공공 의료기관 내 어린이병원 공간계획에 관한 연구 = A Study on the Space Planning of Children's Hospitals in the Public Hospitals

국문 초록 (Abstract)

(연구배경 및 목적) 어린이병원은 어린이 환자의 의학적 특수성뿐만 아니라 정서적 돌봄과 신체적으로나 정신적으로 건강한 어른으로 성장하는데 필요한 교육 및 사회화를 위한 경험을 제공할 수 있어야 한다. 그러나 다수의 국내 어린이병원은 성인과 차별화된 치료 공간을 제공하지 못하고 있으며, 현 의료법의 병원 시설 기준은 최소한의 내용만 규정하고 있다. 이에 성인과는 다른 요구가 존재하는 어린이 의료시설을 위한 별도의 가이드라인이 필요하다. 특히, 출산율이 감소하고 있는 상황에서 공공의료 부문에서 더 적극적으로 어린이 의료 서비스를 책임져야 한다. 따라서 본 연구를 통해 공공병원과 결합된 어린이 환자의 특수성을 고려한 병원 설계를 제안하고, 어린이 전문 치료 공간의 설계에 반영되어야 할 계획 지침에 대한 시사점을 도출하려 한다. (연구대상 및 방법) 어린이병원, 노인요양시설, 어린이시설을 키워드로 한 선행 연구 고찰을 통해 어린이병원 계획 시 고려해야 할 공간적 이슈를 파악하고, 해외 우수 어린이병원 사례분석을 통해 실제로 어떻게 구현되어 있는가를 분석하여 공간적 설계 전략을 도출한다. 특히, 어린이병원, 어린이시설 관련 선행연구를 통해 어린이 환자의 특수성을 확인하고, 노인요양시설 관련 선행연구를 통해 장기간 치료와 거주, 복지 등을 함께 제공하기 위한 공간적 이슈를 파악한다. 최종적으로 수도권 어린이 의료 인프라 분석을 토대로 선정한 대상지에 어린이 환자의 특수성을 고려한 어린이병원의 계획안을 제안하며 본 연구의 시사점을 정리한다. (결과) 어린이 환자의 특수성을 고려한 3가지 공간적 요소는 ① 거주성(프라이버시/ 가족 중심 서비스/ 유니트케어), ② 사회성(놀이 및 학습/ 교류의 기회), ③ 정서성(자연 요소/ 어린이 친화적 환경/ 다양한 공간 경험)을 도출되었으며, 사례분석을 통해 구체화된 공간적 전략을 제시하였다. 또한 거주성 측면에서 유니트 케어 시스템의 병동부 공간, 사회성 측면에서 다양한 규모의 교류 공간, 정서성 측면에서 자연 요소를 도입한 어린이 친화적 환경 조성 공간 등의 어린이병원 공간을 계획하였다. (결론) 본 연구에서는 어린이 의료 서비스를 공공의료 부문에서의 책임으로 인지하며 인천시 의료원에 어린이병원 증설을 제안하였다. 계획안은 앞서 도출한 3가지 공간적 요소(거주성, 사회성, 정서성)를 계획 목표로 하여 치료뿐 아니라 어린이의 성장·발달을 지원하는 공간적 전략을 적용하였다. 본 연구는 기존 의료원과의 시설 공유로 의료자원의 효율적 활용을 제안하면서 어린이 환자 맞춤형 의료시설을 제시했다는 데 의의가 있다. 하지만, 이번 연구는 어린이 환자의 특수성을 고려한 어린이병원의 프로그램 구성 및 배치 계획에 초점이 맞추어져 있었기에 각각의 공간에 대한 적정 수준의 질을 규정하는 세부적인 연구가 보완될 필요가 있다.

다국어 초록 (Multilingual Abstract)

(Background and Purpose) Children's hospitals should be able to address the medical needs of young patients, as well as provide emotional care and the educational and socialization experiences they need to grow into physically and mentally healthy adults. However, many children's hospitals in Korea do not provide a treatment space that is differentiated from that of adults, and the current healthcare law's hospital facility standards are minimal. There is a need for separate guidelines for children's healthcare facilities, which have different needs than adults. Especially in the context of declining birth rates, the public healthcare sector should take more active responsibility for children's healthcare services. Therefore, this study proposes a design for children's hospitals that considers the specificity of children's patients combined with public hospitals and draws implications for planning guidelines for specialized children’s treatment spaces. (Method) Through a review of previous studies on children's hospitals, elderly care facilities, and children's facilities, spatial issues that should be considered in planning children's hospitals are identified, and spatial strategies are derived by checking how they are implemented in practice through analysis of overseas best practices. Especially, research on children's hospitals and children's facilities identifies the special characteristics of child patients, and prior research on elderly care facilities identifies spatial issues for providing long-term care, residence, and welfare together. Finally, based on the analysis of the children's medical infrastructure in the metropolitan area, a plan for a children's hospital that considers the special needs of children is proposed for the selected site, and the implications of this study are summarized. (Results) Three spatial factors were identified as spatial issues considering the special needs of children: ① habitability(privacy/family-centered services/unit care), ② sociality(opportunities for play and learning, and interaction), and ③ emotionality(natural elements/child-friendly environment/various spatial experiences), and spatial strategies were refined through case analysis. This study also presents a spatial plan for children's hospitals by creating a unit care system for ward areas(habitability), social spaces of various scales(sociality), and child-friendly spaces incorporating natural elements(emotionality). (Conclusions) This study proposed expanding a children's hospital at Incheon City Medical Center, recognizing children's healthcare as a public healthcare sector responsibility. This plan is based on three spatial factors (habitability, sociality, and emotionality), and applies spatial strategies that support not only medical treatment but also the growth and development of children. The significance of this study is that it proposed an efficient utilization of medical resources by sharing facilities with existing medical centers, and proposed a medical facility tailored to children. However, as this study focused on the program and layout planning of a children's hospital, it needs to be supplemented with a detailed study that defines the appropriate level of quality for each space.
