알코올중독은 습관적으로 음주함으로써 음주에 대한 조절능력을 상실하여 건강, 직업, 가족, 대인관계, 법적인 문제 등 주요생활 기능에 장애를 일으키는 만성적인 질병이며 개인뿐만 아니라 사회적 문제로 대두되어온 바 이를 해결하기 위한 치료적 방안으로서의 다양한 프로그램 개발이 요구되고 있다.
본 연구는 입원 알코올중독자들의 재발방지와 회복을 위해 A.A 프로그램을 적용하여 이들의 자아존중감과 단주자기효능감에 미치는 효과를 파악하고 이를 통해 정신보건사회복지 분야의 현장에서 일하는 정신보건사회복지사들이 알코올중독자들에게 보다 효율적인 집단치료 및 프로그램을 연구, 개발하고 진행하는데 제공 하고자 본 연구를 진행하게 되었다.
집단은 경남 K시 H병원에 입원한 알코올중독환자들 중 개별면담을 실시하여 프로그램에 참여하는 환자들을 대상으로 실험집단과 통제집단에 각 10명씩 구성하였다. 진행기간은 2011년 12월 29일에서 2012년 2월 16일까지 매주 2회(수, 토요일) 1회기마다 80분씩 총 15회기에 걸쳐 시행하였다. 실험집단과 통제집단 구성은 중도탈락자 각 5명을 제외한 각 10명이었으며, A.A집단프로그램을 제외한 기본적인 치료프로그램은 두 집단에게 모두 동일하게 실시하였고, 프로그램 진행은 A.A 회원 1인과 본 연구자가 담당하였다
단주자기효능감의 척도로 Diciemente 등(1990)이 개발하고, 김성재가 (1996)번안 보완한 척도를 사용하였으며 자아존중감척도는 Rosenberg(1965)가 개발 전병재(1974)가 번안 한 것을 도구로 사용하였다.
본 연구를 수행하는데 있어서 통계학적 분석 자료는 다음과 같은 통계처리 과정을 거쳤다. 실험집단과 통제집단의 응답자의 일반적 특성, 알코올 치료특성, 술로 인해 생겼던 어려움 및 고통 사항을 알아보기 위하여 빈도분석(Frequency Analysis)을 실시하였다. 자아존중감과 단주자기효능감에 대한 각 문항의 신뢰도 검사를 실시하여 문항간의 신뢰도를 측정하여 예측가능성, 정확성 등을 살펴보았으며 문항간의 신뢰도는 Cronbach‘s 의 계수로 판단하였다. 연구의 실증분석은 모두 유의수준 5%에서 검증하였으며, 통계처리는 SPSSWIN 18.0 프로그램을 사용하여 분석하였다.
프로그램 실시 후 그 영향을 분석한 연구결과, 제 1가설인 “A.A 집단에의 참여가 알코올중독자들의 단주자기효능감에 영향을 미치는가?”는 실험집단의 A.A참여 후에 유의미한 차이가 있는 것으로 나타났으며(통제집단 1.99, 실험집단 2.24 ,Z=-2.949, p<.01), 제2가설인 “A.A 집단에의 참여가 알코올중독자들의 자아존중감에 영향을 미치는가?” 는 A.A 프로그램 참여 후 실험집단의 자아존중감이 유의미하게 높아진 것으로 나타났다(통제집단 3.29, 실험집단3.49, =7.189, p<.05).
이에 본 연구는 음주문제에 대한 개입은 알코올중독자의 ‘음주문제 이면의 심리적 어려움과 사회적, 환경적 어려움’을 이해하는 시각과 A.A의 효과를 고려하여 계획되어야 하며 이를 통해 음주문제에 대한 변화로서 자아존중감과 단주자기효능감을 촉진시키는 것이 효과적임을 확인할 수 있었다.
이러한 점에서 정신과 병동에서 진행되는 알코올중독자들에 대한 집단치료프로그램은 일방적 강의식 교육 보다는 환자 스스로의 자발적 참여와 함께 의견(술에 대한 개인적 경험)의 토설(吐說) 발표 및 역할시연 등 적극적인 행동들에 대한 기회가 보다 많이 주어지도록 구성 및 개입이 이루어져야 한다고 판단된다.
또한, 알코올중독자들의 자기문제 인식에서부터 비롯되는 자발적 참여도 중요하지만 프로그램의 참여 효과를 극대화시키기 위해서는 환자들이 직접 의견을 발표하며 참여할 수 있는 적극적이고 치료적인 행동들이 포함된 프로그램의 구성과 진행자의 개입이 효과적인 것으로 분석이 되었다.

Abstract
An Effect of Participation in the Alcoholics Anonymous Program on the Improvement in the Abstinence Self-Efficacy and Self-Esteem of Alcoholics

Alcoholism is a chronic illness, causing the disorder of major life in terms of health, work, family, interpersonal relationships, and legal problems by losing the ability of control due to habitual drinking. It has been also emerging as a social problem as well as an individual problem, so that the development of various programs is required as a therapeutic method.
To prevent recurrence of alcoholism and recover from it, this study attempts to identify the effect of alcoholics' self-esteem on abstinence self-efficacy by applying the alcoholics anonymous(A.A) program which can help mental health social workers, working in the field of mental health social welfare, to study and develop more effective programs of group therapy for alcoholics.
The experimental group and the control group were composed of ten subjects respectively among patients who participate in the program by conducting individual interviews with alcoholics who are hospitalized in H hospital, K city, in Gyeongnam. The experiment was conducted from December 29, 2011 to February 16, 2012, twice every week throughout a total of 15 sessions with 80 minutes per session.
The composition of the experimental group and the control group was based on ten subjects in each group, except for five dropouts. Basic treatment programs except for the A.A group program were applied to these two groups equally. One member of A.A and the author of this research, I assumed the procession of the program.
As the Alcohol Abstinence Self-Efficacy Scale(AASE), the scale of alcohol abstinence self-efficacy, developed by Diciemente et al. (1990), and complemented by Kim Seong Je(1996) was used. As the Self-Esteem Scale, tools, developed by Rosenberg(1965) and adapted by Rosenberg(1965) were used.
The statistical analysis of data in performing this study was conducted. To examine the general characteristics of respondents, the characteristics of alcohol treatment, and difficulties and suffering caused by drinking alcohol, Frequency Analysis was conducted on the experimental group and the control group.
The reliability of questions was measured by conducting the reliability test of each question in regard to self-esteem and abstinence self-efficacy to measure predictability and accuracy. The reliability of questions was determined by the coefficient of Cronbach‘s . In the empirical analysis of the study, verification was made in the significance level of 5%, and statistical analysis was conducted by using the SPSSWIN 18.0 program.
After carrying out the program, the effect of the program was analyzed. The findings are as follows: There was a significant difference in the experimental group before participation in the A.A program and after participation in the A.A program in regard to the first question, "Does the participation of alcoholics in A.A affect the abstinence self-efficacy of alcoholics (the control group 1.99, the experimental group 2.24, Z=-2.949, p<.01). The self-esteem of the experimental group was significantly improved after participation in the A.A program (the control group 3.29, the experimental group 3.49, =7.189, p<.05). in regard to the second question, "Does the participation in A.A affect self-esteem?"
This study indicates that the perspectives of understanding psychological difficulty as well as social and environmental difficulties behind drinking problems should be taken into account in the intervention for drinking problems. It also demonstrates that improvement in self-esteem and abstinence self-efficacy is effective in the change of drinking problems.
In this sense, it is judged that intervention or program that can give patients more opportunities of active behaviors such as revelation of personal experience, presentation, and role play is required along with the voluntary participation of patients rather than one-sided lecture or education in the group therapy program for alcoholics, undertaken in the psychiatric ward.
In addition to the voluntary participation of alcoholics, derived from the self-awareness of alcoholics, this study lays stress on that the organization of the program and the intervention of coordinator that can include aggressive and therapeutic behaviors, allowing patients to express their opinion and join, is effective in maximizing the effect of program participation.
