Effectiveness of Behavioral Health Intervention in an Integrated Primary Care Setting for Patients with Type II Diabetes.

Uncontrolled type II diabetes causes diabetic complications and increases health care cost. Supporting type II diabetic patients through a biopsychosocial and spiritual model of integrated healthcare could possibly improve type II diabetes management, lower diabetic complications and therefore lower healthcare cost. The purpose of this quantitative quasi-experimental study was to compare two type II diabetic patient groups. The first type II diabetic patient group was the treatment as usual group, and the second type II diabetic patient group was the behavioral health group who attended at least one behavioral health session. The behavioral health therapy session (independent variable) was with a marriage and family therapist. The three dependent variables were depression level scores, anxiety measurement scores, and the type II diabetic patient’s glycated hemoglobin (blood sugar; HbA1c) average. Patients’ abstracted mental health scores along with their HbA1c average were the most current reported data scores in the patient’s electronic health record. The research question answered by this study was is there a difference in mental health measurement scores and HbA1c averages for those patients who sought out behavioral health therapy with an MFT/MedFT in comparison to a treatment as usual group? This study used a MANOVA with a Pallai’s Trace test to compare the mental health measurement scores, and the type II diabetic HbA1c average between the two type II diabetic groups. Based on the Pallai’s Trace = .053, F(3, 230) = 4.297, p = .006, the researcher was able to reject the null hypothesis suggesting there was a difference between the behavioral health group (M = 11.09, (SD = 6.914), and treatment as usual group (M = 8.40 (SD = 6.462) for depression measurement scores. The behavioral health group mean depression score confirmed the Southwest Washington medical clinic’s ≥ 10 point score for referral for behavioral health treatment. There however, was not a significant difference between the groups’ HbA1c averages nor their anxiety levels. A longitudinal study to compare the dependent variables at two points in time would allow for the analysis of patient health outcome trends between the type II diabetes patient groups. Clinical protocols could be developed requiring consistent mental health scores during type II diabetic patient medical visits along with the patient HbA1c average. This would provide reliable data for group comparisons at two points in time for analysis of behavioral health treatment outcomes.
