소아암 생존자의 극복력 영향 요인 : 경로분석
It has been reported that childhood cancer survivors are subject to various psychological and social issues due to several aspects (late complications, perceived health problems, social stigma, and defensive reactions) after experiencing cancer. Despite these aspects, other positive elements (challenging reactions, spirituality, hope, family communication, and social support) can also bring about a positive effect of cancer called resilience. As increased resilience plays a key role in improving childhood cancer survivors’ quality of life, medical professionals must provide active management in order to boost survivors’ resilience. As such, this study identified the predicting factors of resilience and conducted a path analysis to investigate the indirect and direct effects of such factors to find ways for developing effective interventions for childhood cancer survivors.
First, this study identified factors affecting resilience using the Resilience in Illness Model suggested by Haase et al. and reviewed previous literature; based on the results, it constructed a hypothetical model with age at the time of diagnosis, solid tumor, social support, and spirituality as exogenous variables; and defensive reaction, challenging reaction, family communication, and hope as endogenous variables. Data used in this study were collected from 139 childhood cancer survivors aged 15 to 29, at a general hospital located in Seoul from February to September 2017. The participants were diagnosed with childhood cancer before they were 19-years-old and completed cancer treatment at least a year earlier. The collected data were analyzed using SPSS 23.0 and Amos 24.0 programs.
The major results are as follows:
1. Resilience of childhood cancer survivors, risk factors, and protection factors
On average, the childhood cancer survivors scored 66.72±9.40 for resilience, 2.32±2.72 for late complication, 7.19±6.91 for perceived health problems, 18.14±5.18 for stigma faced as a cancer patient, 16.25±8.08 for defensive reactions, 34.73±10.71 for challenging reactions, 24.97±8.40 for spirituality, 36.90±5.47 for hope, 72.99±13.62 for family communication, and 47.51±8.77 for social support.
2. Factors affecting childhood cancer survivors’ resilience
It was found that challenging reactions (β=.303, p<.001), hope (β=.317, p=.001), and solid tumor (β=-.144, p=.012) were the most influencing factors of the survivors’ resilience with an explanatory power of 65.2%.
3. Results of hypothesis testing through an analysis of the modified model
1) Resilience increased as did social support, spirituality, challenging reactions, and hope; however, it declined when diagnosis of solid tumor was present. These variables explained 65% of childhood cancer survivors’ resilience.
2) The survivors engaged in more challenging reactions as they had higher spirituality and exhibited more defensive reactions, and these variables explained 39% of the challenging reactions.
3) There was more family communication when the participants were older when diagnosed with cancer and received greater social support, and the two variables explained 52% of family communication.
4) Finally, the participants had greater hope when there was more family communication, social support, spirituality, and when they were older at the time of diagnosis; these variables explained 66% of the participants’ hope.
In conclusion, these results indicate that resilience of childhood cancer survivors is directly and indirectly affected by spirituality, family communication, hope, social support, and age during diagnosis of solid tumor. This study should be followed by further research on development and application of an intervention program intended to promote challenging reactions and hope, which were found to have a significant mediating effect on childhood cancer survivors’ resilience.
소아암 생존자는 암 경험 이후에 여러 부정적인 요소(후기합병증, 인지하는 건강문제, 사회적 낙인, 방어적 대처)들로 인하여 다양한 심리 사회적인 문제를 경험하는 것으로 보고되고 있다. 그러나 이러한 요소들은 다른 긍정적인 요인(도전적 대처, 영성, 희망, 가족 의사소통, 사회적 지지)들로 인하여 극복력이라는 용어로 암의 긍정적 효과를 보여주고 있다. 극복력 상승은 소아암 생존자의 삶의 질에 주요한 역할을 하므로 의료진은 소아암 생존자의 극복력 향상을 위한 적극적 관리를 시행하여야 한다. 본 연구에서는 소아암 생존자를 위한 효과적인 중재를 위하여 극복력을 예측하는 요인을 확인하고, 요인들의 직, 간접 효과를 확인하고자 경로분석을 수행하였다.
극복력에 영향을 주는 요인에 대해 Haase등이 제시한 Resilience in Illness Model과 선행연구 고찰을 바탕으로 극복력에 영향을 주는 요인을 파악하였고, 결과를 바탕으로 가설적 경로모형을 구축하여 진단 시 나이, 고형종양, 사회적 지지, 영성을 외생변수로, 방어적 대처, 도전적 대처, 가족 의사소통, 희망을 내생변수로 구성하였다. 자료 수집은 2017년 2월부터 2017년 9월까지 서울 소재의 일 종합병원에서 만 19세 이전 소아암을 진단 받고, 치료 종료 후 최소 1년이 지난 15세에서 29세의 소아암 생존자 139명을 대상으로 실시하였다. 수집된 자료는 SPSS 23.0 프로그램과 Amos 24.0 프로그램을 이용하여 분석하였다. 연구결과는 다음과 같다.
1. 소아암 생존자의 극복력과 위험요인 및 보호요인
소아암 생존자의 극복력 평균은 66.72±9.40점이며, 후기합병증은 2.32±2.72점, 인지하는 건강문제 7.19±6.91점, 암 낙인18.14±5.18점, 방어적 대처 16.25±8.08점, 도전적 대처 34.73±10.71점, 영성 24.97±8.40점, 희망 36.90±5.47점, 가족 의사소통72.99±13.62점, 사회적 지지 47.51±8.77점으로 나타났다.
2. 소아암 생존자 극복력 영향요인
소아암 생존자의 극복력에 가장 큰 영향요인은 도전적 대처(β=.303, p<.001), 희망(β=.317, p=.001), 고형종양(β=-.144, p=.012)으로 확인되었고, 65.2%의 설명력을 보였다.
3. 수정모형 분석을 통한 가설검증 결과는 다음과 같다.
1) 극복력은 사회적 지지가 높을수록, 영성이 높을수록, 도전적 대처가 높을수록, 희망이 높을수록 높은 것으로 나타났고, 고형종양 진단명에서 낮은 것으로 나타났다. 이 변수들에 의해서 극복력 변인의 65%가 설명되었다.
2) 도전적 대처는 영성이 높을수록, 방어적 대처가 높을수록 높은 것으로 나타났고, 이 변수들에 의해서 도전적 대처 변인의 39%가 설명되었다.
3) 가족 의사소통은 진단 시 나이가 높을수록, 사회적 지지가 높을수록 높은 것으로 나타났고, 이 변수들에 의해서 가족 의사소통 변인의 52%가 설명되었다.
4) 희망은 가족 의사소통이 높을수록, 사회적 지지가 높을수록, 영성이 높을수록, 진단 시 나이가 높을수록 높은 것으로 나타났고, 이 변수들에 의해서 희망 변인의 66%가 설명되었다.
본 연구결과를 통해 소아암 생존자의 극복력은 영성, 가족 의사소통, 희망, 사회적 지지, 고형종양 진단명의 직, 간접 영향을 받는 것을 알 수 있었다. 특히, 소아암 생존자의 극복력에서 유의한 매개효과가 있는 도전적 대처와 희망을 높여주기 위한 중재 프로그램의 개발 및 적용에 대한 추후 연구를 제언한다.
