1960-1980년대 한국의 사회개발 논의와 지역사회보건사업
본 논문은 지역사회보건사업을 통해 한국의 실정에 적합한 대안적인 보건의료체계를 개발하고 의료균점의 과제를 해결하고자 한 일련의 시도를 역사적 관점에서 규명하였다. 지역사회보건사업은 보건의료 자원이 부족한 지역에서부터 모든 사람들에게 기초적인 보건의료 서비스를 제공할 수 있는 체계를 개발한다는 목표를 구현하고자 한 사업이다. 시범사업 형태를 띤 본 사업은 1960년대 후반부터 1980년대까지 의료선교와 의학교육, 정부의 보건개발 차원에서 여러 주체에 의해 실시되었으며, 의료보험만으로 수렴되지 않는 의료보장에 관한 요구와 문제의식을 내포하고 있었다.
1960년대 경제성장을 토대로 의료시설들은 도시를 중심으로 경쟁하며 분화되기 시작하였고 신규로 진입한 의료인들은 선진의학과 안정적인 처우에 대한 지향을 분명하게 표출했다. 하지만 ‘무의촌’으로 대표되는 의료자원 불균형에 대한 정부의 정책은 일관되게 원조에 의존한 보건소 확충과 강제적인 의료인력 동원, 그리고 민간 의료시설 통제로 의료자원의 균점을 달성하는 것이었다. 의료인들은 정부의 부담은 최소화하면서 의료계에 공익성을 요구하는 정부를 비판하면서도, 계속되는 보건의료 격차를 극복할 수 있는 대안을 스스로 제시해야 한다는 책임의식을 갖고 있었다.
지역사회 중심의 보건의료체계 구상은 이와 같은 무의촌 문제와 보건의료의 지역적·계층적 격차를 해결하기 위한 방안 모색의 일환이었다. 그것은 국내외의 지역보건의료사업과 양적성장 중심의 불균형 경제개발의 한계를 극복하려는 여러 층위의 사회개발 논의와 결부되면서 구체화되었다. 이영춘이 주도한 전북 개정지역의 보건의료사업과 농촌위생연구소는 농촌 지역사회를 중심으로 치료와 예방사업, 교육과 연구가 통합된 보건의료 체제를 구축하려는 시도였다. 도시화·산업화에 기반한 경제개발 정책과 의과대학 중심의 의학교육 체계 속에서 농위연의 활동이 보편화되기는 어려웠으나, 1960년대 후반부터 본격화되는 지역사회보건사업의 국내적 기원으로서 의미가 있었다. 또한 농위연이 구축한 시설과 인력들은 1970년대 중반 이후 정부의 지역사회보건 시범사업의 기반이 되었다.
1960년대 후반부터 본격화된 사회개발 논의의 주류는 국제적인 사회개발담론의 경향과 달리 1960년대 한국의 경제개발과 근대화를 성공적으로 인식하는 전제 하에 전개되었다. 경제개발의 효율성을 위한 사회개발의 필요성을 주장하는 입장에서 사회정의와 인간적 발전론은 소극적으로 수용되었다. 보건의료계 인사들은 한국의 경제성장을 긍정적으로 평가하면서도 의료의 사회화·인간화 차원에서 보건의료제도 개선과 사회보장제도의 필요성을 주장하고, 사회정의의 주체로서 의료인의 책임성을 강조했다. 특히 기독교계 의료인들은 전인치유론에 입각해 의료 소외지역에서 지역사회 중심 의료사업을 벌이면서 새로운 의료선교 모델을 구상했다. 의료선교사 주도로 시작된 1960년대 후반의 시범사업은 한국 의학자들과의 교류 속에서 실시되면서 의과대학과 선교병원에서 지역사회보건사업이 실시되는 직접적인 계기가 되었다.
1960년대에 국제적으로 부상한 지역사회의학과 이를 기반으로 한 1970년대 일차보건의료 개발원조의 확대는 한국에서 지역사회보건 시범사업이 본격화되는 이론적·물질적 토대가 되었다. 지역사회의학은 전문의 양성 중심의 의학교육과 종합병원 중심의 보건의료시스템을 비판하고 개발도상국의 실정에 맞는 지역사회 중심의 포괄적 보건의료사업을 실시할 것을 제안하였다. 지역사회의학은 새로운 의학의 기조로서 의학·보건학계에 확산되었고, 1970년대 ‘기본적 욕구’ 담론의 확산과 더불어 일차보건의료 원조가 개발원조의 일환으로서 크게 확대되었다.
한국의 의학계는 이와 같은 국내외의 의학적·사회적·국제정치적 맥락을 인식하면서 지역사회의학을 본격적으로 교육과정에 도입하고 지역사회보건 시범사업을 실시하였다. 한국의 의과대학과 선교병원, 국책보건연구기관의 보건의료전문가들은 국제 원조기구와의 네트워크를 통해 원조를 획득해 지역사회보건사업을 실시하면서 의학교육과 연구, 보건의료정책적 대안을 모색하고자 했다. 시범사업은 농어촌 지역을 대상으로 일차적인 수준의 치료와 예방사업을 통해 지역민들의 건강을 관리하고, 이 과정에 주민들이 참여하면서 전반적인 지역의 건강수준을 높인다는 기획 하에 실시되었다.
의과대학은 지역사회의학을 교육과정에 도입함으로써 농어촌으로 대표되는 지역사회의 보건문제를 해결할 수 있는 지역사회의사를 양성할 수 있는가를 가늠하고자 했다. 그러나 현실적으로 전문의 중심의 의학교육을 근본적으로 변화시키는 데에는 이르지 못했다. 반면 간호계는 간호학과 간호업무의 고유성과 독자성을 강조하면서 지역사회간호를 제도화하는 데 적극적이었다. 간호계는 지역사회간호와 일차보건의료를 통해 간호사의 역할범위를 확대하고 지위를 개선하도록 보건정책에 개입하고자 했다. 그것은 지역사회 건강간호사에 해당하는 보건진료원을 제도화하려는 노력으로 이어졌다.
한편 지역사회보건사업에서는 주민참여의 정도가 사업의 성패를 가늠하는 핵심 지표로 인식되었다. 거제 북부 지역에서의 마을건강사업과 예수병원의 완주 용진면 사업은 모두 지역사회 의료선교적 차원에서 면 단위 사업으로 시작된 것으로 주민참여에 기반한 성공적인 시범사업으로 평가되었다. 이 사업들은 주민들의 친밀한 관계와 헌신적 봉사, 공동체성을 토대로 사업을 유지해나갔고, 사업의 지속성을 위한 해외 원조가 상당한 규모로 이루어졌다. 사업을 주도한 의료진들이 지역에서 주민들과 오랫동안 관계를 맺으며 신뢰를 쌓았던 것도 중요한 영향을 미쳤다. 하지만 동시에 이러한 속성으로 인해 체계와 조직만으로 넓은 지역 범위로 확장되거나 다른 지역에 그대로 적용하기는 어려웠다.
거제사업의 경우 사업주체가 바뀌고 군 단위로 사업 범위가 확장되면서 지역사회와의 관계보다는 인력배치와 체계에 집중했고, 후송병원 운영 문제에 집중하면서 기존 마을건강사업은 약화되었다. 예수병원 사업 또한 1980년대 의료환경의 변화 속에서 여러개 면을 포괄하는 고산지역 사업을 수행하면서 지역사회 보건조직의 자립 운영 문제에 부딪쳤다. 특히 보건의료기관이 아닌 지역사회 조직으로서 보건개발회가 지역의 다른 조직들과 어떻게 연대하고 지역의 사회경제적 이슈들에 대처할 것인가에 관한 과제를 남겼다. 이와 같은 지역사회보건사업의 사례들은 “표준적인 접근방식”이란 존재하지 않는다는 일차보건의료사업의 특징을 보여주었다.
한편 정부는 여러 지역에서 실시되고 있는 지역사회보건사업의 문제의식을 경비절약형 보건의료체계 구축으로 수용하여 구현하고자 했다. 사회개발 정책으로서 제시한 보건의료서비스 확대계획은 경비 부담을 최소화하고 효율성을 높이는 ‘저렴보건의료체계’로 표현되었다. 그것은 일차보건의료 인력과 제도를 구축하고, 해외 사회개발 차관원조로 민간병원 건설을 확대하는 것이었다.
일차보건의료 인력과 시설 구축은 농어촌을 중심으로 한 의료취약지역에 한정되었고, 일반법의 예외를 제도화하는 ‘특별조치법’이라는 틀 안에서 행해졌다. 차관을 매개한 의료취약지역 의료기관 확대와 시설의 현대화는 차관을 받은 민간기관의 자본과 역량에 맡기는 방식이었다. 정부의 민간병원건설 지원 정책 속에서 병원은 예방보다 치료사업에 치중하면서 의료수요가 보장된 곳으로 집중되었고 대형병원과 소형 병의원들간의 경쟁과 갈등 속에서 의료시설의 양극화를 심화시켰다.
1980년대 민간 의료시장의 확대와 치료중심 체계의 공고화 속에서 시범사업을 실시한 지역에서는 의료전달체계 구축을 통해 일차보건의료의 의료균점에 관한 문제의식을 유지해나가고자 했다. 그러나 주민들의 치료요구와 대형병원들의 수도권집중 속에서 보편적으로 의료혜택을 제공해야 한다는 기본권으로서의 건강권과 의료균점의 과제는 의료비를 중심으로 하는 의료보험 요구로 집중되었다. 의료의 사회화와 의료보장을 실현하기 위한 대안적 방법으로서의 지역사회 건강의 문제의식은 일부 공공보건의료제도와 시민운동·지역운동의 영역에서 제한적으로 남게 되었다.
This paper examines community health projects as a series of attempts to develop an alternative health care system suitable for Korean social-economic conditions and to address the challenges of medical disparities from a historical perspective. The community health project was an experimental work to realize the goal of developing a system that could provide basic health care services to all people, even in areas with limited health care resources. These projects were implemented by various actors in medical missions, medical education, and governmental health development from the late 1960s to the 1980s, and they addressed needs and concerns about health coverage that did not converge on health insurance alone.
Along with the economic growth in the 1960s, medical facilities began to compete in cities, and new medical entrants clearly expressed their desire for advanced medicine and stable treatment. However, the government's response to the imbalance of medical resources represented by the "Doctorless Villages" was to rely on aid to expand health centers, forcibly mobilize medical personnel, and control private medical facilities to achieve a balance of medical resources. While medical professionals criticized the government for demanding public service from the medical profession while minimizing the burden on the government, they also felt a sense of responsibility to come up with their own alternatives to overcome the continuing healthcare disparities.
The community-centered healthcare system initiative was part of the search for ways to address the problem of doctorless villages and regional disparities in healthcare. It materialized in conjunction with local healthcare projects in domestic and international levels,, and social development discussions to overcome the limitations of unbalanced economic development centered on quantitative growth. The health program led by Lee Young-chun were attempts to build a healthcare system that integrated treatment and prevention, medical education and research, centered on rural communities. Although it was difficult to become widespread in the economic development policies focused on urbanization and industrialization, it was significant as the domestic origin of community health projects. In addition, the facilities and personnel established became the basis for the government's community health pilot projects in the mid-1970s.
The social development debate, which began in the late 1960s, reevaluated the existing economic development theory and promoted discussions on social development and social justice. Social development theories were divided into two camps: the first camp believed that social development was necessary for efficient economic growth, while the second called for structural social reforms that emphasized social justice and humanization.
Both positions emphasized balance and equity in the development process. In terms of socialization and humanization of healthcare, healthcare professionals discussed the need to improve the healthcare system and social security system. While calling for the primary responsibility of the government for healthcare, they also saw healthcare providers as agents of social justice and should take the lead in addressing healthcare issues. Christian healthcare providers, in particular, envisioned a new model of medical missions by conducting community-centered health projects in medically underserved areas based on the theories of the total Healing and the salvation of the society. The pilot project initiated by a medical missionary was carried out in interation with Korean doctors and was a direct trigger for the implementation of community health projects in medical schools and mission hospitals.
Meanwhile, the international emergence of community medicine in the 1960s and the expansion of primary health care development assistance in the 1970s provided the theoretical and material foundation for community health pilot projects in Korea. Community medicine criticized the medical education centered on specialists training and the hospital-centered health care system, and suggested implementing community-centered comprehensive health care projects that fit the conditions of developing countries. Community medicine spread in the medicine and public health schools as a new tread of medicine, and primary health care aid was greatly expanded as part of development assistance with the spread of the "Basic Needs" discourse in the 1970s.
Recognizing these domestic and international medical, social, and political contexts, the Korean medical professionals has introduced community medicine into the medicine education curriculum and implemented community health pilot projects. Healthcare professionals from medical schools, mission hospitals, and the national health research institute sought to explore alternatives in medical education, research, and healthcare policy by implementing the projects with aid from international organizations. The pilot project was designed to improve the health of the local population through primary-level treatment and prevention in rural areas by involving the local population.
Medical schools tried to test whether they could train community doctors who could solve the health problems of rural communities by community medicine education. However, community medicine education did not fundamentally change the medical education system centered on specialists. The nursing profession, on the other hand, has been active in institutionalizing community health care, emphasizing the uniqueness and professionalism of nursing. Nurses sought to intervene in health policy to expand the scope of nurses' roles and improve their status through community health care and primary health care. This led to efforts to institutionalize the Community Health Practitioner as community health nurse.
In community health projects, the degree of participation of residents was considered important to determine the success of the project. The community health projects in northern Geoje by the Sibley-led team and in Yongjin project by Jesus Hospital (Presbyterian Medical Center)
were successful in terms of community participation, as they combined community medical missionary efforts with community involvement. These projects were sustained by the close relationships with the community, dedicated service, and community spirit, and received substantial foreign aid. It was also important that the health workers who led the projects had long-standing relationships and trust with the local population. In this sense, it is difficult for these system and organization to expand to a large area or to be applied to other areas. In addition, the problem of self-sustaining of a community health organization was raised amid changes in the medical environment in the 1980s. These examples of community health projects illustrate a characteristic of primary health care that there is no standard approach.
The government acknowledged the concerns raised by community health projects and endeavored to address them through the establishment of a low-cost healthcare system. Envisaged as a social development policy, the health plan aimed to construct a healthcare system that would minimize costs while enhancing efficiency. The strategy involved the development of primary healthcare personnel and systems, along with the expansion of private hospitals through international social development loans.
The construction of primary health care personnel and facilities was limited to medically underserved areas, mainly rural areas, and was carried out with the Act on Special Measures, which institutionalized exceptions to the general law. The expansion of health institutions and modernization of facilities in underserved areas was dependent on the capital and capacity of the private institutions that received overseas loans. Under the government's policy of supporting the construction of private hospitals, hospitals focused on treatment rather than prevention, concentrating on places with guaranteed medical needs and deepening the polarization of medical facilities through competition and conflict between large and small hospitals.
The community health projects attempted to build a comprehensive healthcare and healthcare delivery system that combines prevention and treatment as a method of equal medical distribution amid the expansion of the private healthcare market and the consolidation of the treatment-oriented system in the 1980s. However, amid the treatment needs of residents and the concentration of large hospitals in the metropolitan area, the right to health as a basic right and the task of equal healthcare distribution were concentrated on the demand for health insurance centered on medical expenses. The issue of community health as an alternative method to realize the socialization of healthcare and healthcare security remained limited in some public healthcare systems and civil and local movements.
