혈액투석환자의 극복력과 스트레스 및 극복력 수준에 따른 특성 탐구

혈액투석환자는 질병과 지속적인 치료요구사항으로 인해 신체적, 정신적, 사회적 측면에서 삶의 질이 저하된다. 혈액투석환자에게 내재된 극복력을 증진시킴으로써 부적응적 요인은 감소시키고 대상자가 가진 강점은 강화시켜 혈액투석을 하는 대상자들의 긍정적인 적응과 삶의 질을 향상시킬 수 있다.
본 연구는 혈액투석환자의 스트레스와 극복력을 사정하고 극복력 수준에 따른 차이점을 파악하여 개인의 특성에 맞는 개별화된 간호 중재 전략을 마련하고 극복력을 향상시킬수 있는 방법에 대한 기반을 마련하고자 시행하였다.
서울 소재 K 대학교 부속병원 인공신장실에서 혈액투석을 하는 환자 20명을 대상으로 면담은 개별 1회~2회 진행되었다. 심층면담 질문지와 극복력 측정도구를 사용해 면담을 진행하였고 시간은 30분~1시간 내외로 소요 되었다. 자료수집 기간은 2023년 2월 1일부터 3월 30일이며, Elo, S. & Kyngas, H.(2008)의 질적 내용분석법에 따라 자료를 분석하였다.
혈액투석환자들이 가지고 있는 스트레스는 신체적 요인인 긴장, 피로, 오심, 부종등과 혈액투석시의 제한적 움직임이 있었고 정서 사회적 요인으로는 경제적 활동 및 신체활동 제한으로 인한 경제적 어려움, 사회적 고립 및 인간관계의 변화, 미래에 대한 불확실성, 생활습관의 변화가 있었다. 혈액투석환자의 극복력은 관계중심, 가치탐색, 자원의 선택, 계획의 수립 4가지의 유형과 가족구성원으로서의 책임인식, 단계적인 관계 재구성, 나의 존재가치의 정당화, 혈액투석에 대한 긍정적 평가, 비교를 통한 긍정적 평가, 부정적 극복요인에 대한 저평가, 자원으로서 의료인 관계인식, 자원으로서 의료 정보 및 복지활용, 질병의 몰두 회피를 위한 전환시도, 미래를 계획하고자 함, 포기해야 할 것과 해야 할 것의 경계 재수립, 상황인식의 객관화 12가지의 특성으로 도출되었다.
각 특성은 극복력의 수준에 따라 약간의 차이점을 가지고 있었다. 극복력은 한가지 유형으로 나타나기 보다는 다양한 유형 및 특성에 맞게 나타나므로 다양한 각도에서 극복력의 특성에 접근하는 것이 중요하다.
본 연구의 결과는 혈액투석환자들을 대하는 혈액투석실 간호사들에게 혈액투석환자들의 성공적인 적응을 위한 중재 및 전략을 세우는데 기초자료가 될것이며 극복력 증진과 관련된 개별적인 간호중재를 위한 교육 및 연구에 중요한 기여를 제시하였다.
Background: Hemodialysis patients experience a decline in quality of life in physical, mental, and social aspects due to the disease and continuous treatment requirements. Resilience is essential for overcoming these challenges in crisis situations. Enhancing inherent resilience in hemodialysis patients can reduce maladaptive factors and reinforce their inherent strengths, thereby improving their positive adaptation to hemodialysis and quality of life.
Objective: The purpose of this study was to explore the stress and resilience of hemodialysis patients, identify differences according to the level of resilience.
Data Collection and Analysis: Data was collected through individual interviews conducted one to two times with 20 patients undergoing hemodialysis at the Artificial Kidney Room of K University Hospital in Seoul. Each interview, which utilized in-depth interview protocols and resilience measurement tools, lasted approximately 30 minutes to an hour. The data collection period spanned from February 1 to March 30, 2023, and the data were analyzed according to the qualitative content analysis method of Elo and Kyngas (2008).
Results: The stresses experienced by hemodialysis patients included physical factors such as tension, fatigue, nausea, edema, and limited movement during dialysis, as well as socio-emotional factors like economic difficulties due to restricted economic and physical activities, social isolation, changes in relationships, uncertainty about the future, and lifestyle changes. The resilience of hemodialysis patients was categorized into four types: relationship-centered, value-seeking, resource selection, and planning. Twelve characteristics were identified recognition of responsibility as a family member, gradual restructuring of relationships, justification of one's existential value, positive evaluation of hemodialysis, positive evaluation through comparison, underestimation of negative overcoming factors, recognition of healthcare professionals as resources, utilization of medical information and welfare as a resource, attempts to divert from disease immersion, desire to plan for the future, reestablishment of boundaries between what must be given up and what must be done, and objective awareness of the situation.
Conclusion: Each characteristic exhibited slight variations depending on the level of resilience. Resilience does not manifest as a single type but appears in various forms and characteristics, making it crucial to approach the features of resilience from multiple perspectives. Hemodialysis patients cope with stress arising from various characteristics, depending on their type of resilience. Therefore, it is essential to approach the characteristics of resilience from diverse angles. The findings of this study provide foundational data for hemodialysis nurses in developing interventions and strategies for the successful adaptation of hemodialysis patients. Additionally, this study makes a significant contribution to education and research for individual nursing interventions aimed at enhancing resilience.
