목소리 문제에 대하여 이비인후과에서 실시하는 후두검사법 = Larynx Examinations in Otorhinolaryngology Clinic about Voice Problems -Focusiong on Diagnosis & Evaluation of Voice Disorders

국문 초록 (Abstract)

음성 혹은 목소리 문제에 대하여 이비인후과에서 실시하는 검사방법에는 여러 가지가 있다. 일상생활에서 매일 사용하는 우리의 목소리가 일생동안 끊임없이 감정을 표현할 뿐만 아니라 말을 만들어 다른 사람들과 의사소통을 할 때 매우 중요한 수단이 된다. 각종 음성장애 및 후두질환의 정확한 진단 및 치료를 위해서는 후두기능을 보다 객관적으로 검사 할 필요가 있다. 이러한 후두 기능의 검사 일환으로 시행하는 기본적인 음성검사는 후두의 공기역학적 검사, 성대의 진동 검사, 음향학적 검사, 신경근 검사 및 청각심리 검사 등이 있다. 이들 검사의 원리를 약술하면 흡기작용에 의하여 폐로 흡입된 공기가 기관지와 기관을 거쳐 후두를 통과해 나오는 과정에서 성대의 진동이 유발되어서 음성이 나오는 것이므로, 이 호기가 음성의 에너지원이 되어 발성기전이나 발성장애의 기전 설명에서 공기역학적 검사가 필요하게 되는데, 이는 공기압을 음향신호로 변환시키는 일종의 후두의 효율도 검사라고 이해할 수가 있다. 성대의 진동검사는 각종 기기를 사용해서 성대진동양상의 이상 유무를 판별하며, 음향검사는 스펙트로그래프를 이용해서 음성의 여러 음향적 구조를 시각적으로 판별할 수 있도록 그래프로 나타낸다. 신경근 검사는 근전도를 이용해서 발성 중에 후두의 내근 및 외근의 신경학적 검사를 시행하는 것이며, 청각심리 검사는 훈련된 청능을 가지고 쉰 목소리들을 들어서 심한 정도를 분류하는 것이다. 이처럼 여러 측면에서의 검사를 통해서 발성능력의 종합적인 판단이 가능하게 되며, 이로써 후두질환의 진단, 예후, 치료 면에서 적정을 기할 수가 있게 됨으로써 이들 모두가 후두과학 및 음성의학 분야에서 필수적인 평가 자료이다.

다국어 초록 (Multilingual Abstract)

There are various examination methods at an otorhinolaryngology clinic(ENT, ear-nose-and-throat clinic) to diagnose voice problems. Voice is not only a means to express our emotion everyday through our whole life, but is also one of the important means to communicate with others by making words. In order to exactly diagnose and treat various kinds of voice disorders and larynx diseases, there is a need to examine the larynx more specifically. Basic voice examination methods to check proper functions of the larynx are an aerodynamic examination on the larynx, a vibratory examination on the vocal cords, an acoustic examination, a neuromuscular examination and a psychologistic examination and so on. The summary of the principle of these examinations is as follows. Voice is generated in the process that the inhaled air in the lungs passes through the bronchial tubes, the trachea and the larynx to be exhaled, and, this process causes the vocal cords to vibrate, and thus, to make voice. Since the exhaled air is the energy of voicing, an aerodynamic examination is needed to explain the mechanism of voicing and voice disorders, and this examination can be considered to be one of the tests to measure the rate of efficiency in converting air pressure to acoustic signal by the larynx. Through a vibratory examination on the vocal cords, one can discern whether there is any disorder in the vocal fold vibratory pattern, with help of various examination machines, and, through an acoustic examination, one can get a visual graph that will show him/ her the various acoustic structures of voice visually by using a spectrograph. A neuromuscular examination is carried out to check the neurological status of interior and exterior muscle of the larynx in the middle of voicing by using electromyography(EMG), and a psychologistic examination is performed to discern the degree of being hoarse after listening one"s voice with trained and very delicate auditory ability. As above, based on various kinds of examinations, a comprehensive evaluation on voicing ability can be made, and thus, diagnosis, prevention and treatment of voice disorder can be much more easier. Then, it can be said that all of these are essential evaluation data in the science related to larynx and voice medicine.
