Background
A clinical data warehouse (CDW) system offers an advantage of establishing a real-world evidence based on electronic medical records (EMRs). Although various clinical studies have been conducted on hydroxymethylglutaryl-CoA reductase inhibitors, evidences for the role of statin use in patients with airway diseases are controversial.
Methods
The effects of first statin therapy on annual acute exacerbation (AE), inhaler use and pulmonary function Results were analyzed in patients with airway diseases including chronic obstructive lung disease (COPD), asthma and COPD/asthma overlap. Data was extracted from the CDW system of Seoul St Mary’s hospital from March 1, 2008 to December 31, 2018.
Results
Of the total of 6,989 patients diagnosed with COPD or asthma, 386 COPD, 759 asthma, 944 COPD/asthma overlap patients were remained after exclusion of inhaler use < 36 months or statin use < 6months. COPD, asthma and overlap patients were selected based on ICD code and inhaler prescription. Among them, 47 (12.2%) COPD, 117 (15.4%) asthma, 188 (19.9%) COPD/asthma overlap patients used statin for median 717 (180-853), 651 (180-892) and 691 (180-940) days respectively. In all three study groups, total and LDL cholesterol levels were effectively decreased by statin treatment after 12 and 24 months. In asthma patients, prescription rate of short acting β2 agonist (SABA) and ICS/long acting β2 agonist were lower in statin users compared with non-users at 12 months-visit (P = 0.04 and <0.001 respectively). Rate and frequencies of AE showed no differences according to statin use during 24 months. Moreover, changes of lung function test Results had no differences between statin users and nonusers after the adjustment of age and underlying comorbidities.
Conclusion
Statin use showed little benefit in patients with airway diseases in this dataset, multicenter cohort study can be performed based on our study through clinical data mart development.
