Impact of surgical experience on postoperative complications in transconjunctival buried suture blepharoplasty: a single-center retrospective study

Background Transconjunctival buried suture blepharoplasty (TCB) is a minimally invasive surgical technique that eliminates the need for a skin incision, thereby reducing the incidence of postoperative complications, preventing visible dermal scarring, and facilitating the durable formation of double eyelids. These advantages have contributed to the increased adoption of TCB in recent years. However, due to the technical complexity and intricacy of the procedure, clinical outcomes appear to be significantly influenced by the surgeon’s level of experience. Therefore, we evaluated the impact of surgical experience on postoperative outcomes following TCB.Methods This retrospective study analyzed 200 cases of TCB performed by a single surgeon at our institution. Cases were stratified into two cohorts: the early-phase group (n=100) and the late-phase group (n=100). Clinical characteristics and the incidence of postoperative complications were compared between the two groups.Results The incidence of postoperative complications was significantly lower in the late-phase group than in the early-phase group (13% vs. 3%, P=0.017). Furthermore, the late-phase group exhibited a significantly lower frequency of both subjective complications and complications occurring within 1 month (P=0.030 and P=0.016, respectively). Notably, postoperative swelling, corneal irritation, and loss of the double eyelid crease were completely absent in the late-phase group.Conclusions A marked reduction in postoperative complications was observed with increased surgical experience. Progressive refinement of surgical technique and enhanced proficiency contributed significantly to the decrease in the incidence of complications associated with TCB.
