서울시 정신응급 대응 현황조사 보고서 = A Report of the Survey on the Current State of the Psychiatric Emergency Response in Seoul

조사목적
서울지역 기초정신건강복지센터와 서울시자살예방센터를 중심으로 서울시의 정신응급 대응 요청 및 개입, 정신응급 대응 업무 수행과정에 대한 현황을 파악하여 관련 유관기관과의 협력도모와 향후 정신응급 적정서비스 제공을 위한 대안을 제시하고자 본 조사를 실시하였다.
조사방법
본 조사는 2020년 정신응급 대응 현황에 대해 정신응급 대응업무를 수행하고 있는 자치구보건소 및 정신건강복지센터 각 25개소와 서울시자살예방센터 1개소를 대상으로 2021년 2월 22일부터 3월 31일까지 서면조사, 정신건강복지센터 실무자 대상으로 2월 22일부터 3월 31일까지 온라인조사를 실시하였다.
기관종별 업무 성격에 따라 보건소 대상으로 지정정신의료기관 등 의료기관 현황조사, 치료비 지원 현황, 외래치료지원제 현황에 대해 조사하였으며, 정신건강복지센터와 자살예방센터 대상으로는 정신응급 대응체계 구성, 진단 및 보호신청, 정신응급 출동 요청 및 주요 조치 사항에 대해 조사하였다. 실무자 대상 설문조사 내용은 정신응급 대응체계에 대한 총 13개 문항으로 구성하여 실시하였다.
조사결과
서울시의 지정정신의료기관은 2020.12.31.기준 8개소로 전년과 동일하며, 정신응급대응 협의체는 23개 자치구에서 운영 중이었다. 경찰과의 협력정도도 자치구별로 상이하였으며, 일부 자치구의 경우 경찰 요인보다는 정신의료기관의 병상문제 등의 해결이 시급하다고 제시하였다. 조사 결과, 외래치료지원제 청구 및 결정 건수는 2건이었으며, 제공횟수나 범위는 각 지역마다 차이가 있었다. 실무자 대상 조사에서는 전체 응답자의 93.9%가 정신응급 대응 업무를 수행하고 있으며, 기관의 여건에 따라 담당이나 출동인력을 구성하는 방법에 차이가 있었다. 정신과적 입원치료를 위해 74.7%가 호송 시 협력기관과의 네트워크 차원으로 동행하였으며, 정신과적 응급상황에서의 현장대응안내 2.0에의 실효성에 대해 54%가 5점 이상으로 평가하였다. 2020년도 한해 정신응급 출동 요청 1,142건 중 1,038건(90.9%)에 실제 개입하였으며, 경찰의 요청이 49%로 가장 많았다. 주요조치 결과로는 응급입원 연계가 24.6%로 가장 많았고, 전년대비 응급출동 개입건수는 감소하였으나 입원치료 연계 비중은 2019년 39%에서 2020년 45%로 증가하였다. 정신응급출동 개입 건 중 정신과적 문제로 인한 대상자가 55.2%, 자살문제 25.9%, 나머지는 복합문제 18.9%로 나타났다. 이들 중 정신건강복지센터 등록회원은 19%이었고, 미등록자 중 추후 모니터링을 통해 신규등록한 회원이 7%로 나타났다.
결론 및 제언
서울시 전체의 정신응급 현황과 규모를 파악하는 데 한계가 있으나 본 조사 결과를 바탕으로 현장에서 경험하고 있는 다양한 의견을 통해 서울시 정신응급 대응체계에 대해 몇 가지 제안을 하고자 한다.
첫째, 서울지역 내에서 신체 질환이 동반된 정신질환자 급성기 치료를 포함한 정신응급의료센터 또는 응급 및 행정입원이 가능한 정신과 병상 확보, 이를 모니터링할 수 있는 체계 마련이 필요하다. 둘째, 정신응급 대응 유관기관 간 역할 조정 및 주요 이슈논의가 필요하나 이해관계가 복잡하고 법적 역할 등이 모호하여 상호 간 협의점을 도출하기 어려우므로 서울시의 컨트롤타워로서의 역할이 강화되어야 한다. 셋째, 정신응급 대응 업무와 관련된 범위와 기준, 역할 등이 명시되어 있는 가이드라인 및 표준화가 필요하다. 본 현황조사를 근거로 정신응급 대응 규모를 파악하고 관련된 객관적 자료를 수집하여 포괄적인 정신응급 대응체계 개선을 위한 자료로 활용될 수 있기를 기대한다.
The purpose of the survey
By finding out the current state of psychiatric emergency responses and interventions in Seoul and the process of carrying out the duty of responding to psychiatric emergencies centered around Mental Health Welfare Centers in Seoul and the Seoul Suicide Prevention Center, the survey was conducted to seek cooperation with the related institutions and to suggest alternatives to provide appropriate services for psychiatric emergencies in the future.
The methods of the survey
From February 22 to March 31, 2021, a written survey was conducted with the autonomous districts, 25 Mental Health Welfare Centers, and Seoul Suicide Prevention Center that are carrying out the duty of responding to psychiatric emergency situations in 2020. And an online survey was performed with staff at Mental Health Welfare Centers from February 22 to March 31.
By following the characteristics of the duties of each institution, the survey looked into the current status of psychiatric medical institutions such as Psychiatric institutions designated by the Ministry of Health and Welfare for administrative hospitalization, the current status on the supporting of medical expenses, and the current outpatient treatment support system were investigated for Public Health Centers. The survey looked into the composition of the psychiatric emergency response system, requests for diagnoses and protection, requests for psychiatric emergency mobilizations, and key measures were investigated targeting With Mental Health Welfare Centers and the Suicide Prevention Center. The survey for the staff members was carried out by consisting of a total of 13 items for the psychiatric emergency response system.
The results of the survey
As of December 31, 2020, the number of Psychiatric institutions designated by the Ministry of Health and Welfare for administrative hospitalization in Seoul was 8, which was the same as last year, and psychiatric emergency response consultative bodies were in operation in 23 autonomous districts. The degree of cooperation with the police differed in each autonomous district. Some autonomous districts argued that solving the problem of obtaining hospital beds in psychiatric medical institutions was more urgent than the police factor. The number of the demands for the outpatient treatment support system and such decisions was 2, and the number of provisions and the scope of such a system differed in each region. In the survey conducted for staff members, 93.9% of the respondents said that they are carrying out the duty of responding to psychiatric emergencies, and the methods in organizing those in charge of psychiatric emergencies and in the mobilizing of manpower differed depending on the conditions of the institutions. 74.7% of the respondents said that for the network dimension with cooperative institutions, they accompanied patients when they were transported for psychiatric hospital treatments, and 54% of the respondents gave more than 5 points with respect to the effectiveness of the on-site response guide 2.0 of psychiatric emergency situations. For the year 2020, among 1,142 cases of the mobilization requests for psychiatric emergencies, the respondents intervened in 1,038 cases (90.9%), and 49% was the requests of the police, which was the highest. For the results of key measures, 24.6% was linked to emergency hospitalization, which was the highest. The number of emergency mobilization interventions was reduced compared to last year, but the rate linked to hospital treatment was increased from 39% in 2019 to 45% in 2020. Among the cases of psychiatric emergency mobilization interventions, 55.2% of people were related to mental problems, and 25.9% of people were involved with suicidal problems. And 18.9% of people showed complex issues. Among them, 19% of people were registered members at Mental Health Welfare Centers. Among the unregistered people, further monitoring discovered that 7% of them became newly registered members.
Conclusions and suggestions
Although there are limitations in figuring out the current state and the scale of psychiatric emergencies in the entire area of Seoul, several suggestions can be laid out for the psychiatric emergency response system in Seoul through various opinions on on-site experiences based on the results of the survey.
First, it is required to secure psychiatric emergency medical centers that include acute treatments for those with mental illness accompanied by physical illness or beds at psychiatric hospitals where emergency and administrative hospitalizations are possible. And forming a system that can monitor them is necessary. Second, although an adjustment of the roles of the related institutions for psychiatric emergency responses and a discussion on the major issues are required, their interests are complex, and their legal roles are vague that is difficult to reach an agreement. Therefore, the role of Seoul Government as a control tower should be strengthened. Third, the guidelines and standardization that explicitly state the range, standards, and roles related to the duty of psychiatric emergency responses are required. It is expected that this current status survey can be used as the data for improving a comprehensive psychiatric emergency response system by finding out the scale of the psychiatric emergency responses and collecting the related objective data.
