배경 및 목적: 신종담배는 안전성이 명확히 규명되지 않은 채 청소년 노출이 증가하고 있다. 새로운 신종담배 제품이 출시될 때 청소년들의 선택 행동에 대한 연구가 필요하다. 이에 본 연구에서는 담배제품 복합사용 경험 청소년을 대상으로 궐련담배와 전자담배 두 제품의 사용 시작 연도의 차이와 가열담배 사용 경험의 관련성에 대해 알아보고자 한다.
연구방법: 이 연구는 ‘제14차(2018년) 청소년건강행태조사’를 이용하여 중1~고3 60,040명을 분석하였다. 신종담배 출시에 따른 담배제품별 복합사용 양상을 단독사용군과 복합사용군으로 분류하였고, 복합사용군을 이중사용군과 삼중사용군으로 구분하여 파악하였다. 담배제품 복합사용군 대상을 궐련담배 시작 연도와 전자담배 시작 연도의 차이(Difference of start year between Conventional cigarette and Electronic cigarette, DCE)를 활용하여 궐련담배 먼저 시작군, 동시 시작군, 전자담배 먼저 시작군으로 구분하였다. 인구사회학적 요인, 건강행동요인, 흡연환경요인과 담배제품 복합사용 및 궐련담배, 전자담배 사용 시작 연도 차이의 양상과 관련성을 기술 통계로 확인하였다. 전체 대상 및 중학생, 고등학생에 대해 혼란변수를 통제한 후 궐련담배와 전자담배 시작 순서와 가열담배 경험의 관련성을 로지스틱 회귀분석으로 알아보았다.
연구결과: 궐련담배 현재사용자는 3.4%(2,013명), 전자담배 현재사용자는 0.3%(203명)이었다. 궐련담배, 전자담배, 가열담배를 모두 사용하고 있고 경험한 적이 있는 삼중사용자의 분율은 1.1%(남학생 1.7%, 여학생 0.5%)였다. 궐련담배와 전자담배 사용 시작 연도 차이를 활용하여 분석한 결과, 궐련담배와 전자담배를 동시에 시작한 군이 남학생 32.7%(1,127명), 여학생 28.6%(250명)로 가장 많았다. 중고등학생을 구분하여 분석 시, 중학생의 경우 ‘전자담배 2~6년 먼저 시작군’의 분율이 전체 흡연 중학생의 4.1%(33명), 고등학생의 경우 2.7%(72명)였다. 두 담배제품의 시작 순서와 가열담배 경험의 관련성 분석에서, 중학생은 ‘동시 시작군’에 비해 ‘궐련담배 먼저 시작군’의 오즈비는 2.0(95% CI= 1.1-3.7), ‘전자담배 먼저 시작군’의 오즈비는 2.9(95% CI=1.0-8.5)로 높았다. 고등학생의 경우 ‘동시 시작군’에 비해 ‘궐련담배 먼저 시작군’의 오즈비는 1.2(95% CI= 1.0-1.6), ‘전자담배 먼저 시작군’은 0.9(95% CI=0.5-1.6)였다.
결론: 궐련담배와 전자담배 시작 순서에 있어서, 같은 연도에 시작한 경우에 비해 궐련담배 또는 전자담배를 먼저 시작한 중학생은 추후 가열담배도 사용할 위험이 높았고, 고등학생에서는 궐련담배를 먼저 시작한 경우 추후 가열담배 경험 위험이 높았다. 조사 시점(2018년) 중학교 1~3학년 학생들은 가열담배가 국내 출시된 2017년도에 초등학교 6학년, 중학교 1~2학년이었다. 본 연구 자료는 가열담배 국내 출시 후 편의점을 중심으로 광범위한 마케팅이 이루어진 2018년도에 시행되었으므로, 결과 해석에 있어서, 흡연 시작이 가장 많은 연령대인 중1~2학년들이 가열담배 마케팅이라는 위험에 노출된 시기에 조사되었음을 고려해야 한다. 새롭게 출시되는 신종담배에 대한 청소년의 반응에 대한 지속적인 연구가 필요하며, 적극적인 교육 활동과 시대 변화에 맞춘 정책 제시를 통해 청소년들 담배의 유혹으로부터 보호할 수 있는 환경을 만들어야 할 것이다.
Background and Purpose: New types of tobacco(including e-cigarette and heat-not-burn tobacco) have increased exposure to young adolescents without knowing the safety of new tobacco products. When new tobacco products are launched, it is necessary to study adolescent’s tobacco selection behavior. Therefore, the purpose of this study is to explore the relationship between the year of using conventional cigarette and electronic cigarette and the use of heated cigarette for adolescents who have experienced the use of tobacco products.
Methods: This study analyzed 60,045 students aged 13-18 years using the 14th(2018) Korean Youth Risk Behavior Survey(KYRBWS). According to the launch of new tobacco products, student smokers were classified into Single user, Multiuser by number of tobacco products they used. Once more, Multiuser was divided into double user and triple user. By using Difference of start year between Conventional cigarette and Electronic cigarette, DCE, Multiuser was classified in to ‘Start conventional cigarette first group’, ‘Start conventional cigarette and electronic cigarette at the same time group’, ‘Start electronic cigarette first group.’ First, the 14th Korean Youth Health Survey data was divided into middle school students and high school students and the trend of usage of tobacco products and DCE distribution among student smokers were analyzed using descriptive statistics. The relationship between conventional cigarette and electronic cigarette starting order and the risk of using heat-not-burn tobacco was examined by logistic regression after controlling the confounders such as socio-demographic factors, health behavior factors, smoking environment factors.
Result: The current conventional cigarette use group was 3.4%(2,013) and the e-cigarette current user group was 0.3%(203). The percentage of triple users who used conventional cigarettes, electronic cigarettes, and heated cigarettes was 1.1% (1.7% male students, 0.5% female students). As a result of analyzing DCE, 32.7%(n=1,127) of male students and 28.6%(n=250)of female students started cigarettes and electronic cigarettes at the same time. For middle and high school students, the proportion of 'Electronic cigarettes 2~6 years started first' was 4.1%(n=33) for all smoking middle school students and 2.7%(n=72) for high school students. The result of analyzing the relationship between cigarette and electronic cigarette starting order and the risk of using heated cigarette, middle school students showed 2.0(95% CI=1.1-3.7) for 'Start conventional cigarette first group' and 2.9(95% CI=1.0-8.5) for 'Start e-cigarette first group' compared to ‘Started conventional cigarettes and electronic cigarettes at the same time group.’In case of high school students, 'Start conventional cigarette first group' was 1.2 (95% CI=1.0-1.6) and 'Start e-cigarette first group' was 0.9 (95% CI=0.5-1.6) compared to‘Started conventional cigarette and electronic cigarette at the same time group.’
Conclusion:
Middle school students who started e-cigarettes before conventional cigarettes were at higher risk of using heating cigarettes later than those who started in the same year. At the time of the survey (2018), the first to third graders of middle school were sixth graders in elementary school and first and second graders in middle school in 2017, when heated cigarettes were released in Korea. This study was conducted in 2018 when a wide range of marketing was conducted around convenience stores after the domestic launch of heated cigarettes. Therefore, in analyzing the results, it should be considered that the first and second graders who are the most likely to start smoking were surveyed at the time of exposure to the risk of heating cigarette marketing. The response of adolescents to new cigarettes should be continuously studied. Through active educational activities and policy presentations tailored to changes in the times, it is necessary to create an environment in which young people can protect themselves from the temptation of tobacco.
