도시 및 농촌 지역 주민의 정신건강 수준에 대한 연구 = A Study on Mental Health States of the Urban and Rural Residents

연구목적:
증가하고 있는 정신건강 문제를 효과적으로 해결하기 위해서는 지역사회 중심의 체계적인 정신보건사업이 필요하다. 이를 위해 대도시, 중소도시, 농촌 지역을 대상으로 이들 지역 주민의 정신건강 수준과 이에 영향을 주는 주요 인구사회학적 변인을 분석 하고, 정신질환의 고위험군 집단을 파악해 지역정신보건 사업의 우선 순위 대상을 선정하고자 하였다.
방 법:
광주광역시, 목포시, 강진군의 18세 이상 주민을 대상으로 각 행정 단위에서 무작위 추출하여 총 1716명을 대상으로 하였다.
정신건강 수준을 측정하기 위한 집단 선별조사 도구로 일반건강검사(GHQ)를 사용하여 설문지를 통한 면접조사를 실시하였다.
대상인구의 인구사회학적 특성을 조사하여 정신건강 수준과의 상관관계를 분석하였고, 정신장애 위험군을 분류하였다.
결 과:
거주지역별로는 광주, 강진, 목포 순으로 정신건강 수준이 높았다. 30대와 40대의 전반적인 정신건강 수준이 유의하게 좋았고, 60대 이상은 신체증상, 50대는 불안 및 불면증이 유의하게 높았다. 반면 사회적 부적응은 20대 이하가 가장 높고, 60대 이상이 가장 낮았다. 결혼 후 이혼, 별거, 사별 등의 이유로 배우자 없이 혼자 사는 대상군이 신체증상, 불안 및 불면증, 우울증 등의 문제를 많이 갖고 있는 정신건강 문제의 고위험군으로 나타났다. 기타 종교는 기독교나 불교에 비해 사회적 부적응이 유의하게 높은 정신질환의 고위험군이었다. 학력이 낮거나 직업이 없거나 월세 거주하는 교육·경제 수준이 낮은 이들의 전반적인 정신건강 수준이 낮았고 정신질환의 고위험군으로 분류되었다. 정신과 치료의 가족력과 개인력이 있는 경우도 정신건강 수준이 유의하게 낮고 정신질환의 고위험군이었다. 반면 여론주도층은 정신건강 수준이 유의하게 좋았다.
결 론:
교육·경제 수준이 낮은 주민, 정신과 치료의 가족력이나 개인력이 있는 주민, 결혼 후 이혼, 별거, 사별한 주민, 기독교, 천주교, 불교 등의 주요 종교가 아닌 기타 종교를 갖고 있는 주민 그리고 중소도시 거주자의 정신건강 수준이 유의하게 낮거나 정신장애의 고위험군으로 나타났다. 따라서 이들을 대상으로 한 지역사회 정신보건 대책을 우선적으로 개발하고 적절한 정신보건 프로그 램을 제공할 필요가 있다고 본다.
Objective:Increasing mental health problems need systematic community mental health service. This study was performed to get basic data in order to develop a plan and strategy for community mental health services. The purposes of this study were to investigate the mental health states of a large community, including both urban and rural areas, and to identify the factors influencing their mental health states.
Methods:A total of 1716 subjects were selected through stratified random sampling among residents of Gwang-ju city, Mok-po city, Gang-jin gun, who were 18 years and older. The General Health Questionnaire(GHQ) was used to evaluate the mental health states. The survey was administered using a paper copies. The mental health states were analyzed by overall mental health state and four subscales:somatic symptom, anxiety and insomnia, social dysfunction and severe depression. The socio-demographic characteristics influencing mental health were found out and the high risk groups of mental health were discovered.
Results:1) Residents of Gwang-ju city were significantly better in overall mental health states than residents of other regions.
2) Overall mental health states of subjects in their 30’s and 40’s were relatively good. The somatic symptom subscale score was highest for those in their 60’s and older, and anxiety and insomnia subscale was highest for those in their 50’s.
On the other hand, social dysfunction was shown to be highest in subjects in their 20’s and younger but lowest in for those 60’s and older.
3) Those who were divorced, separated or bereaved from their spouse showed significantly worst overall mental health as well as more somatic symptom, anxiety and insomnia and severe depression; while those who were single showed more social dysfunction.
4) Subjects who followed a religion other than the major religions which were Christianity, Catholicism and Buddhism showed more social dysfunction.
5) Residents of low socioeconomic status such as low education levels, unemployment or living with monthly rent were relatively worse in overall mental health states and scored high in most subscales.
6) The opinion leaders had significantly better overall mental health states and low social dysfunction.
7) Subjects who had family or personal experiences of psychiatric treatment were significantly worse in their overall mental health states.
8) Comparing the high risk group to the non-risk group, those who divorced, bereaved or separated from spouse, had a religion that was not major religion, lived in Mok-po city or paid monthly rent, were unemployed, and had experienced with psychiatric treatment, were more apt to have mental disease.
Conclusions:The high risk groups of mental illness had the following characteristics:low education or economic level, family or personal histories of psychiatric treatments, divorce or separation during marriage, having a religion that was not major religion and residents in small-sized city. Therefore, appropriate community mental health programs should be provided for these high risk people.
