A Study to Inspect the Nutrients and Other Related Factors on Bone Mineral Density in Korean Women and Men Using a Structural Equation Modelling

세계 인구는 빠른 속도로 고령화가 되고 있고, 한국 또한 급속하게 고령화 사회로 진입하고 있다. 고령이 되면 골 손실로 인한 골감소증, 골다공증 등의 근골격계 질환율의 위험이 높아진다.
본 연구는 제4차 및 5차 국민건강영양조사 데이터를 바탕으로 한국 성인 여성과 남성에서 구조방정식 모델을 사용하여 영양소 섭취와 그 외 요인과 골밀도와의 연관성을 조사하였다. 30세에서 75세 여성 4,160명 (폐경 전 여성 2,863명, 폐경 후 여성 1,297명)과 50에서 75세 남성 1,588명을 대상으로 하였다. 구조방정식 분석은 영양소 섭취 및 그 외 요인 (나이, 체질량 지수, 신체활동, 혈청 비타민 D, 혈청 PTH)의 직접효과, 총효과 및 매개효과를 조사하였다. 외부 오지정 (misspecifications)에 대한 모형의 민감도와 통계적 유의성은 부트스트래핑과 팬텀변수에 의해 결정되었다. 구조방정식의 신뢰도 평가는 크론바흐 알파를 이용하여 수행하였다.
본 연구 결과, 골밀도 (대퇴골 전체, 대퇴골 경부, 요추 및 전신)에 대한 미네랄 (칼륨, 칼슘 및 인)의 직접효과는 폐경 전 및 폐경 후 여성, 남성에서 모두 나타났다 (폐경 전 여성 P = 0.045, 폐경 후 여성 P = 0.048, 남성 P = 0.003). 연령과 체질량지수는 폐경 전 및 폐경 후 여성의 골밀도에서 총효과가 있는 것으로 나타났다 (각각 P = 0.002). 미네랄 섭취는 남성의 나이와 골밀도 사이에 매개효과가 있는 것으로 나타났다 (P = 0.008). 남성에게 있어서 신체활동은 골밀도에 직접효과가 있었고 (P = 0.021), 나이는 골밀도에 총효과 및 직접효과가 있었으며 (총효과: P = 0.002, 직접효과: P < 0.001), 체질량지수도 총효과 및 직접효과가 있는 것으로 나타났다 (총효과: P = 0.002, 직접효과: P < 0.001).
따라서 30세에서 75세의 폐경 전∙후 한국 여성에서 미네랄 섭취 (칼륨, 칼슘 및 인), 나이 및 체질량지수가, 50세에서 75세 한국 남성에서는 미네랄의 섭취, 나이, 신체활동 및 체질량지수가 골밀도에 주요하게 영향을 주는 인자임을 알 수 있었다.
Background: no studies have assessed the associations of nutrient intake, physical activity, age, and body mass index (BMI) with bone mineral density (BMD) using structural equation modelling (SEM) in Korean women. The aim of this study was to examine the effects of nutrient intakes, physical activity, and body mass index (BMI) on BMD in Korean premenopausal and postmenopausal women, with the SEM approach, based on the fourth and fifth Korea National Health and Nutrition Examination Surveys (KNHANES) 2008–2011.
Methods: SEM analysis was performed with 4,160 women (2,863 premenopausal women and 1,297 postmenopausal women) aged 30–75 years in order to investigate total, direct, or mediating effects of nutrient intake, physical activity, age, and BMI on BMD. Model sensitivity to external misspecification and statistical significance of SEM was determined by phantom variables and bootstrapping. Reliability assessment of the SEM was done by Cronbach’s alpha.
Results: a direct effect of minerals (potassium, calcium, and phosphorus) on BMD (total femur, femoral neck, lumbar spine, and whole body) was observed in premenopausal and postmenopausal women (P = 0.045 and P = 0.048, respectively). Age and BMI showed a total effect on BMD in premenopausal and postmenopausal women (P = 0.002, respectively).
Conclusions: our study suggests that mineral intake (potassium, calcium, and phosphorus), age, and BMI are major contributors to BMD in Korean premenopausal and postmenopausal women aged 30–75 years.
Background: Insufficient studies have been conducted to assess associations among bone mineral density (BMD) with nutrient intakes and clinical parameters using a structural equation modeling (SEM) in men. The objective was to assess mediating influence of nutrient intakes, blood concentrations of parathyroid hormone (PTH) and 25-hydroxyvitamin D (25(OH)D) on BMD in men based on the 4th and 5th (2008-2011) Korea National Health and Nutrition Examination Surveys (KNHANES).
Methods: A SEM analysis was conducted to assess the influence of nutrient intakes, physical activities, blood PTH and blood 25(OH)D, age and BMI on bone density in men (mean age = 50-75 years; n = 1,588). Phantom variables were used for model sensitivity to external misspecification. Bootstrapping was used for statistical significance of a SEM. Cronbach's alpha was measured to assess the reliability of a SEM.
Results: A direct influence of mineral intakes (calcium, phosphorus and potassium) on BMD (lumbar spine, femoral neck, total femur and whole body) was found (P = 0.003). Mineral intakes (calcium, phosphorus and potassium) played a mediating influence on the relationship among age and bone density (P = 0.008). Physical activity directly influenced BMD (P = 0.021). Age showed total and direct influences on BMD (P = 0.002 and P < 0.001, respectively). Moreover, BMI showed total and direct influences on BMD (P = 0.002 and P < 0.001, respectively).
Conclusions: Mineral consumptions (calcium, phosphorus and potassium), age, physical activity and BMI are main factors influencing bone density in men (mean age = 50 to 75 years).
