Computer-Aided Design/Computer-Aided Manufacturing (CAD/CAM) has introduced revolutionary changes in dental clinic area. Previously, casting dental model followed by manual crafting stone models was typical process in dental clinics, which requires longer manufacturing time and physical spaces to store them. Using CAD/CAM in dental clinic area, however, it is much easier to perform all the steps in detal clinic process; from diagnosis to treatment, and it is not necessary to save physical models in storage and is possible to reproduce them whenever needed from the scanned data, therefore no extra efforts are needed to avoid breakages or deformations.
Among these CAD/CAM approaches, subtractive manufacturing approach has drawbacks, such as more waste from subtractive process itself and impossible to produce morphologically difficult models. Additive manufacturing (AM) methods, however, use only required materials to cumulate into 3-D models, and can manufacture complicated models easily.
Of these AM methods used for dental models, 1) SLA (Stereo Lithography Apparatus) method builds the three-dimensional (3-D) shapes by accumulating cured layers of liquid resin in water bath by laser beam, 2) PolyJet (Photopolymer Jetting) method uses liquid photopolymers and solidifies them with UV light into 3-D models by photocure process, and 3) FDM (Fused Deposition Modeling) method extrudes liquified plastic material thorough a nozzle and builds 3-D models.
Although these AM methods have various advantages over the conventional method, their uneven surfaces induce less accurate models and less effective dental clinic process. In order to replace the conventional stone model, along with attempts to minimize these problems, it is necessary to validate the accuracy of the current AM technology.
Even there have been active research efforts on the accuracy of the AM models, less attempts to measure them in digitalized 3-dimensions. As the standard approach to accuracy, ISO standard about the accuracy (5725-1 and 12836) was applied, which specifies that accuracy has two factors; trueness as quantified systemic errors and precision as quantified random errors. In other words, the trueness is the measure of deviation from the given reference and precision is the measure of deviation from repeated measurements in the same group.
The research aim of this study is to compare the trueness and precision of the full-arch models by four AM thethods, such as, FDM method, which is already commercialized in orthodontics area, SLA method, which is commercialized in some prosthesis area, and two PolyJet methods. Therefore, with the results of this comparison, I will suggest which AM methods can be applied in which area of dental clinics.
The purpose of the first study is to evaluate the accuracy (trueness and precision) of full-arch models fabricated using additive manufacturing methods such as PolyJet and FDM which are commonly used in orthodontic area due to enough accuracy with low costs.
The experimental method is following: a reference data was acquired from the maxillary full-arch model using extraoral scanner (Identica blue®, Medit, Seoul, South Korea), 10 stone models (Everest Rock, KaVo Dental GmbH) were acquired for the control group by scanning a maxillary full-arch model and as for the experimental group, 10 PolyJet models (PolyJet 3500 HD Max, 3D systems, Rock Hill, SC, USA) and 10 FDM models (Cube®, 3D systems, Rock Hill, SC, USA) were fabricated from digital impressions using an intraoral scanner. All 30 models were then scanned using extraloral scanner (Identica blue®, Medit, Seoul, South Korea) and RMS (Root Mean Square) values were measured as trueness and precision using three-dimensional analysis software. Both trueness and precision are compared with one-way ANOVA and post-hoc Tukey’s test and were validated the statistical significance. Moreover, using digital microscope the roughness of the surfaces and the thickness of the layers were measured to double-check the relationship to the accuracy.
The result is that trueness did not significantly differ between the stone and PolyJet models (p>.05), but precision of the AM models was significantly higher than that of the stone models (p<.05). As the FDM models had much rougher surfaces than other models, the layer thicknesses of the FDM models were also greater than other models.
The purpose of the second study is to evaluate the trueness and precision of full-arch AM models using SLA and PolyJet, which are used in manufacturing complicated parts and precise operations in prosthesis area.
The experimental method is following: a reference model were acquired by scanning the maxillary full-arch model using extraoral scanner (Identica blue®, Medit, Seoul, South Korea), 10 stone models (Jade stone, Whip Mix Corp, Louisville, KY, USA) were acquired for the control group by scanning a maxillary full-arch model and as for the experimental group, we acquired digital data using an intraoral scanner (CEREC Omnicam) and made 10 SLA models (projet®6000, 3D systems, Rock Hill, SC, USA) and 10 PolyJet models (Objet30 OrthoDesk, Stratasys, Eden Prairie, MN, USA). All models were then scanned by using an extraoral scanner (Identica blue®, Medit, Seoul, South Korea) and saved as STL files. Three-dimensional analysis software was used to measure the difference between the 3-D images in RMS (Root Mean Square) values. One-way ANOVA and Tukey’s post hoc analysis were applied to the trueness and precision values.
The result is that there is no significant difference in trueness between stone model and SLA model (p>.05) and, contrary to the first study, there was a significant difference in trueness between stone model and PolyJet model (p<.05). Also, dental replica models appeared to have statistically significant better precisions than that of stone model (p<.05). In tolerance, all three groups showed that the under-tolerance rates were higher than the over-tolerance rates, showing shrinkage tendency.
Summarizing the two study results, the FDM method can be applied in orthodontics area, especially orthodontic diagnosis model and Invisalign aligner. The PolyJet method can be applied in orthodontics and in some prosthesis area. The SLA method is more proper to be applied in dental surgery and prosthesis system requiring high trueness.
This implies that AM models are not ready to replace the stone models, however, they can be acceptable in some area with the proper accuracy in clinical settings.
치과임상에 computer aided design/computer aided manufacturing (CAD/CAM)시스템이 도입되면서 혁신적인 변화가 일어나고 있다. 기존의 치과 진료에서는 인상채득 후 stone model을 제작하는 작업이 이루어 졌는데, 이 방식은 제작까지 많은 시간이 소요되고 만들어진 stone model을 보관 할 수 있는 대규모 공간이 필요했다. 반면에 CAD/CAM시스템을 이용하면 실물 모델의 제작 시간이 대폭 줄어들고, 스캔 및 디자인된 모델이 디지털 파일로 컴퓨터에 영구 저장됨에 따라 실물 모델을 보관할 장소가 필요 없게 되고 변형과 파절 우려가 없어졌다.
CAD/CAM시스템 중 절삭가공 방식은 재료를 깎아서 원하는 형태를 만들기 때문에 남는 재료가 많고, 복잡한 형상을 재현해 내지 못하였다. 반면에 적층가공 (additive manufacturing; AM) 방식은 원하는 형태만큼의 재료를 사용하여 쌓아 3차원 모델을 제작하기 때문에 재료의 낭비가 없고, 절삭가공 방식으로는 불가능했던 복잡한 형상의 재현이 가능하다.
현재 치과분야에서 사용되고 있는 대표적인 AM 방식 으로는 1) 액상 수지가 담긴 수조에 레이저 빔을 주사하여 경화를 진행하면서 적층하여 원하는 형태를 만드는 SLA (Stereo Lithography Apparatus) 방식, 2) 액상 광경화성 수지를 노즐에서 분사한 후 광 에너지를 이용하여 굳혀 적층하는 PolyJet (Photopolymer Jetting) 방식, 그리고 3) 열가소성의 플라스틱 재료를 녹여 노즐에서 분사하여 적층하는 FDM (Fused Deposition Modeling) 방식이 있다.
AM 방식으로 제작된 model은 stone model에 비하여 많은 장점을 가지고 있지만, 고르지 못한 표면으로 인해 모델의 정확도가 떨어지게 되고, 치료 효과와 효율을 떨어뜨리는 단점이 있다. stone model을 대체하기 위해서는 이러한 단점을 최소화하는 기술 개발과 동시에 정확도에 대한 지속적인 검증이 필요하다.
AM model의 정확도 (accuracy)에 관한 연구는 활발히 진행 중 이지만, AM model을 삼차원으로 디지털 분석한 연구는 아직 미비하다. 따라서 본 연구에서는 ISO (5725-1, 12836)의 규격에 따라 삼차원 디지털 분석을 하였고, 이 규격에서 accuracy는 trueness와 precision 을 포함한다. trueness는 기준이 되는 모델과 각 그룹의 모델 사이의 오차를 말하고, precision은 같은 그룹의 모델 사이의 오차를 말한다.
본 연구의 목적은 현재 치과 임상에서 적용이 가능한 4가지 AM 방식 중 치과 교정분야에서 상용화 되고 있는 FDM 방식과 치과 보철분야에서 상용화 되고 있는 SLA 방식과 2가지 PolyJet 방식으로 제작한 full-arch model의 trueness와 precision를 비교 평가하는 것이다.
그 결과를 토대로 각 AM 방식이 치과 임상에서 어떤 시스템에 적용될 수 있는지 제시하고자 한다.
첫 번째 연구의 목적은 치과 교정분야에서 충분한 정확도와 낮은 비용으로 가장 많이 사용되고 있는 AM 방식 중 FDM 방식과 PolyJet 방식으로 제작된 full-arch model의 trueness 와 precision을 비교 평가하는 것이다.
실험방법은 상악의 full-arch model을 사용하여 실험하였다. 대조군으로 기존의 방식을 통해 10개의 stone model (Everest Rock; KaVo Dental GmbH)을 제작하였다. 실험군은 구강 내 스캐너를 이용하여 채득한 디지털 데이터를 통해 10개의 PolyJet model (PolyJet 3500 HD Max, 3D systems, Rock Hill, SC, USA)과 10개의 FDM model (Cube®, 3D systems, Rock Hill, SC, USA)을 제작 하였다. 제작된 30개 모델 모두를 스캔 한 후 3차원 분석 소프트웨어를 사용하여 RMS (Root Mean Squared)값을 측정하여 trueness와 precision을 구하였다.
Trueness와 precision은 one-way ANOVA와 post-hoc Tukey’s test를 통하여 비교되었다. 추가로 디지털 현미경을 이용하여 accuracy와 관련이 있는 표면의 굴곡과 layer의 두께를 측정하였다.
연구결과로 trueness에서는 stone model과 PolyJet model 사이에서 통계적으로 유의한 차이가 없었고 (p>.05), PolyJet model과 FDM model이 stone model보다 precision이 높았다. FDM model이 다른 방식에 비해 표면굴곡이 매우 컸는데 두께 측정에서도 FDM model이 상당히 두꺼운 layer 두께가 측정되었다.
두 번째 연구의 목적은 치과 보철분야에서 복잡한 부품 제작과 정밀한 작업에 이용되고 있는 AM 방식 중 PolyJet 방식과 SLA 방식으로 제작된 full-arch model의 trueness와 precision을 비교 평가하는 것이다.
실험방법은 상악의 full-arch model을 사용하여 실험하였다. 대조군은 기존의 방식을 통해 10개의 stone model (Jade stone, Whip Mix Corp, Louisville, KY)을 제작하였다. 실험군은 구강 내 스캐너를 이용하여 채득한 디지털 데이터를 통해 10개의 SLA model (projet®6000, 3D systems, Rock Hill, SC, USA)과 10개의 PolyJet model (Objet30 OrthoDesk, Stratasys, Eden Prairie, MN, USA)을 제작 하였다. 제작된 모델 모두를 스캔 한 후 3차원 분석 소프트웨어를 사용하여 RMS (Root Mean Squared)값으로 trueness와 precision을 구하였다.
연구결과로 trueness에서는 stone model과 SLA model 사이에서 통계적으로 유의한 차이가 없었고 (p>.05), 첫 번째 연구 결과와 다르게 두 번째 연구에서는 다른 재료의 stone model과 PolyJet model의 사용에서 stone model과 PolyJet model이 통계적으로 유의한 차이가 있었다 (p<.05). 또한 SLA model과 PolyJet model이 stone model보다 precision이 높았다. 세 그룹 모두 Under-tolerance 비율이 높은 수축의 경향을 보였다.
이 결과를 토대로 AM 방식의 치과 임상적용을 검토하면 FDM 방식은 치과 교정분야에서 적용할 수 있으며 특히 치열 교정 목적을 위한 진단용 model과 Invisalign aligner에서 적용도가 높다. PolyJet 방식은 치과 교정분야와 일부 치과 보철분야에서 적용할 수 있으며 높은 trueness를 요구하는 외과적 치료와 치과 보철분야의 시스템에는 SLA 방식이 적합하다.
따라서 현재의 기술력으로는 치과 임상에서 AM model은 stone model을 완전히 대체하여 사용할 수 없지만, trueness와 precision의 요구도에 따라 적합한 시스템에 적용 가능할 것이다.
