요양병원 노인환자가 지각한 절망감과 가족지지에 관한 연구 = A Study on Perceived Hopelessness and Family Support among the Elderly Patients

국문 초록 (Abstract)

연구목적: 요양병원에 입원한 노인환자의 절망감과 가족지지와의 관계를 파악하고 절망감에 영향을 미치는 요인을 파악하여 요양병원 입원 노인의 삶의 질 향상을 위한 간호중재 개발에 기초자료를 제공하고자 시도되었다. 연구방법: 본 연구는 요양병원 노인 환자가 지각한 절망감과 가족지지에 대해 알아보는 서술적 조사 연구이다. 연구대상은 전라남도와 광주광역시 지역 2개의 노인 요양병원에 입원중인 65세 이상의 환자로 본 연구에 참여할 것을 서면으로 동의한 자를 대상으로 하였다. 연구도구는 절망감의 측정도구는 Beck과 Weissman & Lester, Trexler(1974)에 의해 개발된 Beck Hopelessness Scale (BHS)로 신민섭(1990)이 번안한 도구를 사용하였고 가족지지의 측정도구는 Cobb (1976)가 개발하고, 강현숙(1984)이 번안한 가족지지 측정도구를 사용하였다. 자료수집은 C대학 IRB(13-006) 승인 후 진행되었으며, 자료수집기간은 2013년 2월 1일부터 3월 31일까지였다. 수집된 자료는 SPSS/WIN 21.0 프로그램으로 분석하였다. 대상자의 일반적 특성은 빈도와 백분율로 분석하였고 대상자의 가족지지와 절망감 정도는 평균과 표준편차로 분석하였다. 대상자의 일반적 특성에 따른 절망감과 가족지지 정도는 independent t-test와 one-way ANOVA, 사후검증은 scheffé test을 이용하였다. 대상자의 절망감과 가족지지와의 관계를 알아보기 위해 Pearson's correlation coefficients를 분석하였으며 대상자의 절망감에 미치는 영향요인을 알아보기 위하여 단계적 회귀분석(stepwise regression analysis)을 실시하였다. 연구결과: 대상자의 절망감 정도는 평균평점 3.41점(SD=13.46)이었다. 가족지지의 정도는 평균평점 3.04점(SD=9.35)으로 나타났다. 대상자의 일반적 특성에 따른 절망감과 가족지지의 정도를 분석한 결과, 절망감 정도에서 유의한 차이를 나타낸 항목은 종교(t=-2.79, p=.006), 입원기간(F=5.37, p<.001), 경제상태(F=15.01, p<.001), 용돈 만족도(F=7.65, p<.001)등이었고 대상자의 가족지지는 교육정도(F=5.18, p=.002)와 종교(t=3.77, p<.001), 병원비 부담(F=2.54, p=.041), 경제상태(F=8.08, p<.001), 용돈 만족도(F=13.95, p<.001)에 따라 느끼는 가족지지 정도에 유의한 차이가 있었다. 본 연구에서 대상자의 절망감은 가족지지(r=╶.40, p<.001)와 유의한 부적 상관관계가 있는 것으로 나타났다. 회귀분석 결과 노인의 절망감에 영향을 미치는 요인은 경제상태(β=.23, p<.001), 입원기간(β=.18, p=004) 그리고 가족지지(β=-.31, p<.001)가 유의미한 예측력을 가지는 변인으로 나타났다. 결론: 본 연구는 요양병원 입원 노인환자가 지각한 절망감과 가족지지의 정도를 파악하고 이에 영향을 미치는 변수를 규명함 하는 것으로 본 연구결과 요양병원 입원 노인의 절망감 감소에 영향을 미치는 요인은 높은 가족지지, 입원기간, 경제상태의 여유로움임을 알 수 있었다.

다국어 초록 (Multilingual Abstract)

Purpose: This study was done to identify to examine the degrees of the perceived hopelessness and family support among the elderly patients in Long-Term Care Hospitals and the relationship among the variables. Methods: Subjects were 226 elderly patients with the age over 65 years in 2 long-term care hospitals located in Gwangju-si & Jeollanam-do. To investigate perceived hopelessness and family support of subjects in long-term care hospitals, a structured questionnaire was used. to measure the level of hopelessness, the instrument developed by Beck & Weissman, Lester & Trexler (1974) (1974) and adapted by Shin(1984) was used. To survey the family support, the instrument developed by Cobb(1976) and modified by Kang(1984) was used. Data were collected from February 1 to March 31, 2013. Data were analyzed by frequency, percentage, mean, standard deviation, t-test, ANOVA, Pearson's correlation coefficients, and, stepwise regression analysis using the SPSS program. Results: Findings showed negative correlation with perceived hopelessness and family support in subjects in long-term care hospitals(r=╶.40, p<.001). Concerning the perceived hopelessness compared between subjects in long-term care hospitals, significant differences were found in Family Support, Period of hospitalization, Economic state. Conclusion: Based on the findings, significant differences were found in perceived hopelessness and family support among the elderly patients in long-term care hospitals. The family support are needed to decrease of hopelessness of the elderly. Thus, the general characteristics of elderly should be considered to individual intervention programs developing to decrease hopelessness and improve family support
