본 연구의 목적은 당사자운동에 참여하는 정신장애인이 어떠한 변화과정을 겪는지 분석하는 것이다. “변화과정”은 시간의 축적과 다양한 관계들과의 상호작용을 포함하고 있기 때문에 개인의 경험 속에서 드러나는 변화는 당사자운동 전체적인 역동과 깊은 관련이 있을 수밖에 없다. 이는 비인과적인 이해를 요구하기 때문에 질적연구방법으로 그 과정을 분석하는 것이 적절하다고 판단하였다. 자료수집은 경력 1년 이상 이면서 유병기간 5년인 경험 당사자 6명을 선정하여 심층면접, 5개월간 참여관찰을 통해 수집하였다. 이렇게 수집된 자료는 귀납적 주제분석의 과정과 방법에 따라 분석되었다.
“자리”를 찾아가는 여정은 자리의 상실에서부터 시작했다. 정신장애인은 진단 이후 치료를 중심으로 일상이 재편되면서 환자 외에 다른 모양새로 살아갈 수 있는 선택지가 거의 주어지지 않았다. 치료를 위해 직장이나 학교를 그만두어야 했고 이전에 만나던 친구들과의 연락도 끊긴 채 아무런 활동도 하지 않고 살아가던 정신장애인의 삶의 궤적은 “자리”를 상실하는 과정으로 드러났다. “자리”는 개별적인 존재이면서 동시에 사회적 존재로 살아갈 수밖에 없는 인간의 다층적인 공간을 상징한다. 즉 자리의 상실과 또 찾아가는 여정은 개별적인 존재로서의 ‘나’와 사회적 존재로서의 ‘나’가 어떻게 만나고 관계하느냐와 관련이 있다.
본 연구의 분석 내용은 자리를 상실했던 정신장애인이 당사자운동이라는 공적 활동을 통해 어떻게 사회적 존재로 자리 잡아가는지 또 이 활동이 어떻게 자신에 대한 새로운 해석의 순환과정으로 까지 나아갈 수 있는지 보여주고 있다.
정신장애인의 당사자운동 진입은 사회에서 ‘무엇이 되기 위한’ 시도의 일환으로 시작되었다. 일자리는 외부에서 인정하는 분명한 사회적 활동이기 때문에 활동가라는 일자리가 주는 ‘호칭’을 얻는 것은 큰 만족감으로 다가왔다. 즉 초기 당사자운동 활동가는 자리를 ‘갖는 것’이라기보다는 사회적으로 승인된 역할을 통해 사회적 공간으로 진입하는 것, 그래서 자리를 ‘얻는’ 것으로서 시작하고 있었다.
연구참여자들은 당사자운동 조직에 참여하면서 공적 역할을 얻었고 이것은 어느 정도 생활세계에서 인정받는 외형적 틀을 얻게 되었다. 그러나 활동가라는 자리의 의미를 이해하지 못하면 그간 자신을 무능력하고 나약한 존재로 인식하던 내용을 변화시킬 만한 새로운 자극을 갖지 못했다. 즉 활동가로서의 자기인식이 일어나지 않으면 활동의 동력을 얻지도 못할 뿐 아니라 자신에 대한 새로운 해석의 순환과정으로 까지 나아갈 수 없었다.
연구참여자들은 조직에서 활동 하면서 당사자운동이 사회적으로 의미 있는 활동이라는 인식, 정신장애인 처한 사회적 어려움을 해결하는 주체가 당사자라는 인식을 갖게 되었다. 이를 통해 자긍심, 자신감과 같은 긍정적인 태도를 갖게 되었고 이를 통해 ‘나’에 대한 새로운 해석과 이해를 할 수 있었다. 자신이 중요한 사회적 역할을 하고 있다는 발견을 통해 그간 자신을 실패한 사람, 나약한 사람으로 인식하던 당사자가 자신을 바라보는 관점을 변화시켰다. 당사자운동이 ‘일’자리에서 ‘삶’자리로 변화되는 기점을 맞이했다고 할 수 있다. 이 변화를 보여주는 대표적인 장면이 자신의 정신질환에 대해 설명 ‘받는’ 병에서 설명 ‘하는’ 병으로 바뀌는 지점이었다. 자신의 병을 설명 할 수 있게 된 것은 수동적이고 순응적인 삶의 태도로 살아가야만 했던 정신장애인의 세계관이 변화되었음을 보여준다.
당사자운동 활동가라는 하나의 호칭으로 시작했던 일이 자신의 개별성과 고유성을 설명하는 “자리”를 잡을 수 있도록 한 것은 당사자운동 활동을 위해 필요한 수단인 “말”과 “일”을 통해서였다. 활동가 자리는 당사자들에게 ‘말’할 자리를 제공한다. 활동가는 토론회에서는 토론자로, 대학교‧보수교육과 같은 자리에서 강연자로, 집회에서 발표자로 참여한다. 동료들을 도울 수 있는 중요하고 핵심적인 수단이다. 그간 정신장애인의 말은 전문가들이 내리는 판단의 대상이었다. 당사자의 말은 중요한 의견이나 생각으로 다루어지지 않을 뿐더러, 일상적 대화로도 받아들여지지 않았다.
또한 ‘일’은 그간 사회적 역할이 제한되어 있던 정신장애인에게 의미 있는 사회적 활동과 관계를 맺는 매우 중요한 통로가 되어주었다. 특히 활동가는 일의 기획부터 준비, 진행, 마무리하는 과정에 참여하였다. 활동가에게 담당자로서 일이 ‘온전히 맡겨’질 때 결국 ‘일의 책임감’과 함께 따라오기 때문에 더 많은 선택지를 제공되고, 선택의 범위가 넓어질 때 개인은 최선의 선택을 하기 위한 더 많은 노력을 기울이게 된다. 그리고 이 안에서 생각하고 통제하는 힘이 길러진다.
이렇듯 정신장애인들에게 당사자운동은 보통의 세계를 살게 해주는 중요한 환경이 되어 줌에도 불구하고 이 자리를 포기하는 경우도 많았다. 활동가들의 자리를 위태롭게 하는 것은 정신질환의 특성에 기인하는 내재적 요인도 있었으며, 일을 수행하는데 필요한 기술이 없어 업무 스트레스를 느끼거나 낮은 임금으로 인한 생활의 어려움과 같은 외부 영향도 있었다. 그러나 이러한 이유로 일을 그만두더라도 자신의 일상에서 정신장애인의 사회적 문제에 대해 지속적으로 관심을 두고, 당사자운동 활동에 대해 직‧간접적으로 참여하고자 하였다.
본 연구에서 분석한 당사자운동 참여 정신장애인의 변화과정과 그 변화를 추동하는 요인들이 정신보건 사회복지 이론과 실천에 줄 수 있는 함의는 다음과 같다.
첫째, 본 연구의 분석결과는 기존의 임파워먼트와 회복 패러다임이 지향하는 목표와 중요한 가치를 지지하면서 동시에 이론적 개념들이 정신장애인의 삶에서 어떻게 구현되는지를 보여준다.
회복 패러다임은 정신질환을 진단 받고 나서 개인이 가지고 있는 개별성과 독특성이 사라지는 것에 대한 비판적 시각에서 출발한다. 본 연구는 연구참여자가 자신의 병을 “설명 받는 병에서 설명 하는 병”으로 새롭게 위치 짓는 것을 통해 질병 중심적인 정체감에서 벗어났음을 선명하게 보여주었다. 또한 임파워먼트 관점은 상대적으로 권력이 결핍되어 있는 상태의 대상자들에게 권한과 능력을 부여하는 것으로 이야기된다. 이 결핍은 본질적으로 사회적이고 구조적인 문제라고 본다. 이는 당사자운동에서 장애문제를 이해하는데 있어서 접점이 있다. 정신장애인인이 경험하는 사회적 문제를 권력 관계 속에서 읽어내는 것은 활동가로서의 자기인식의 중요한 출발점이었다.
둘째, 본 연구 결과를 통해 정신보건 사회복지 실천 현장이 추구해야하는 전문성에 대한 새로운 해석을 제안할 수 있다.
당사자 운동을 하기 이전, 여러 형태의 복지 서비스 이용자로서의 경험과 활동가로서의 경험의 대조는 사회복지 체계 내에서 서비스 공급자와 정신장애인간의 힘의 불균형을 드러냈다. 도움을 주는 편에서 도움을 받는 편의 ‘더 나은 상태’를 규정하는 순간, 힘의 균형은 도움을 주는 쪽으로 기울 수밖에 없다. 자원을 가진 쪽의 ‘돕는 행위’ 안에는 도움을 결정하는 힘이 생기기 때문이다. 도움을 주는 편에서 규정한 더 나은 상태를 따르지 않는다면 도움은 제한된다. 이용자가 겪는 ‘나’의 문제를 설정하고 해결의 주체로 인정하기 위해서는 그 문제를 해결할 수 있는 자원과 방식이 이용자들에게도 열려 있어야 한다. 돌봄과 보호의 역할은 사회복지사의 역할로 한정할 필요가 없다. 공간에 대한 규칙을 정하는 자와 따르는 자의 구분도 선명할 필요가 없다. 이를 위해서 정신보건 사회복지 영역이 정신장애인과 함께 일하는 파트너로서의 역할이 강조되는 것으로 그 전문성의 성격과 내용이 전환되는 방식을 생각해볼 수 있다.

The purpose of this study is to analyse what changing process mentally challenged people go through as they participate in a disabled-led disability rights movement. Since the "changing process" involves the accumulation of time and interaction with various relationships, it is inevitable that such a change occurring from personal experience is closely related to the overall dynamics of the disabled-led movement. Considering the changing process requires a non-causal understanding, I thought it would be appropriate to use the qualitative research method in conducting analysis. The data were collected by selecting six mentally challenged people with one or more year of experience as a disabled-led movement activist and with a mental illness history of five or more years, followed by conducting in-depth interviews and observing their participation in the disabled-led movement for five months. The collected data were analyzed through the process and method of inductive topic analysis.
The journey to finding their "place" began with the loss of their "place." After being diagnosed with a mental disease, the daily life of the mentally challenged was all about treatment and they did not have much choice in their life other than living as a patient. They had to quit work or school for treatment and lost contact with their friends, without engaging in any social activity. This course of mentally challenged people's lives was the process of losing their "place." What I mean by "place" symbolizes multilayer space for humans who are individual beings but are at the same time destined to live as a social being. In other words, both the loss of place and the journey to finding it have to do with how "I" as an individual being meet and interact with "I" as a social being.
The analysis carried out in this study shows how mentally challenged people, who have lost their place, find and take their place as a social being through an official activity of disabled-led movement, and how this activity leads them to a cycle of new interpretations of themselves.
The subjects' first step toward joining a disabled-led movement began with an attempt to become a "part" of society. Since getting a job at a workplace is evidently an externally recognized social activity, earning a "job title" of disability activist came as great satisfaction to them. So, in the beginning, rather than actively engaging in the disabled-led movement, they were just starting off with entering the social space by gaining a socially recognized role and positioning themselves.
As the study participants got involved in the disabled-led movement organization, they were given an official role and this was an external token that they were recognized in the real world to a certain degree. But for those who failed to discover any significance of their role as disability activist, they could not find new motivation to change their existing self-awareness that they were an incapable and weak being. In other words, without developing a self-awareness of their role as activist, not only were they not motivated to engage in the movement, they were not led to a cycle of new interpretations of themselves.
Working in an organization, the subjects of the study came to develop an awareness that disabled-led movements are a socially meaningful activity, and that it is themselves who should be agents in addressing social difficulties suffered by the mentally challenged. This led them to develop positive attitudes, such as self-esteem and self-confidence, which made it possible for them to form new interpretations and understanding of "I." By discovering that they were playing an important social role, they, as mentally challenged ones, who had perceived themselves as failures and weak people, changed the way they look at themselves. At this point, it can be said that the disabled-led movement changed from the place of "work" to the place of "life." The scene that I witnessed that best represented this transition was the moment when they started to "speak" about their mental disease, rather than just being "diagnosed" with it. The fact that they were now speaking about their illness shows that their view of the world as mentally challenged people, who had had to live with a passive and pliable attitude toward life, changed.
It was through "speech" and "work," the two means necessary to carry out a disabled-led movement activity, that the job the participants first started under the title of disabled-led movement activist ultimately allowed them to take their "place" where they were empowered to speak about their own individuality and uniqueness. The position of activist provides the mentally challenged with places where they can 'speak'. Activists participate as debaters in debates, as lecturers in places as university and refresher training, and as presenters at rallies. This is an important and central means to help their peers. Until then, words from them as mentally challenged people were just for experts to make judgments or diagnoses. Whatever they said, it was not thought much of, nor was it even accepted as a normal conversation.
"Work" also served as a very important channel for the mentally challenged, who had been limited in their social role, to engage in meaningful social activities. Especially, the activists took part in all the work stages from planning to preparation, execution, and wrapping up. When an activist, as a person in charge, is entrusted with certain work "in its entirety," it imposes greater responsibility for the work on them and more choices are given to make. When they are faced with a wider range of choices, they end up giving more efforts to make the best choices. And the power to think and control is acquired going through this process.
Even though the disabled-led movement provided an important environment like this, where the mentally challenged could lead a normal life, many of the participants gave up this place. What undermined their position as activist was internal factors originating in the characteristics of their mental illnesses, as well as external effects such as working under stress because they lacked skills required to perform the given work or financial difficulties due to low wages. However, even if they quit their work for these reasons, they continued to pay attention to social issues involving the mentally challenged in their daily lives and tried to participate directly or indirectly in disabled-led movement activities.
This study analyzed the changing process occurring in the mentally challenged through their participation in a disabled-led movement, and the factors driving such a change. Their implications for the theory and practice of mental health social work are as follows.
First, the analysis results of this study support the goals and important values pursued by the existing empowerment and recovery paradigm, while at the same time showing how theoretical concepts are implemented in the lives of the mentally challenged. The recovery paradigm starts from a critical view of individuals losing their individuality and uniqueness after they are diagnosed with a mental illness. This study has clearly shown that the participants liberated themselves from their disease-centered identity by repositioning their status from "just being diagnosed with an illness" to "speaking about it." The empowerment standpoint is said to grant power and ability to those who are relatively in a state of lack of power. I believe this deficiency is fundamentally a social and structural problem and shares an interface with understanding disability issues from disabled-led movements. Grasping the social problems experienced by the mentally challenged in the context of power was an important starting point in their self-awareness as an activist.
Second, the results of this study can suggest a new interpretation of the kind of professionalism to be pursued on the site where mental health social work is practiced. An obvious contrast between the participants' experience as a user of different forms of social welfare services before he joined the disabled-led movement, and their experience as an activist revealed an imbalance of power between service providers and the mentally challenged within the social welfare system. The moment those who provide help define a "better condition" for those who receive it, the balance of power gets tipped in favor of the former, because that empowers the former with resources to give a definition of "help." If they do not accept the better condition defined by providers, help gets limited for them. To be able to set up problems suffered by mentally challenged users and recognize them as agents in addressing them, the resources and methods with which to do so must be open to them as well. Care and protection need not be limited to fall under the role of social workers only. There is no need for a clear distinction between those who set rules for the space and those who follow them. To this end, I would suggest that the current paradigm of professionalism required in the field of mental health social work be converted in a way that emphasizes its role as partner working together with the mentally challenged.
