소아 감염증 환자에서 Augmentain®(Amoxicillin/Clavulanic acid)의 임상효과 = Clinical Experience with Augmentain® in the Treatment of Pediatric Infectious Diseases
Augmentin® is a combination drug of amoxicillin and clavulanic acid in a 4 : 1 ratio. Clavulanic acid is a potent inhibitor of β-lactamase produced by cretain gram-positive and gram-negative bacteria. Clavulanic acid alone exhibited a low level of antibacterial activity against most bacterial strains. However, amoxicillin has been significantly more active in the presence of clavulanic acid against a wide range of β-lactamase producing bacteria, including isolates of S. aureus, H. influenzae, Proteus species and B. fragilis than either amoxicillin or clavulanic acid alone.
We report here our experience with Augmentin® in the treatment of various pediatric infectious diseases.
Augmentin® was administered to 60 infants and children aged 1mo. to 14 yrs. in open trial. There were 22 cases with acute enterocolitis, 5 cases with Shigellosis, 12 cases with urinary tract infection, 16 cases with pneumonia, 3 cases with acute pharyngotonsillitis, 1 case with Salmonellosis, and 1 case with sepsis.
Inntially Augmentin was administered intravenously at a dose of 60!100mg/kg/day divided into three doses and then per oral at dose of 30~50mg/kg/day divided into three doses. The mean duration of overall therapy was 8.1 days.
The clinical response was satisfactory (effective+partially effective) in 5(100%) of 5 patients with Shigellosis, 21(95%) of 22 patients with acute enterocolitis, 10(83%) of 12 patients with urinary tract infection, 15(94%) of 16 patients with pneumonia, and 3 of 3 patients with acute pharyngotonsillitis. So overall satisfactory clinical response was recorded in 54(90%) patients.
22 organisms were cultured from 20 patients. Twenty were assessable and a successful results(success+partial success) achieved in 16(80%). Stool culture showed S. flexneri in 5 cases, and the bacteriological assessment was successful in all 5 cases.
Urine culture showed E. coli, K. pneumoniae, P. mirabilis and S. 리exneri, of 7 E. coli isolates, successful results were achieved in 6(87%). Successful results were achieved in of 4 K. pneumoniae isolates.
In 5 cases of Shigellosis, all isolated pathogens, S. flexneri, were resistant-to ampicillin, but sensitive to Augmentin® . In 6 cases of urinary tract infection isolated organisms, E. coli. were resistant to ampicillin, but sensitive to Augmentin®
Diarrhea occurred in 6 patients(10%) as a side effect, but Augmentin® therapy was not discontinued because of diarrhea. Skin rash developed in one patient(1.7%) and the therapy was discontinued immediately.
