Secondhand Smoke Exposure and ADHD: A Hospital-Based Case-Control Study

Objectives: To decrease the prevalence of ADHD and its deterioration, it is helpful to modify controllable environmental factors that may affect its severity. Secondhand smoke may negatively affect the neurodevelopment of children. The present study investigated the relationship between ADHD, a representative neurodevelopmental disorder, and exposure to secondhand smoke through a hospital-based case-control study.
Subjects and Methods: In this hospital-based case-control study, participants included a patient group of 107 children diagnosed with ADHD and a control group of 209 children; all children were aged 6-12 years. ADHD diagnosis was made according to the DSM-IV diagnostic criteria using a semi-structured interview instrument. For variables related to exposure to secondhand smoke, the current smoking of both parents, smoking during pregnancy, and smoking up to one year after birth were investigated. In addition, age, sex, financial status, parents' education levels, low birth weight, and premature birth were investigated as sociodemographic variables. The relationship between exposure to secondhand smoke and ADHD were analyzed using multivariate logistic regression analysis. Differences in scores of ADHD-RS and CPT between ADHD with and without paternal smoking were analyzed.
Results: Among the variables related to secondhand smoke exposure, father's smoking during pregnancy (OR=1.68, 95% CI=1.03-2.76) and father's smoking up to one year after birth (OR=1.69, 95% CI=1.04-2.77) were significantly related to their offspring's ADHD. Those two risk factors showed significance even after adjustment for covariates such as age, gender, financial status, parents' level of education, low birth weight, and premature birth. Secondhand smoke exposure due to paternal smoking was more significantly related with offspring’s hyperactivity/impulsivity than inattention by using the subscales of ADHD-RS and CPT.
Conclusions: The current results suggest that exposure to secondhand smoke at home during the fetal and newborn period, which is the initial development stage, increases the risk of ADHD. These findings suggest that reduced exposure to secondhand smoke at the early stage of life may decrease the incidence of ADHD in offspring.
