Centrifugal blood pumps artificially circulate blood and have various medical applications such as in ventricular assist devices (VADs) and extracorporeal membrane oxygenation (ECMO) devices. However, pump head thrombosis is a serious problem that can occur while using centrifugal blood pumps. This is fatal to patients and requires immediate action. The detection of pump head thrombosis relies entirely on audiovisual analysis and blood tests conducted by experienced medical staff; this makes it difficult to respond in real time, thereby reducing patient survivability. Therefore, it is necessary to develop an algorithm that automatically detects pump head thrombosis. The objective of this study was to improve the performance of an algorithm that continuously monitors the thrombus that is attached to and rotates with the impeller of a centrifugal blood pump. This study proposes various parameters for machine fault detection and a deep neural network-based convolutional neural network (CNN). The algorithm was validated through a performance evaluation by in-vitro testing.
The in-vitro tests were performed using 6 porcine thromboses and 58 imitation thromboses . The imitation thromboses made by casting silicone possessed various physical properties and dimensions. A different thrombus was used in each experiment. The thrombus was fixed inside the pump head, and the pump was activated. The vibrations generated from the pump head was measured using an accelerometer. The sampling rate was 10,000 Hz.
First, the machine fault detection parameters that were used to detect pump head thrombosis were validated. When a thrombus forms on the impeller, it displaces the center of mass of the impeller, which destabilizes it and causes specific vibrations. In this study, 13 parameters including root mean square (RMS), skewness, kurtosis, shape factor (SF), crest factor (CF), clearance factor (CLF), impulse factor (IF), energy operator (EO), energy ratio (ER), zero-order figure of merit (FM0), fourth-order figure of merit (FM4), M6A, and M8A, which are used for detecting machine faults, were selected based on this principle. The vibration signals measured in the in-vitro tests were converted into the selected parameters. The parameters were applied to design a feature- and classification-based detection algorithm using adaptive boosting (AdaBoost). The performance was assessed using the area under the receiver operating characteristic (ROC) curve (AUC). As a result of the analysis, the performance of energy ratio (imitation thrombus detection AUC 0.922) and kurtosis (porcine thrombus detection AUC 0.961) was the highest among each machine fault detection parameters. The above parameters outperformed the state-of-the-art (SOTA) 3rd harmonic change of rotation speed of impeller (imitation thrombus detection AUC 0.888, porcine thrombus detection AUC 0.916). By recursive feature addition (RFA), a combination of 4 machine fault detection parameters was selected to improve the performance (imitation thrombus detection AUC 0.9848 and porcine thrombus detection AUC 0.9880). In addition, the performance was further improved when the 3rd harmonic change (SOTA) and the machine fault detection parameters selected by RFA were used in combination (imitation thrombus detection AUC 0.9853 and porcine thrombus detection AUC 0.9891). This proved that the machine fault detection parameters could be used to provide important information for detecting pump head thrombosis.
Second, we verified whether the performance of the detection algorithm could be progressed by applying a deep learning-based CNN. A short-time Fourier transform (STFT) was applied to transform the vibration signals into spectrograms. A detection algorithm was developed using a CNN referring to the very deep convolutional network (VGGNet) structure. An evaluation of the detection algorithm performance using the AUC showed that the CNN (imitation thrombus detection AUC 0.965 and porcine thrombus detection AUC 0.956) outperformed artificial neural networks (ANNs) (imitation thrombus detection AUC 0.784 and porcine thrombus detection AUC 0.793) and AdaBoost (SOTA) (imitation thrombus detection AUC 0.913 and porcine thrombus detection AUC 0.923). Thus, it was confirmed that a CNN is an effective learning method for detecting pump head thrombosis.
Third, we verified whether the performance of the detection algorithm could be improved by training a CNN with machine fault detection parameters. A detection algorithm was developed by adding machine fault detection parameters to the down-sampled features after passing the convolution and pooling layers. Upon verifying the detection performance using the AUC, the algorithm was confirmed to demonstrate a high performance (imitation thrombus detection AUC 0.992 and porcine thrombus detection AUC 0.987). Based on this observation, an effective algorithm for detecting pump head thrombosis was developed by adding the machine fault detection parameters to the CNN.
In this study, the performance of the algorithm was improved by applying machine fault detection parameters and a CNN to continuously monitor the thrombus attached to and rotating with the impeller of a centrifugal blood pump. It is envisaged that the methods proposed in this study will improve the survivability of patients by detecting pump head thrombosis in clinical practice. It is also expected that this study will serve as a basis for convergence research that connects the fields of centrifugal blood pumps, machine fault detection, and deep neural network.
원심성 혈액 펌프는 인공적으로 혈액을 순환하는 체외 막 산소 공급 (extracorporeal membrane oxygenation, ECMO)장치 및 심실 보조 장치 (ventricular assist devices, VADs) 등 다양한 의료기기에 사용된다. 그러나 상기 기기는 펌프 헤드 혈전증 (pump head thrombosis)이라는 환자에게 중대한 결함이 발생한다. 이는 환자에게 치명적이므로 즉각적인 조치가 필요하다. 그러나 펌프 헤드 혈전증 감지는 숙련된 의료팀의 시청각 및 혈액 검사에 전적으로 의존한다. 이로 인해 실시간 대응이 어려워 환자의 생존성이 감소한다. 그러므로 펌프 헤드 혈전증을 자동으로 감지하는 알고리즘에 대한 연구가 필요하다. 본 연구의 목적은 원심성 혈액 펌프의 임펠러 (impeller)에 부착되어 함께 회전하는 혈전을 지속적으로 감지하는 알고리즘의 성능을 향상시키는 것이다. 이를 위해 기계 고장을 감지하는 다양한 파라미터 (machine fault detection parameter)들과 심층신경망 (deep neural network) 기반의 합성곱 신경망 (convolution neural network, CNN)을 제안하고 생체 외 테스트로 성능 평가를 실시하여 유효성을 검증했다.
모조 혈전 58개 돼지 혈전 6개를 사용하여 생체 외 테스트 (in-vitro test)를 수행했다. 실리콘을 주조해서 제작한 모조 혈전은 다양한 물리적 특성과 치수를 가지고 있습니다. 한번의 실험에서 하나의 혈전을 사용했다. 혈전을 펌프 헤드 내부에 고정시키고, 펌프를 작동시킨 후, 펌프 헤드에서 발생하는 진동을 가속도센서로 측정했다. 샘플링 속도는 10,000 Hz이다.
첫 번째 단계에서는 기계 고장 감지 파라미터를 이용한 펌프 헤드 혈전증 탐지의 유효성을 검증했다. 임펠러에 혈전 덩어리가 부착 시 임펠러의 무게 중심을 이동시켜 정상적인 중심 위치에서 변위가 발생한다. 본 연구에서는 상기 원리를 고려하여 제곱 평균 제곱근 (root mean square, RMS), 비대칭도 (skewness), 첨도 (kurtosis), 형상 계수 (shape factor), 파고율 (crest factor), clearance factor, impulse factor, 에너지 연산자 (energy operator), 에너지비 (energy ratio), zero-order figure of merit (FM0), fourth-order figure of merit (FM4), M6A, M8A로 구성된 기계 고장 감지에 사용하는 파라미터 13개를 선정했다. 생체 외 테스트에서 측정한 진동 신호를 상기 파라미터들로 변환하고, 에이다 부스트 (adaptive boosting, AdaBoost)로 학습하여 특징 기반 분류법 (feature-based classification) 기반의 탐지 알고리즘을 개발하였다. 탐지 성능은 수신자 조작 특성 (receiver operating characteristics, ROC)의 아래 면적 (area under the ROC Curve, AUC)으로 평가하였다. 분석 결과 각각의 기계 고장 진단 파라미터 중 에너지비 (모조 혈전 탐지 AUC 0.922)와 첨도 (돼지 혈전 탐지 AUC 0.961)의 성능이 가장 높았다. 상기 파라미터들은 최신(state-of-the-art, SOTA)의 임펠러 회전속도의 3차 고조파 변화 (3rd harmonic change) (모조 혈전 탐지 AUC 0.888, 돼지 혈전 탐지 AUC 0.916)보다 성능이 높았다. 재귀 변수 추가법 (recursive feature addition, RFA)으로 4개의 기계 결함 감지 파라미터들로 구성된 조합을 선정하여 성능을 향상시켰다 (모조 혈전 탐지 AUC 0.9848, 돼지 혈전 탐지 AUC 0.9880). 또한, 최신 (SOTA)의 3차 고조파 변화와 재귀 변수 추가법으로 선정한 기계 결함 감지 매개변수들을 조합하여 혈전 탐지 성능을 더욱 향상시켰다 (모조 혈전 탐지 AUC 0.9853, 돼지 혈전 탐지 AUC 0.9891). 이를 통해 기계 고장 감지 파라미터가 펌프 헤드 혈전증을 탐지하는 객관적인 지표로 이용될 수 있음을 확인했다.
두 번째 단계에서는 합성곱 신경망으로 구현한 펌프 헤드 혈전증 탐지 알고리즘의 유효성을 검증했다. 진동 신호에 단시간 푸리에 변환 (short-time Fourier transform)을 적용하여 스펙트로그램 (spectrogram)으로 변환하고, 이를 매우 깊은 합성곱 네트워크 (very deep convolutional network, VGGNet) 구조의 합성곱 신경망을 학습하여 심층신경망 기반의 탐지 알고리즘을 개발하였다. 수신자 조작 특성의 아래 면적으로 탐지 성능을 평가한 결과 합성곱 신경망 (모조 혈전 탐지 AUC 0.965, 돼지 혈전 탐지 AUC 0.956)이 인공신경망 (artificial neural network, ANN) (모조 혈전 탐지 AUC 0.784, 돼지 혈전 탐지 AUC 0.793) 및 최신의 (SOTA) 에이다 부스트 (모조 혈전 탐지 AUC 0.913, 돼지 혈전 탐지 AUC 0.923) 보다 높은 성능을 확인했다. 이를 통해 합성곱 신경망이 펌프 헤드 혈전증을 탐지하는 효과적인 학습방법임을 확인했다.
세 번째 단계에서는 기계 고장 감지 파라미터들이 추가된 합성곱 신경망으로 학습시킨 펌프 헤드 혈전증 탐지 알고리즘의 성능 향상 여부를 검증했다. 컨볼루션 계층 (convolution layer)와 풀링 계층 (pooling layer)를 지나 다운 샘플링 (down sampling)된 변수 (feature)들에 기계 고장 감지 파라미터들을 추가하고 합성곱 신경망을 학습하여 탐지 알고리즘을 개발하였다. 수신자 조작 특성의 아래 면적으로 탐지 성능을 검증한 결과 합성곱 신경망에 기계 고장 감지 파라미터들을 추가하여 학습시킨 알고리즘의 성능이 향상되었다 (모조 혈전 탐지 AUC 0.992, 돼지 혈전 탐지 AUC 0.987). 이를 통해 합성곱 신경망에 기계 결함 감지 매개변수를 추가하여 펌프 헤드 혈전증을 탐지하는 효과적인 알고리즘을 개발했다.
본 논문은 기계 고장 진단 파라미터와 합성곱 신경망을 적용하여 원심성 혈액 펌프의 임펠러에 부착되어 함께 회전하는 혈전을 지속적으로 감지하는 알고리즘의 성능을 향상시켰다. 본 연구에서 제안한 방법들이 임상에서 펌프 헤드 혈전증을 진단하여 환자들의 생존성을 향상시킬 수 있을 것으로 기대한다. 또한 본 연구가 원심성 혈액 펌프, 기계 고장 진단 및 딥러닝 분야를 이어주는 융합연구의 기반이 되기를 기대한다.
