노인의료복지시설 요양보호사의 직무환경, 노인학대 인식, 역할스트레스, 직업정체성 간의 구조적 관계분석 = A study on the structural relationship among work environment, awareness of elder abuse, role stress, and professional identity of nursing care workers in medical welfare facilities for the elderly

국문 초록 (Abstract)

본 연구는 노인의료복지시설 요양보호사의 직무환경이 노인학대 인식에 영향을 미치는 구조적 관계를 파악하고, 나아가 이들 변인 간 영향 관계에서 역할스트레스와 직업정체성이 매개효과를 나타내는지 밝히고자 하는 연구이다. 이러한 연구결과를 바탕으로 노인의료복지시설 요양보호사의 노인학대 행위 발생 원인을 미연에 확인하고 노인의료복지시설의 노인학대 예방을 통해 입소 노인이 안심하고 시설에서 질 좋은 장기요양서비스를 받을 수 있는 환경조성을 모색하고자 하였다.
본 연구의 목적을 달성하기 위해 다음과 같은 연구문제를 설정하였다.
첫째, 노인의료복지시설 요양보호사의 일반적 특성에 따른 직무환경, 노인학대 인식, 역할스트레스, 직업정체성의 차이는 어떠한가?
둘째, 노인의료복지시설 요양보호사의 직무환경, 역할스트레스, 직업정체성은 노인학대 인식에 미치는 영향은 어떠한가?
셋째, 노인의료복지시설 요양보호사의 직무환경과 노인학대 인식의 관계에서 역할스트레스의 매개효과는 어떠한가?
넷째, 노인의료복지시설 요양보호사의 직무환경과 노인학대 인식의 관계에서 직업정체성의 매개효과는 어떠한가?
이에 본 연구에서는 대구, 경북지역의 노인의료복지시설에 재직하고 있는 요양보호사를 대상으로 자기기입식 설문조사를 진행하였으며, 493부를 자료 분석에 활용하였다.
연구문제에 따른 주요 연구결과를 요약하면 다음과 같다.
첫째, 노인의료복지시설 요양보호사의 일반적 특성에 따른 직무환경, 노인학대 인식, 역할스트레스, 직업정체성 간의 관련성 정도를 파악한 결과 대부분이 통계적으로 변인들 사이에 유의한 상관이 있는 것으로 나타났다.
둘째, 노인의료복지시설 요양보호사의 직무환경은 노인학대 인식에 통계적으로 유의한 영향을 미치지 않은 것으로 나타났다. 이는 요양보호사의 직무환경을 투입한 회귀분석에서 직무환경이 노인학대 인식에 통계적으로 유의한 영향을 미치지 않은 것을 확인하였다.
셋째, 노인의료복지시설 요양보호사의 역할스트레스가 직무환경과 노인학대 인식 관계에서 부분매개효과를 가지고 있는 것으로 나타났다. 직무환경의 업무전문성, 보상체계가 매개변인 역할스트레스에 유의한 영향을 미치는 것으로 확인되었다.
넷째, 노인의료복지시설 요양보호사의 직업정체성은 직무환경과 노인학대 인식 관계에서 부분매개효과가 있는 것으로 확인되었으며, 특히 직무환경의 인간관계, 물리적 환경이 매개변인 직업정체성에 유의미한 영향을 미치는 것으로 나타났다.
본 연구결과를 근거로 다음과 같은 결론을 도출하였다.
첫째, 직무환경의 하위요인인 업무전문성과 보상체계는 노인학대 인식수준에 영향 관계가 있었다. 따라서 요양보호사의 업무전문성과 보상체계 강화를 위한 보수교육, 세미나 등 다양한 기회를 제공하고 나아가 요양보호사의 열악한 처우가 개선될 수 있도록 국가와 지자체 차원에서 제도적·정책적 노력이 필요하다.
둘째, 노인의료복지시설 요양보호사의 직무환경과 노인학대 인식 관계에서 역할스트레스와 직업정체성이 부분매개효과가 확인되었다. 따라서 심층 상담기구 신설과 힐링프로그램의 제도 마련을 통해 역할스트레스를 감소시켜 나가고 보수교육 시 전문성 증진과 직업정체성 향상을 위한 교육내용 추가를 통한 전문성 향상과 자기계발의 기회가 제공되어야 한다.
본 연구는 노인의료복지시설에서 발생하는 노인학대 예방과 장기요양서비스 질 향상을 위한 보다 효과적인 실천 방안을 개발하는데 기여할 것으로 기대한다.

다국어 초록 (Multilingual Abstract)

This study aims to identify the structural relationship in which the work environment of nursing care workers in elderly medical welfare facilities affects awareness of elder abuse, and furthermore, to reveal whether role stress and professional identity have a mediating effect in the relationship between these
variables. Based on these findings, it was intended to identify the causes of elder abuse by nursing care workers in elderly medical welfare facilities in advance and
seek to create an environment in which the elderly in the facility can receive high-quality long-term care services with confidence by preventing elder abuse in
elderly medical welfare facilities.
To achieve the purpose of this study, the following research questions were set up.
First, what is the difference in work environment, awareness of elder abuse, role stress, and professional identity according to the general characteristics of nursing
care workers in elderly medical welfare facilities?
Second, what is the effect of work environment, role stress, and professional identity of nursing care workers in elderly medical welfare facilities on awareness
of elder abuse? Third, what is the mediating effect of role stress in the relationship between the work environment and awareness of elder abuse of nursing care workers in elderly medical welfare facilities?
Fourth, what is the mediating effect of professional identity in the relationship between the work environment and awareness of elder abuse of nursing care
workers in elderly medical welfare facilities?
Therefore, in this study, a self-written survey was conducted for nursing care workers working in medical welfare facilities for the elderly in Daegu and
Gyeongbuk, and 493 copies were used for data analysis.
The main research results according to the research questions are summarized as follows.
First, the degree of relationship between work environment, awareness of elder abuse, role stress, and professional identity according to the general characteristics of nursing care workers in elderly medical welfare facilities was determined, and most of them were statistically significant between variables.
Second, it was found that the work environment of nursing care workers in elderly medical welfare facilities for the elderly did not have a statistically significant effect on awareness of elder abuse. It was confirmed that the work environment did not have a statistically significant effect on awareness of elder abuse in the regression analysis using the job environment of nursing care workers.
Third, it was found that the role stress of nursing care workers in elderly medical welfare facilities for the elderly has a partial mediating effect on the relationship between the work environment and awareness of elder abuse. It was confirmed that the work expertise and compensation system of the work in elderly medical welfare facilities had a partial mediating effect on the relationship between the work environment and awareness of elder abuse, and in particular, the human relationship and physical environment of the work environment had a significant effect on the professional identity of the mediating
variable.
Based on the results of this study, the following conclusions were drawn.
First, the work expertise and compensation system, which are sub-factors of the work environment, were affected by the level of awareness of elder abuse.
Therefore, institutional and policy efforts are needed at the national and local levels to provide various opportunities such as conservative education and
seminars to strengthen the work expertise and compensation system of nursing care workers, and to improve the poor treatment of nursing care workers.
Second, a partial mediating effect of role stress and professional identity was confirmed in the relationship between the work environment of nursing care
workers in elderly medical welfare facilities and awareness of elder abuse.
Therefore, opportunities for professional improvement and self-development should be provided by reducing role stress through the establishment of an in-depth
counseling organization and the establishment of a healing program system.
This study is expected to contribute to the development of more effective action measures to prevent elder abuse and improve the quality of long-term care
services in elderly medical welfare facilities.
