을 알아보고자 하였다.
연구방법
15마리의 암컷 삼원교잡돈(YorkshireⅹLandraceⅹDuroc)을 이용하여 저혈량성 쇼크 모델을 구현하였다. 15마리의 돼지는 소생 수액 조합에 따라 5마리씩 3개 군으로 나뉘었다. 7프렌치의 3관 중심정맥관을 내경정맥에 삽입하였고, 동맥관은 좌측 대퇴동맥에 삽입하였다. 방혈을 위한 5프렌치 안지오 카테터는 우측 대퇴동맥에 삽입하였다. 최소 침습 혈역학 모니터링 기기(Flo Trac™, Edwards Lifesciences, Irvine, CA, USA)를 이용하여 맥박수(Pulse rate, PR), 평균동맥압(Mean arterial pressure, MAP), 일회박출량(Stroke volume, SV) 및 일회박출량변이(Stroke volume variation, SVV)를 측정하였다. 생체전기 임피던스(Bioelectrical impedance)는 분절형 다주파 생체전기저항 분석기인 BWA 2.0(Inbody co., Seoul, Korea)으로 측정하였다. 8점 전극이 사용되었고 4개 전극은 각 발에 4개는 각 사지에 부착되었다. 돼지의 5가지 분절(우측 팔, 좌측 팔, 몸통, 우측 다리, 좌측 다리)에서 6개의 다른 주파수(1kHz, 5kHz, 50kHz, 250kHz, 500kHz, 1000kHz)로 각 돼지마다 총 61번의 임피던스가 측정되었다. 실험은 크게 3개 구간; 1) 방혈(총 1L 방혈, 60분), 2) 소생 1기(1L 수액 주입, 60분) 3) 소생 2기(1L 수액 주입, 60분)으로 나뉜다. 방혈은 우측 대퇴동맥에 삽입된 카테터에 삼방 콕크(3-way cock)를 연결하여 매 6분마다 100ml씩, 총 10회, 60분간, 1000ml를 시행하였다. 소생 1기와 2기는 연속적으로 진행되었다. 각 동물마다 소생을 위해 총 2L의 수액이 투여되었다.
결과
방혈을 진행하는 동안, 저주파(5k Hz) 및 고주파(1M Hz) 모두에서 임피던스가 증가하였으며 수액 보충을 시행한 뒤 모든 주파수에서 임피던스가 감소하였다. 시간-임피던스-임피던스 비율의 3차원 그래프에서 0분-60분까지의 공통된 방혈 구간동안 직선형태로 변화하며 이는 방혈로 인한 외수분의 변화를 의미한다. 균형 정질액2L를 주입한 군에서는 방혈 후 수액 주입 시 그래프가 직선으로 변화하는 양상을 보였고 이는 수액 주입으로 인한 외수분의 변화를 의미한다. 균형 정질액 1L + 5% 포도당 수액 1L 및 균형 정질액 1.6L + 20% 알부민 400ml를 주입한 군에서는 시간-임피던스-임피던스비율 3차원 그래프에서 방혈 후 수액 주입 시 그래프가 직선이 아닌 곡선형으로 변화하는 양상을 보였고 이는 내수분의 변화가 있음을 의미한다. 총체수분을 반영하는 1M 임피던스의 시간에 따른 변화는 방혈구간동안 세 군에서 모두 1M 임피던스가 증가하였고, 수액주입부터 실험 종료까지 1M 임피던스는 지속적으로 감소하였다. 실험 시작시의 1M 임피던스값이 회복되는 시간은 균형정질액 146분, 균형정질액 + 5% 포도당 수액 128분, 균형정질액 + 20% 알부민 135분 이었다.
세 수액에서 공통적으로 실험시작 후 방혈종료까지 맥박수 및 일회박출량변이는 증가하는 추세를 보였고, 일회박출량과 평균동맥압은 감소하는 추세를 보였다. 또한 수액주입부터 수액주입 종료까지 맥박수와 일회박출량변이는 감소하는 경향을 보였고, 일회박출량과 평균동맥압은 증가하는 경향을 보였다. 수액 주입으로 인한 평균동맥압 증가, 맥박수 감소, 일회박출량의 증가, 일회박출량변이의 감소효과는 균형 정질액 + 20% 알부민 수액 조합에서 명확하게 관찰되었는데, 수액 주입 후 약 70분까지 수액이 약 1.2L 주입될 때 까지만 관찰되었고, 70분 이후에는 수액 추가 주입에도 평균동맥압 및 일회박출량의 증가가 관찰되지 않았다. 그림 6-3에서는 평균동맥압 및 일회박출량의 상승이 수액 주입에도 더 진행되지 않는 지점과 총체수분을 반영하는 1M 임피던스가 방혈 이후 증가하였다가 수액주입 후 감소하면서 초기값을 회복하는 시점이(*로 표시) 일치하는 것을 확인할 수 있다. 6-3의 * 시점(135분)의 평균동맥압과 일회박출량은 각각50.2mmHg, 67.6ml/회였고, 이 시점부터 실험 종료까지의 평균동맥압과 일회박출량의 평균은 각각 50.4mmHg, 68.4ml/회였으며 실험 종료 시점의 평균동맥압과 일회박출량은 각각 49.6mmHg, 67.2ml/회 로 135분부터 실험 종료시점까지 정체된 상태(plateau)를 보였다.
총체수분을 반영하는 1M 임피던스는 세 군에서 동일하게 실험 시작 후 방혈종료까지 증가하였다가, 수액 주입부터 수액주입 종료까지 감소하는 양상을 보였다. 수액 주입 70분 시점부터 평균동맥압 및 일회박출량이 더 증가하지 않았는데, 방혈 시작 이후 증가하였던 1M 임피던스가 실험 시작 지점만큼 회복되는 시점과 수액주입 70분 시점이 일치하는 것이 관찰되었다.
각 소생 수액 군별로 실험시작, 방혈 종료, 소생 2기 시작, 1M 임피던스의 초기값 회복시점 및 수액주입 종료 후의 평균동맥압 및 일회박출량을 비교한 결과, 균형 정질액 + 20% 알부민 수액 조합에서만 방혈 전 평균동맥압 수준의 혈압 회복 및 방혈 전 일회박출량 수준의 회복을 보였다(p=0.164, p=0.099). 1M 임피던스의 초기값 회복 시점과 수액주입 종료 시점의 평균동맥압 및 일회박출량은 세 군에서 모두 통계적으로 유의한 차이가 없었다(p=0.757, p=0.284, p=0.707 / p=0.648, p=0.527, p=0.906).
따라서 균형 정질액 + 20% 알부민 조합에서 수액주입 종료 시점이 아닌 1M 임피던스의 초기값 회복시점까지 수액주입을 시행하여도 평균동맥압 및 일회박출량 증가 효과는 동일하다.
결론
돼지에서 유발된 저혈량성 쇼크에서 수액을 통한 소생술을 시행하였을 때 생체전기저항 분석을 통한 임피던스 측정으로 세포외수분 및 세포내수분 이동이 있음을 알 수 있었다. 총체수분을 반영하는 1M 임피던스는 방혈에 따라 증가하고 수액 주입에 따라 감소하는 결과를 보였다. 1M 임피던스와 혈역학 지표를 동시에 분석했을 때, 균형 정질액 + 20% 알부민 군에서 평균동맥압 및 일회박출량의 상승이 수액 주입에도 더 진행되지 않는 지점과 총체수분을 반영하는 1M 임피던스가 방혈 이후 증가하였다가 수액주입 후 감소하면서 초기값을 회복하는 시점이 일치했다. 또한 균형 정질액, 균형 정질액 + 5% 포도당 수액, 균형 정질액 + 20% 알부민 중 균형 정질액 + 20% 알부민만이 유의하게 수액 주입 종료 후에 실험 시작시점만큼 평균동맥압과 일회박출량을 회복시키는 결과를 보여주었다. 또한 균형 정질액 + 20% 알부민 군에서 1M 임피던스의 초기값 회복 시점까지 주입된 수액의 평균동맥압 및 일회박출량 회복 효과는 수액 주입 종료 시점과 그 효과가 동일하였다.
응급실에서 저혈량성 쇼크 환자에서 혈액제제 투여에 시간이 소요되어 손상 통제 소생술을 즉시 시행할 수 없는 경우가 많기 때문에 혈액제제가 아닌 일반 수액을 투여하는 경우가 많다. 본 연구 결과는 균형 정질액 단독 수액 요법보다 알부민을 혼합한 균형 정질액을 응급실에서 사용하는 것이 혈역학적 안정성 확보에 도움이 될 수 있음을 시사한다. 본 연구에서는 균형 정질액 800ml에 20% 알부민 200ml를 혼합한 수액 2L를 사용하였으나, 최적의 조합 비율에 대해서는 추후 후속연구가 필요하다. 평균동맥압과 일회박출량과 같은 생체징후 및 혈역학 지표의 회복이 총체수분을 반영하는 1M 임피던스의 변화 및 회복과 일치하므로 추후 중환자 수액치료에 생체전기저항 분석을 이용한 수액치료의 가능성을 제시한다.
