▪연구목적: 본 연구는 기초정신건강복지센터의 안정적인 운영을 지원하고 전문적인 기능이 수행될 수 있도록 현안을 점검하고자 하였다. 다양한 조사 및 근거 자료를 바탕으로 정신 건강 환경변화에 맞춰 기초정신건강복지센터의 현황을 파악하고 향후 나아갈 방향성을 제시하는 등 기초정신건강복지센터의 운영 전반을 지원하고자 하였다.
▪연구방법: 본 연구는 국가 정신건강 정책 방향성 및 서울시 정신건강 및 자치구별 현황을 기반으로 2021년부터 2022년까지 서울시 기초정신건강복지센터 사례관리 현황을 분석하였다. KOSIS 국가통계포털 등을 활용한 기본 현황뿐만 아니라 자치구 기초 정신건강복지센터별 정신건강사례관리시스템(MHIS) 현황, 집중사례관리서비스(ICM) 현황 등 정신건강복지센터 중심의 환경 및 현황 전반에 대한 분석을 진행하였다.
▪연구결과: 2022년 서울시 기초정신건강복지센터 일반상담(접수상담)은 총 185,274건(평균 7,411건)으로 전년 대비 50,417건(평균 2,017건) 감소하였다. 전년과 동일하게 우울증 관련 상담이 가장 높았으며, 전화 상담이 가장 높은 비율을 보였다. 재난 상담의 영향으로 정보제공 비율이 가장 높았던 전년과 달리, 2022년에는 지속상담이 47.8%로 가장 높은 비율을 차지하였다. 등록회원은 총 9,775명(평균 391.0명)으로 20대의 비율이 가장 높았던 일반상담(접수상담)과 달리 50대가 21.2%로 가장 높은 비율을 보였다. F30코드의 비율이 45.5%로 가장 높았으며 전년 대비 많은 증감(1.4%p▲)을 보였다. 신규 등록회원은 2,643명(평균 105.7명)으로 신규 등록률은 27.0%로 나타났으며, 연계 회원은 2,032명(평균 81.3명)으로 연계율은 20.8%로 나타났다. 퇴록회원은 2,788명(평균 111.5명)으로 퇴록율은 22.2%로 나타났으며, 서비스 종결로 인한 퇴록이 57.1%로 가장 높았고, 세부 사유로는 본인 희망이 52.0%로 가장 높았다. 개별상담은 총 165,195건(평균 6,607.8건) 진행되었고, 이 중 대면상담 건수는 72,298건으로 전체 상담 건수 중 43.8%를 차지하였다. 연간 1회 이상 ISP를 수립하지 않은 대상자는 334명(평균 13.4명)으로 전체 등록회원 중 3.4%를 차지, 서울시 기초정신건강복지센터 등록회원은 대부분 ISP를 수립하고 있는 것으로 보이며, ISP 수립 실인원 11,282명(평균 451.3명) 중 관리 구분 기간을 초과한 대상자는 3,039명(평균 121.6명)으로 26.9%를 차지하였다. 집중사례관리 서비스(ICM) 연인원은 대상자 10,460명(평균 418.4명), 사례관리자 2,701명(평균 108.0명)으로 사례관리자 1인당 대상자 3.9명을 관리하였다. 선정 사유는 불안정한 정신질환자가 1,022명(평균 40.9명)으로 51.9%를 차지하며 가장 높았으며, 기능 향상으로 인한 종결이 640명(평균 25.6)으로 47.6%를 차지하며 가장 높았다. 평균 개입 기간은 200.0일(약 6.7개월)로 나타났다.
▪결론: 정신건강 사업 관련 국가 동향 및 서울시 현황을 기반으로 2022년 기초정신건강복지센터에서 제공된 일반상담(접수상담), 등·퇴록 회원, 직접 서비스 등 MHIS 현황, ICM 현황 등을 중심으로 서울지역 기초정신건강복지센터의 현주소를 파악할 수 있었다. 일반상담(접수상담) 대상자는 주로 2·30대가, 등록회원은 주로 50대가, 신규 등록회원은 1·20대가 많은 특징을 보이며, 최근 주요 관심 이슈 중 하나인 정신질환자 치료비 지원 등을 위해 비교적 젊은 층의 신규 등록 대상자가 많은 것을 추정할 수 있었다. 일반상담(접수상담) 정신건강 문제 종류는 우울증이, 등록된 회원의 진단 코드는 F30코드에서 많은 비율을 차지한 점 역시 특징적으로 보이며, 코로나19의 영향으로 감소되었던 대면상담의 비율이 조금씩 증가한 것으로 보인다. 한편, 정신건강 관련 이슈의 증가 및 지속적인 정신 응급상황 발생 등 급변하는 환경 속에서 서울시 기초정신건강복지센터 종사자의 불안정한 인력 현황은 지속적인 한계점으로 지적된다. 특히 최근 이상 동기 범죄와 정신건강 문제를 연결 짓는 등 관련 이슈가 다양해지면서 정신건강 서비스에 대한 관심이 높아지고, 서비스 전달체계 구축을 위한 정신건강복지센터의 역할을 강화할 것으로 예상된다. 이에 지역사회 내 기초정신건강복지센터 본연의 업무인 정신질환자 대상 사례관리 서비스 제공과 정신건강 위기 상황 대처를 할 수 있도록 다양한 전문 기관에서 연계되는 대상자의 유입체계를 단일화하고 전문적인 사례관리 서비스를 제공하며 이후 증상이 안정화되면 지역에서 다시 생활할 수 있도록 지원해야 한다.

▪Purpose: This study examined current issues to ensure the stable operation of basic mental health welfare centers while upholding their professional functions. Utilizing various surveys and evidence, this study aimed to support the overall functioning of these centers by assessing the current state of such welfare centers in alignment with the evolving mental health landscape and providing insights into future directions.
▪Methods: We analyzed the case management status of basic mental health welfare centers in Seoul spanning the years 2021 to 2022. This analysis was based on the national mental health policy directives and considered the mental health status and the specific dynamics of autonomous districts within Seoul. Furthermore, in addition to the fundamental statistical data provided by the KOSIS national statistics portal, the study encompassed an assessment of the broader environment and the status of such centers. This included the evaluation of the mental health case management system (MHIS) and the intensive case management service (ICM) provided by autonomous district basic mental health welfare centers.
▪ Findings: In 2022, basic mental health centers in Seoul recorded a total of 185,274 general consultations, which are intake consultations. On average, there were 7,411 consultations conducted throughout the year. This marked a decrease of 50,417 consultations (average of 2,017) compared to the previous year. Similar to the previous year, consultations related to depression remained the most common. Telephone consultations accounted for the highest percentage of these interactions. However, in contrast to the previous year, when information provision was the most frequent owing to disaster counseling, in 2022, ongoing counseling was most prevalent, comprising 47.8% of the total consultations. There were 9,775 registered members, with an average of 391.0 members per center. Unlike general counseling (intake counseling), where the highest participation rate was among individuals in their 20s, in 2022, the rate was highest among those in their 50s, at 21.2%. Notably, the percentage of F30 diagnostic codes was the highest, at 45.5%, indicating a significant increase (1.4 percentage points higher than the previous year). Regarding membership, there were 2,643 and 2,032 new and affiliated members on average, representing a new registration rate of 27.0% and 20.8%, respectively. However, there were 2,788 withdrawals, averaging 111.5 withdrawals per center, a withdrawal rate of 22.2%. Of these, 57.1% were due to service termination, while 52.0% were initiated by the members themselves. In 2022, there were a total of 165,195 individual consultations, with an average of 6,607.8 consultations per center. Of these, 72,298 consultations were conducted face-to-face, accounting for 43.8%. A total of 334 participants (average 13.4) did not establish an Individualized Service Plan (ISP) at least once a year, representing 3.4% of the total registered members. This indicates that the majority of registered members at the basic mental health centers in Seoul are actively engaged in the establishment of an ISP. Among the 11,282 participants (average 451.3) who did not establish an ISP, 3,039 (average 121.6) exceeded the management separation period, comprising 26.9% of this group. The ICM population comprised 10,460 clients, with an average of 418.4 clients per center, and 2,701 case managers, with an average of 108.0 case managers per center. This resulted in a ratio of 3.9 clients per case manager. The most common reason for selecting clients for ICM was unstable mental illness, accounting for 1,022 cases (with an average of 40.9), representing 51.9%. Additionally, closure due to functional improvement was a common reason, with 640 cases (with an average of 25.6), accounting for 47.6%. On average, the duration of intervention within the ICM program was 200.0 days, approximately 6.7 months.
▪Conclusions: Drawing from national trends and the current state of mental health projects in Seoul, this study provides an overview of the aforementioned centers in Seoul for 2022, with a focus on elements including MHIS status, ICM status, general counseling (intake counseling), registered and unregistered members, as well as the direct services offered at these centers. The analysis revealed some significant findings: clients receiving general counseling (intake counseling) predominantly fell within the age range of 20-30. Registered members were primarily in their 50s. Newly registered members were mainly in their 10-20s, highlighting a notable influx of younger individuals seeking support for mental health treatment costs. This is a pertinent issue of concern in recent times. Depression emerged as the most common mental health concern in general counseling (intake counseling), and the diagnostic code F30 accounted for a significant portion of registered members’ diagnoses. The proportion of face-to-face counseling, which had previously decreased due to the impact of COVID-19, saw a slight increase. However, it is important to recognize that Seoul’s basic mental health centers face challenges related to staffing stability in a rapidly evolving mental health landscape. The increasing prevalence of mental health-related issues and the recurring occurrence of mental emergencies add to these challenges. Notably, the diversification of issues, such as the link between abnormal crimes and mental health problems, is expected to intensify interest in mental health services. This underscores the need for basic mental health centers to strengthen their role in building a service delivery system. The primary responsibility of such centers remains to provide case management services for individuals with mental illness in the community. This involves coordinating the intake system for individuals connected to various professional organizations to help them navigate mental health crises, deliver expert case management services, and support their reintegration into the community once their symptoms stabilize.
