In the event of a disaster on the inland water such as the Han River, rescuers strictly observe the ascent speed of 9m per minute when ascending to the surface. However, when an emergency situation occurs or directly witnesses the occurrence of a drowning person, rescuers wear diving equipment and dive, and when a drowner is found, there may be a situation in which it rapidly rises to the surface. As time passes, the chances of resuscitation of the drowned become slim, and they attempt to ascend to the surface as quickly as possible.
This study surveyed fire water rescue workers about their experiences of sudden rise in the inland water level and the appearance of symptoms of decompression sickness.
In addition, through the experiment, the rate of change of air consumption according to the depth of water was verified, and the rate of change of cortisol and symptoms in the human body during rapid rise were analyzed.
The summary of improvement measures according to survey analysis and experimental results is as follows.
The results of questionnaire analysis of human symptoms caused by sudden rise are as follows.
First, the majority (41%) showed that ‘adhering to the speed of 22m per minute when descending underwater and 9m per minute when ascending’ is not observed. Most of the respondents answered 'Not being observed' (57.4%) to 'when ascending to the surface from more than 10m underwater, stop at 5m underwater and perform safety decompression for 3 to 5 minutes'. In addition, 95.1% of respondents answered, 'In an emergency, they have experienced a sudden rise in excess of 9m per minute after finding a drowning person underwater.' 69% of respondents answered '5~6 seconds' for'the average time it takes to reach the surface from 5m underwater when experiencing a sudden rise after discovery of a target.' 91.2% of the respondents said, “The reason for the sudden rise after discovery of the drowned man is the possibility of resuscitation.”
Second, looking at the manifestation of decompression sickness symptoms, psychological anxiety during underwater search (3.32), increased fatigue after underwater search (3.19), hypothermia due to heat loss (2.62), pain and bleeding due to abnormal ear pressure balance, eardrum rupture (2.49), Pain and bleeding due to sinus compression (2.31), followed by dizziness (2.18) after a sudden rise.
Third, when the decompression chamber was installed, 56.5% of the respondents responded most to the optimum depth of 10m or more and less than 20m.
The experiment on the rate of change of air consumption according to the water depth compared and analyzed human characteristics and theoretical values. The experimental results are as follows.
First, subjects A and B showed similar ascent rates according to the water depth.
Second, test subject C showed a significantly higher rate of increase in air consumption at 25m underwater. The reason is that the body reacts sensitively to the deep water pressure, and breathing is somewhat faster than other participants, which is analyzed to increase the air consumption.
Third, test subjects D·E showed significantly lower air consumption overall. This is analyzed as a result of D and E's age 37 and 35, respectively, the youngest among the subjects, strong physical strength, and rich experience in deep sea diving at the time of military service.
Fourth, the average air consumption per minute of test subjects increased by 1.45 times for 5m underwater, about 1.85 times for 10m underwater and 2.8 times for 20m underwater, compared to the water surface.
Fifth, the difference between the experimental mean value and the theoretical value is a result of the difference in the experience and physical strength of each of the five rescuers, the degree to which the body adapts in the water, and consumption of air more or less than the theoretical value depending on the underwater breathing method Is analyzed.
The second experiment was limited to the underwater search part of the inland water crisis and rescue situation, and 5 rescuers were the subjects of the experiment. The rate of change in the stress hormone cortisol concentration and symptoms affecting the body of rescuers were analyzed when the rescuers searched for 20 minutes at a depth of 5m, where they were most frequently dispatched, and then rapidly rose to 50m per minute. The rapid rise took about 6 seconds. Two separate blood samples were collected before and after the experiment and requested to the medical foundation. The experimental results are as follows.
First, the average cortisol concentration before the experiment increased from 10.17ug/dl to 16.94ug/dl after the experiment, confirming a difference of 6.77ug/dl on average. However, subject D showed significantly higher cortisol elevation before and after diving, 11.00ug/dl. The reason is judged to be the increased anxiety caused by the poor of the line of sight in underwater and the exhaustion of physical fitness through tension during a sudden rise. Subject E showed a significantly lower level of cortisol before and after diving, 4.28ug/dl, which appears to be the result of E's age being the youngest among the subjects, and having less anxiety due to the rich experience of deep sea diving. The above results are judged to have affected cortisol concentration even at 5m underwater, just as severe trauma experience caused by electric shock caused a rapid increase in cortisol.
Second, as human symptoms, fatigue was 7~8 and anxiety was 7~9, both of which were high. It is analyzed that psychological tension increases fatigue during underwater search, and poor visible distance increases anxiety. The test participants complained of mild dizziness, as a common symptom of the test participants, which, during a sudden rise, caused a sudden pressure difference, increasing nitrogen saturation, and generating a small amount of air bubbles, which affects the human body, which brings stress to the human body, leading to an increase in cortisol concentration. It is believed to have had an effect. Therefore, even if it is not more than 10m underwater, it is analyzed that frequent sudden rise and repeated diving in 5m underwater will have a fatal effect on human safety.
Improvements are as follows.
First, it is necessary to install a decompression chamber in order to reduce the symptoms of decompression sickness caused by the rapid rise of the Korean water rescue team. In order to improve the underwater rescue environment and prevent safety accidents, it is necessary to understand the actual condition of the use of diving equipment, the number of dives, and the standard for installing the decompression chamber, and measures for a safe rescue environment.
Second, it is necessary to secure a spare rescue member to avoid repeated diving in order to improve the problem of repetitive diving without sufficient rest because reserve rescue member is not secured.
Lastly, for the management of obesity, etc, the body composition analyzer, etc. in the training center of the water rescue team, a physical strength strengthening program was prepared, and the physical evaluation criteria for the water rescue team were specified so that the firefighters with excellent physical strength could work in the rescue team. Needs to be.
The results of the study will be provided as empirical data on the safe underwater search activities of the rescue team.
한강 등 내수면의 수난 구조상황에서 구조대원들은 수면으로 상승할 때 분당 9m의 상승속도를 철저히 준수한다. 그런데, 익수자의 발생을 직접 목격하거나 위급한 상황이 발생하면, 구조대원은 잠수장비를 착용하고 잠수하여, 익수자를 발견하면 신속히 수면으로 급상승(急上昇)하는 상황이 발생하기도 한다. 시간을 지체할수록 익수자의 소생 가능성은 희박해져, 가능한 한 빨리 수면으로 상승을 시도하게 된다.
이 연구는 소방 수난구조대원을 대상으로 내수면으로의 급상승 경험 및 감압병 증상 발현 여부를 설문 조사 후, 실험을 통하여 수심에 따른 공기소모량의 변화율을 검증하고, 급상승 시 코르티솔의 변화율과 인체에 나타난 증상을 분석하였다. 설문 분석 및 실험 결과에 따른 개선방안을 요약하면 다음과 같다.
급상승으로 인한 인체 증상의 설문 분석결과는 다음과 같다.
첫째, ‘수중으로 하강 시 분당 22m, 상승 시 분당 9m의 속도 준수’는 지켜지지 않는다는 것이 대다수(41%)로 나타났다. ‘수중 10m 이상에서 수면으로 상승 시 수중 5m에서 안정정지를 하고 안전 감압을 3~5분 실시’에 대해 대부분이 ‘지켜지지 않는다‘(57.4%)로 대답하였다. 또한, 응답자의 95.1%가 ‘긴급한 상황인 경우, 수중에서 익수자를 발견한 후 분당 9m를 초과하여 급상승한 경험이 있다’라고 답하였다. ‘익수자 발견 후 급상승한 경험 시 수중 5m에서 수면까지 도달하는데 평균 소요 시간’ 은 69%의 응답자가 ‘5~6초’로 응답하였다. ‘익수자 발견 후 급상승한 이유는 소생 가능성 때문‘이 응답자의 91.2%를 차지하였다. 급상승 경험자가 95% 넘게 나타난 것은 신속히 인명구조 후 소생시키고자 하는 의지의 결과로 분석된다.
둘째, 감압병 증상 발현 등을 살펴보면 수중수색 중 심리적 불안감(3.32), 수중수색 후 피로도 증가(3.19), 열 손실로 인한 저체온증(2.62), 귀 압력 평형 이상으로 통증 및 출혈, 고막파열(2.49), 부비동 압착으로 통증 및 출혈(2.31), 급상승 후 어지러움(2.18)의 순서로 높게 나타났다.
셋째, 감압챔버 설치 시 적정 수심은 10m 이상 20m 미만이 56.5%로 가장 많이 응답하였다.
수심에 따른 공기소모량의 변화율 실험은 인간의 특성 및 이론값을 비교·분석하였다. 실험 결과는 다음과 같다.
첫째, 실험대상자 A·B는 수심에 따라 각각 비슷한 상승률을 보였다.
둘째, 실험대상자 C는 수중 25m에서 공기소모량의 상승률이 유의하게 높게 나타났는데, 그 이유는 깊은 수심의 수압에 신체가 민감하게 반응하여, 호흡이 다른 참여자보다 다소 빨라 공기소모량을 증가시킨 것으로 분석된다.
셋째, 실험대상자 D·E는 공기소모량이 전반적으로 유의하게 낮게 나타났다. 이는 D와 E의 연령이 각각 37세, 35세로 실험대상자 중 가장 젊고, 체력이 강건하며, 군 복무 당시 해상의 심해 잠수경험이 풍부하여 나타난 결과로 분석된다.
넷째, 실험대상자 분당 평균 공기소모량은 수면과 비교하여 수중 5m는 약 1.45배, 수중 10m는 약 1.85배, 수중 20m는 약 2.8배로 분당 공기소모량이 증가하였다.
다섯째, 실험 평균값과 이론값의 차이는 실험대상자 구조대원 5명 각자의 경험과 체력, 신체가 수중에서 적응하는 정도의 차이, 그리고 수중 호흡법에 따라 이론값보다 많거나 적게 공기를 소모하여서 나타난 결과로 분석된다.
두 번째 실험은 내수면 수난 구조상황의 수중수색 부분에 한정하고, 구조대원 5명을 실험대상으로 하였다. 수난구조대원들이 가장 빈번하게 출동하는 수심 5m에서 20분간 수색 후 분당 50m로 급상승할 때, 스트레스 호르몬 코르티솔 농도의 변화율과 구조대원 인체에 미치는 증상을 분석하였다. 급상승은 약 6초가 소요되었고. 실험 전후에 2회 개별 채혈하여 의료재단에 의뢰하였다. 실험 결과는 다음과 같다.
첫째, 실험 전의 평균 코르티솔 농도는 10.17ug/dl에서, 실험 후 16.94ug/dl로 상승하여 평균 6.77ug/dl의 차이를 확인했다. 그런데, 실험대상자 D는 잠수 전후 코르티솔 상승 수치가 11.00ug/dl로 유의하게 높게 나타났다. 그 이유는 수중 시계의 불량으로 인한 불안감 가중 및 급상승 시 긴장을 통한 체력소모 고갈로 판단된다. 실험대상자 E는 잠수 전후 코르티솔 상승 수치가 4.28ug/dl로 유의하게 낮게 나타났는데, 그 이유는 E의 연령이 실험대상자 중 가장 젊고, 해상의 심해 잠수경험이 풍부하여 불안감이 작아서 나타난 결과로 보인다. 위와 같은 결과는 전기충격 등으로 인한 심각한 외상 경험이 코르티솔의 급격한 상승을 유발하듯이, 수중 5m에서 급상승도 코르티솔 농도에 영향을 준 것으로 판단된다.
둘째, 인체 증상으로서 피로도는 7∼8, 불안감은 7∼9로 둘 다 높은 수치를 나타내었다. 수중수색 시 심리적 긴장감이 피로도를 높이고, 불량한 시계가 불안감을 높인 것으로 분석된다. 실험 참여자의 공통 증상으로 가벼운 어지러움을 호소하였는데, 이것은 급상승 시, 급격한 압력 차가 발생하여, 질소 포화도를 올리고, 소량의 기포가 발생하여, 인체에 영향을 미치고, 이것이 인체에 스트레스를 가져와 코르티솔 농도 상승에 영향을 미친 것으로 판단된다. 따라서, 수중 10m 이상이 아니더라도 수중 5m에서 빈번한 급상승과 반복잠수는 인체 안전에 치명적인 영향을 줄 것으로 분석된다.
개선사항은 다음과 같다.
첫째, 한국 수난구조대원의 급상승에 따른 감압병 증상 발현을 감소시키기 위해 감압 챔버를 설치할 필요가 있다. 수중구조환경을 개선하고 안전사고 예방을 위해 잠수장비 사용실태와 잠수횟수, 감압 챔버 설치기준에 대한 실태 파악 및 안전한 구조환경을 위한 대책이 필요하다.
둘째, 예비대원이 확보되어 있지 않아 충분한 휴식 없이 반복잠수를 할 수밖에 없는 문제점을 개선하기 위해 예비대원을 확보하여 반복잠수를 지양하게 할 필요가 있다.
마지막으로, 수난구조대원 비만 등의 관리를 위하여 수난구조대 체력단련장에 체성분 분석기 등의 기구보완, 체력강화 프로그램 마련과 수난구조대원 체력평가 기준을 구체화하여 체력이 우수한 소방대원이 수난구조대에 근무할 수 있도록 할 필요가 있다.
연구결과는 수난구조대원의 안전한 수중수색 활동에 대한 실증적 자료로 제공될 것이다.
