The correlation between blood glucose control and number of tooth loss according to physical activity levels : utilizing the 2014 Korea National Health and Nutrition Examination Survey

Research Background: According to various epidemiological studies, diabetes is known as a risk factor of severe periodontal diseases, and it is reported that diabetic patients have a high prevalence of periodontal diseases and high number of tooth loss.
Since tooth loss can issue various problems such as degradation of chewing and speaking abilities, as well as nutrient imbalance, preventing and treating diabetes by controlling blood glucose level is critical.
Exercise therapy is effective in controlling blood glucose. Since physical activities reduce blood glucose, improve insulin sensitivity, and enhance glucose tolerance, it prevents onset of complications caused by diabetes.
Although there are studies that physical activities are effective in controlling blood glucose level of diabetic patients and that periodontal diseases are improved by controlling the blood glucose level, it was difficult to find studies on relationship between physical activities, diabetes, and the number of tooth loss.
Therefore, this study is to research the relationship between diabetes and the number of tooth loss according to physical activity levels.
Research Subjects and Methods: This study utilized the data from the Korea National Health and Nutrition Examination Survey VI, second year (2014), and excluded 2,264 subjects who were under 30 years of age to select 5,286 subjects who were 30 years of age or older; then it analyzed the data by dividing the study subjects into two groups: one group with tooth loss of below 5 and another group with 5 tooth loss or more. Among them, 463 were excluded for missing factors of physical activities, 461 for missing factors of diabetes, and 361 for missing factors of confusion variables, bringing the final number of research subjects selected to 4,001.
In order to determine the relationship between diabetes and number of tooth loss depending on physical activity levels, this study did not revise any values in Model 1, revised gender and age in Model 2, and additionally revised education level, income level, marriage status, status of economic participation, smoking, drinking, body mass index, and status of chronic diseases in Model 3. In Model 4, this study conducted Multiple Logistic Analysis by correcting the Model 3 with status of dental inspections in less than a year.
Research Results: In Model 1, there was statistical significance on the relationship between blood glucose control and the number of tooth loss in every physical activity level with exception to the group of men in below average physical activity level group.
For all subjects, compared to subjects in normal blood glucose control group within 'above average physical activity level' group, subjects with diabetes had 1.76 times higher odds ratio (95% Confidence Interval, CI: 1.00-3.10) of having 5 or more tooth loss in Model 2 which had corrections for gender and age. However, this significance disappeared in Model 3.
The subjects in the ‘excellent physical activity level’ group with diabetes also displayed higher odds ratio of having tooth loss of 5 or more compared to those with normal blood glucose control.
Subjects with diabetes had 4.49 times the odds ratio (95% CI: 2.72-7.41) of having 5 or more tooth loss in Model 1, 2.33 times (95% CI: 1.28-4.25) in Model 2, 2.25 times (95% CI: 1.17-4.32) in Model 3, and 2.25 times (95% CI: 1.17-4.33) in Model 4.
In males with impaired fasting glucose in the 'poor physical activity level' group, the odds ratio of having 5 or more tooth loss was 1.12 times (95% CI: 1.10-4.07) in Model 2, 2.51 times (95% CI: 1.21-5.22) in Model 3, and 2.61 time (95% CI: 1.25-5.44) in Model 4. Subjects with diabetes in the 'excellent physical activity level' group displayed high odds ratio of having 5 or more tooth loss at 2.24 times (95% CI: 1.12-4.47) in Model 2, but this significance disappeared in Model 3.
The female subjects with diabetes in the 'above average physical activity level' group displayed high odds ratio of having 5 or more tooth loss at 2.29 times (95% CI 1.18-4.42), but this significance also disappeared in Model 3.
Conclusion: Subjects with impaired fasting glucose and diabetes displayed partially increased odds ratio of having 5 or more tooth loss compared to those with normal blood glucose control, and there was significant difference depending on physical activity level.
This study is significant in that it researched into the relationship between blood glucose control and the number of tooth loss based on physical activity level.
Henceforward, follow-up research that takes into consideration the various factors related to physical activity, blood glucose control, and number of tooth loss is needed, as well as applying the above information for the research on the role of physical activity in efforts to improve oral health including the reduction of number of tooth loss.
연구배경: 당뇨병은 여러 역학 연구를 통해 심한 치주질환의 위험인자(risk factor)로 알려져 있으며, 당뇨병환자는 치주질환 유병률이 높고, 상실치아가 많은 것으로 보고되고 있다.
치아 상실은 저작능력의 저하, 영양 불균형, 의사소통의 어려움 등 많은 문제를 발생시킬 수 있기 때문에 혈당을 조절하여 당뇨병을 예방하고, 치료하는 것은 매우 중요하다.
혈당을 조절하기 위한 방법으로 운동요법이 효과적인데, 신체활동은 혈당을 감소시키고 인슐린 감수성을 향상 시키며 당내성을 향상시키기 때문에 당뇨병으로 인한 합병증을 예방한다.
신체활동이 당뇨병환자의 혈당조절에 효과적이라는 것과 혈당이 조절됨으로써 치주질환이 개선된다는 연구는 있지만, 신체활동과 당뇨병 및 상실치아 수의 관련성에 관한 연구는 찾아보기 어려웠다. 따라서 이 연구에서는 신체활동 수준별로 당뇨병과 상실치아 수의 관련성을 알아보고자 한다.
연구대상 및 방법: 본 연구는 국민건강영양조사 제 6기 2차년도(2014년) 자료를 이용하여 만 30세 미만 대상자 2,264명을 제외한 만 30세 이상 5,286명을 대상으로 상실치아 수를 5개 미만, 5개 이상의 두 그룹으로 분류하여 분석하였다. 그 중 신체활동 변수에 결측이 있는 463명, 당뇨병 변수에 결측이 있는 461명, 혼란변수에 결측이 있는 361명을 제외하여 최종적으로 4,001명을 연구대상자로 선정하였다. 신체활동 수준별 당뇨병과 상실치아 수의 관련성을 파악하기 위하여 Model 1에서는 보정하지 않았고, Model 2에서는 성별, 연령을 보정하였고, Model 3에서는 Model 2에 추가적으로 교육수준, 소득수준, 결혼상태, 경제활동여부, 흡연, 음주, 체질량지수, 만성질환여부를 보정하였다. Model 4에서는 Model 3에 추가적으로 1년간 구강검진여부를 보정하여 다중 로지스틱 회귀분석(Multiple Logistic analysis)를 시행하였다.
연구결과: Model 1에서는 신체활동 수준별로 혈당조절양상과 상실치아 수의 관련성에서 남자 대상자 중 ‘신체활동 중하’ 군을 제외한 모든 군에서 모두 통계적으로 유의한 차이가 있었다.
전체 대상자의 경우, ‘신체활동 중상’ 군에서 혈당조절양상이 정상인 경우에 비해 당뇨병인 경우가 연령, 성별을 보정한 Model 2에서 상실치아가 5개 이상일 오즈비가 1.76배(95% Confidence Interval, CI: 1.00-3.10)높았지만, 이러한 유의성은 Model 3에서 사라졌다.
‘신체활동 상’ 군에서도 혈당조절양상이 정상인 경우에 비해 당뇨병인 경우 상실치아 수가 5개 이상일 오즈비가 모두 높았다
Model 1에서는 당뇨병일 경우, 상실치아 수가 5개 이상일 오즈비가 4.49배(95% CI: 2.72-7.41)높았고, Model 2에서는 2.33배(95% CI: 1.28-4.25), Model 3에서는 2.25배(95% CI: 1.17-4.32), Model 4에서는 2.25배(95% CI: 1.17-4.33) 높았다.

결론: 공복혈당장애 혹은 당뇨병인 경우 혈당조절양상이 정상인 경우에 비해 상실치아 수가 5개 이상일 위험이 부분적으로 증가했다.
특히 남성 중 ‘신체활동 하’ 군에 속할 경우 혼란변수를 모두 보정한 후에도 공복혈당장애인 경우 상실치아 수가 5개 이상일 위험이 2.61배 증가함으로써 신체활동량이 적은 공복혈당장애인 남성의 경우 여성에 비해 상실치아 수가 많아질 위험이 크다.
또한 여성 중 ‘신체활동 중상’ 군에 속할 경우 연령, 성별을 보정하고도 당뇨병인 경우 상실치아 수가 5개 이상일 위험이 2.29배 증가하였다.
따라서 공복혈당장애 및 당뇨병 환자에게 구강건강향상을 위하여 혈당조절 뿐만 아니라, 신체활동량의 증가도 강조되어야 할 것으로 생각된다.
본 연구는 신체활동 수준별로 혈당조절양상과 상실치아 수의 관련성을 알아보았다는 데에 의의가 있으며, 향후 신체활동과 혈당조절양상 및 상실치아 수에 관련된 요인을 다각적으로 고려한 후속 연구와 이를 적용한 구강건강 향상을 위한 신체활동의 역할에 대한 연구가 필요할 것으로 사료된다.
