13년간 신종양으로 신절제술을 시행한 경우를 대상으로 신세포암 외의 신종양으로 수술 후 밝혀진 증례들을 수집하여 과연 술전 신세포암 외의 다른 진단은 불가능하였는지 후향 분석하였다.
더보기Objective : With greater use of ultrasound, CT, and MRI, the ability to detect renal tumors earlier and in lower stages has now significantly increased. Most of solid renal tumors should be thought as malignant renal cell carcinoma until proven otherwise. The authors reviewed unusual renal tumors after redical nephrectomy for presumed renal cell carcinoma.
Methods and Materials : We reviewed the pathologic findings in patients with surgically confirmed renal mass. One hundred twenty seven patients underwent nephrectomy for renal mass suspicious for renal cell carcinoma and were retrospectively analyzed, excluding all patients preoperatively felt to have non-renal cell pathology, from 1985 to 1998.
Results: One hundred fourteen patients(89.8%) were confirmed to have renal cell carcinoma. Thirteen patients(10.2%) were found to have other pathology. Non-renal cell, benign renal mass was 7 patients(53.8%) ; angiomyolipoma 2, oncocytoma 1, lipoma 1, leiomyoma 1, medullary fibroma 1 and mesoblastic nephroma 1. Other malignant renal rrlass was 6 patients(46.2%) ; Wilms' tumor 2, transitional cell carcinoma of renal pelvis 1, and atypical mixed cell carcinoma 1. There was no difference in mass size, radiologic findings and preoperative stage between renal cell carcinoma and non-renal cell mass. Preoperative differential diagnosis was difficult wish radioloigic studies. In spite of thorough preoperative radiologic imaging, 10.2% of patients undergoing nephrectomy to suspect renal cell carcinoma have other pathologic diagnosis, and 46.2% of these patients are found to have other malignant tumors.
Conclusion : At the time of preoperative diagnosis of the renal cell carcinoma, urologic surgeon should be prudent because the incidence of non-renal cell, benign and malignant renal tumor are relatively high. Intraoperative frozen biopsy or preoperative percutaneous needle biopsy may be necessary in case of suspicious non-renal cell renal tumor.
