비결핵항산균 폐질환 치료에 따른 마이크로바이옴 변화와 치료 반응과의 연관성 = Microbiome of patients with nontuberculous mycobacterial pulmonary disease and its association with treatment response

Background: Nontuberculous mycobacterial pulmonary disease, whose prevalence and incidence rates are rapidly increasing worldwide, is difficult to treat due to poor treatment outcomes and frequent adverse drug reactions. For this reason, discovering biomarkers to predict treatment response is recognized as a top research priority. In recent studies, the human microbiome has been recognized as a potential biomarker for predicting the prognosis and treatment response of chronic respiratory diseases, but its role in nontuberculous mycobacterial pulmonary diseases has not been revealed. Accordingly, this study aimed to determine the relationship between changes in the gut and airway microbiomes before and after antibiotic treatment and treatment response in patients with Mycobacterium abscessus pulmonary disease, which has the poorest treatment outcome among nontuberculous mycobacterial pulmonary diseases.
Methods: Adult patients aged 19 years or older who were diagnosed with M. abscessus pulmonary disease at Seoul National University Hospital and began antibiotic treatment were prospectively enrolled in an observational cohort after obtaining informed consent. Stool and sputum samples were collected before antibiotic treatment, 2 weeks after antibiotic treatment, and at 6 months. After extracting total bacterial DNA from the sample, 16S rRNA amplicon sequencing analysis was performed to characterize the gut and airway microbiomes. Based on the sputum culture results at the second week of treatment, patients were classified into treatment responders and non-responders. Then the differences in the microbial communities of these two groups were identified using microbiome analysis techniques.
Results: Among 27 prospectively enrolled patients (median age 66 years, 85.2% female, 48.1% M. abscessus subspecies abscessus), 55.6% of patients achieved conversion to negative acid-fast bacilli culture at week 2 and were classified as treatment responders. The remaining patients were classified as non-responding group. Clinical characteristics such as age, sex, body mass index, causative subspecies, and the presence of cavity were comparable between the two groups. However, when the gut microbiome was analyzed, alpha diversity in the treatment responder group showed a significant decrease compared to baseline at 2 weeks of treatment, which was not noticed in non-responding group (P=0.029). The relative abundance of the genus Eubacterium hallii in baseline stool specimens described the non-responding group with great performance (area under the receiver operating characteristic, 0.83; 95% confidence interval, 0.65-0.99). Conversely, the relative abundance of Enterococcus in stool specimen at 2 weeks of treatment was associated with favorable treatment response (area under the receiver operating characteristic, 0.77; 95% confidence interval, 0.59-0.95). The relative abundance of the genus Burkholderia-Caballeronia-Paraburkholderia in baseline sputum specimen was associated with favorable treatment response as well (area under receiver operating characteristics, 0.83; 95% confidence interval, 0.70-0.97).
Conclusions: Patients with M. abscessus pulmonary disease showed differences in the diversity of the gut and airway microbiome and microbial community distribution according to treatment response defined at the second week of antibiotic treatment. These results suggest that the human microbiome in M. abscessus pulmonary disease could be a potential source of discovering novel biomarkers.
서론: 전 세계적으로 유병률과 발병률이 가파르게 증가하고 있는 비결핵항산균 폐질환은 치료 성적이 낮고 빈번한 약물 부작용으로 인해 치료가 난황을 겪고 있다. 이로 인해 치료 반응을 예측하기 위한 바이오마커를 발굴하는 것이 최우선 연구 과제로 인식되고 있다. 최근 연구에서는 인간 마이크로바이옴이 만성 호흡기 질환의 예후 및 치료 반응을 예측하기 위한 잠재적 바이오마커로서 인식되고 있으나, 아직까지 비결핵항산균 폐질환에서의 역할은 밝혀지지 않았다. 이에 본 연구는 비결핵항산균 폐질환 중 가장 치료 성적이 불량한 Mycobacterium abscessus 폐질환 환자의 항생제 치료 전후 장과 폐의 마이크로바이옴 변화와 치료 반응과의 연관성을 확인하였다.
방법: 서울대학교병원에서 M. abscessus 폐질환으로 진단되어 항생제 치료를 시작하는 19세 이상 성인 환자를 전향적으로 동의서 취득 후 코호트에 등록하여 항생제 치료 전, 항생제 치료 2주 후, 그리고 6개월 째에 환자들의 대변 및 객담 검체를 수집하였다. 검체에서 총 세균 DNA를 추출한 뒤 16S rRNA 앰플리콘 염기서열 분석을 진행하여 장과 폐의 마이크로바이옴을 확인하였다. 치료 2주째 객담 배양 결과를 바탕으로 환자를 치료 반응군과 불응군으로 분류한 뒤, 이 두 군의 미생물군집의 차이를 마이크로바이옴 분석 기법을 이용해 규명하였다.
결과: 전향적으로 등록된 27명의 환자(중앙값 66세, 여성 85.2%, M. abscessus subspecies abscessus 48.1%) 중 55.6%의 환자가 2주차에 항산균 배양 음성 전환을 달성하여 치료 반응군으로 분류되었고, 나머지 환자는 치료 불응군으로 분류되었다. 양군에서 연령, 성별, 체질량지수, 원인균종, 공동 형성 여부 등 임상적인 특성은 차이를 보이지 않았다. 그러나 장 마이크로바이옴을 분석하였을 때 치료 반응군에서는 알파 다양성이 기저 대비 치료 2주째 유의한 감소를 보였으나(P=0.029) 치료 불응군에서는 변화를 보이지 않았다. 기저 대변 검체에서 Eubacterium hallii 속에 속하는 균종의 상대적 풍부도는 치료 불응군을 잘 설명하였다(수신기 작동 특성 아래 면적 0.83, 95% 신뢰구간, 0.65-0.99). 반면, 치료 2주째 대변 검체에서 Enterococcus 속에 속하는 균종의 상대적 풍부도는 치료 반응군과 관련이 있었다(수신기 작동 특성 아래 면적 0.77, 95% 신뢰구간, 0.59-0.95). 기저 객담 검체에서 Burkholderia- Caballeronia-Paraburkholderia 속에 속하는 균종의 상대적 풍부도는 치료 반응군과 연관되어 있었다(수신기 작동 특성 아래 면적 0.83, 95% 신뢰구간, 0.70-0.97).
결론: M. abscessus 폐질환을 가진 환자들은 항생제 치료 2주째 치료 반응에 따라 장과 폐 마이크로바이옴의 다양성과 미생물군집 분포에 차이를 보였으며, 이는 향후 치료 반응을 조기에 예측하고 환자별 맞춤형 치료 전략을 수립하는데 잠재적 바이오마커로서 역할할 수 있을 것으로 기대된다.
