Objective: To investigate the association between radiologic findings and genomic alterations in patients with surgically resected lung adenocarcinoma.
Methods: From 2009 to 2016, patients with surgically resected lung adenocarcinoma who had available results for EGFR mutation, ALK immunohistochemistry (IHC), and KRAS mutation were included in the study. We retrospectively analyzed clinical characteristics, radiologic features and genomic alterations.
Results: Of 164 patients, 95 patients were positive for EGFR mutation; 9 patients were positive for ALK IHC; and 11 patients were positive for KRAS mutation. EGFR mutation was associated with younger age, female gender, and never smoker. For radiologic findings, subsolid nodules and positive air-bronchogram sign were associated with positive EGFR mutation. In subgroup analysis of EGFR mutation, L858R point mutation was associated with pleural tagging and air-bronchogram sign, while exon 19 deletion was detected more frequently in younger patients. Positive ALK IHC was associated with solid nodules and advanced stage, while positive air-bronchogram sign was more common in ALK-negative lung adenocarcinoma. In case of KRAS mutation, it was detected more frequently in male gender and patients with smoking history.
Conclusion: The radiologic features are associated with genomic alterations in patients with resected lung adenocarcinoma. Solidity, air-bronchogram sign, and pleural tagging of lung nodules could help to predict the genomic alterations of lung adenocarcinoma.
