최근 학령기 아동들 가운데 주의가 산만하고 과잉활동적인 즉, ADHD 아동들에 대한 관심이 증가되고 있다. 이 장애는 예전에 비해 출현율이 높아지고 있고 가정 및 학교, 사회생활, 또래 관계에 있어서 발생하는 문제행동들로 많은 어려움을 겪고 있다. 그리하여 본 연구는 무용/동작치료 프로그램을 통해 ADHD 아동의 문제행동을 변화시켜 무용/동작치료의 효과성을 검증하는데 목적을 두었다.
본 연구의 무용/동작치료 프로그램은 무용/동작치료 선구자인 Marian Chace의 무용/동작치료 기본 원리 중 Rhythmic Group Activity 원리 그 하위 목표를 바탕으로 진행하였다. Marian Chace는 리듬을 통해 신체를 자각하고 의사소통하기 위해 치료적 수단으로 사용하였는데 Rhythmic Group Activity 원리에서 리듬을 강조하였다. 그리고 단체 및 그룹 활동을 통해 행동을 조직화하고 감정을 표현할 수 있게 하였다. 이는 학교나 사회, 단체 생활에서 그리고 자기통제력 결핍으로 타인과의 상호작용과 의사소통에 문제를 일으키는 ADHD 아동에게 리듬의 인식을 강조한 무용/동작치료 프로그램이 문제행동 변화에 영향을 줄 수 있다는 의미이다. 또한 아동기는 이미 신체발달과 감각운동이 발달되어 있으므로 아동의 신체 발달 영역에서 생활과 밀접한 몸과 움직임으로 신체를 협응시켜 신체인식을 높이기 위해서는 무용동작치료를 통해 신체리듬을 익혀 신체자각을 시키는 것이 필요하다.
Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램이 ADHD 아동의 문제행동 변화의 효과를 목적으로 2004년 11월 6일에서 12월 31일까지 ADHD 아동을 모집하여 16명을 선발하였다. 치료집단, 통제집단 각 8명씩 무선배치하여 치료집단 8명 대상으로 2005년 1월 22일부터 4월 9일까지 매주 1회 50분씩 총 12회기의 무용/동작치료 프로그램을 서울시에 소재한 00복지관의 무용/동작치료실에서 실시하였다.
ADHD 아동의 문제행동 변화를 평가하기 위해 K-CBCL 총문제행동 증후군 척도, SCRS를 사용하여 부모에 의해 사전-사후 검사를 실시하였고 아동의 매 회기 문제행동 변화를 보기 위해 ACRS를 사용하여 보조교사 4명이 평가하였다. 자료 분석은 ADHD 아동이 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램 참여 전과 후에 문제행동 변화와 자기통제력을 알아보기 위해 집단별 t-검증을 하였다.
그 결과, 첫째 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램은 ADHD 아동의 문제행동 변화를 시킬 것인가? 치료집단은 통제집단의 비해 K-CBCL 총문제행동 증후군, 내재화 문제, 외현화 문제가 유의미하게 나타났으며 K-CBCL 하위영역 중에서 위축, 우울/불안, 사회적 미성숙, 주의집중 문제, 공격성이 감소되어 나타났다. 따라서 본 연구는 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램이 ADHD아동의 문제행동을 감소시키는데 효과를 준 것으로 나타났다.
둘째, Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램은 ADHD 아동의 자기통제력을 증진시킬 것인가? 치료집단은 통제집단의 비해 자기통제력이 사전-사후검사가 유의미하게 나타났다. 따라서 본 연구는 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램이 ADHD 아동의 자기통제력을 증진시키는데 효과를 준 것으로 나타났다.
셋째, 치료집단은 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램 과정에서 어떤 변화가 있을 것인가? 치료집단 ADHD 아동을 ACRS로 평가하여 1회기 평균점수 19.63점에서 12회기에는 13.00점으로 6.63점이 낮아진 차이가 보였고 각 회기마다 점차 감소되었다. 따라서 치료집단은 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램 과정에서 ACRS의 점수가 감소되어 문제행동의 변화가 나타났다.
본 연구는 Rhythmic Group Activity 원리를 적용한 무용/동작치료 프로그램을 ADHD 아동에게 실시하여 프로그램 효과를 입증했다는 점에서 의의가 있다. 즉, 약물치료의 제한점을 보완할 수 있는 방법으로 행동치료, 인지행동치료를 이어 표현예술치료의 한 방법으로 ADHD 아동의 대한 치료적인 대안이 될 수 있음을 시사한다.
Recently people are more interested in attention-deficient and hyperactive children, that is, ADHD children among the children of school age. Compared with past, ADHD children are increasing and they are in trouble at home, in school, and social life including peer relationship. Therefore this study is to verify that dance/movement therapy’s effect on the problematic behaviors of ADHD children.
The dance/movement program in this study is based on one of the principles of Marian Chace, especially Rhythmic Group Activity and its subcategories as DMT rationale. Marian Chace used rhythm as a therapeutic factor for body awareness and communication and she focused on rhythm through Rhythmic Group Activity. According to her theory and practice, organizing behavior and expression of emotions through group activity is considered therapeutic. It means by the potentiality of DMT’s effect on ADHD children who have difficulty in interaction and communication due to the deficit of self-control ability in their communities including school. In addition, By coordinating through body and movement connected with everyday life and enhancing body awareness DMT helps ADHD children to learn body rhythm and awareness because children have already developed their physical development and sensorymotor.
In order to study the effect of the DMT program based on Rhythmic Group Activity, this program was applied to 8 ADHD children group(experiment group), 12 weeks, once a week, from Jan 22 to April 9 and another 8 ADHD children group(control group). All the children are selected at a social health center in Seoul.
To measure the change of problematic behaviors of the children, their parents rated K-CBCL Total Behavior Total Behavior Problems scale, SCRS before and after the DMT program while ACRS was used by their teachers to measure the change of problematic behaviors. The analysis was executed by t-test in order to verify DMT program’ effect, that is, the difference between experiment and control group as to the change of problematic behaviors and self control ability.
The result is as follows:
First, will the DMT program based on Rhythmic Group Activity change ADHD children’s problematic behaviors? As the result of the analysis of the pre and post test on the change of K-CBCL Total Behavior Problems, Internalizing Problems, Externalizing Problem was meaningful difference between the two groups. In subscales such as withdrawal, depression/anxiety, social immaturity, attention problem, aggression, there was a significant decrease. It means that the DMT program was effective for the decrease of problematic behaviors.
Secondly, will the DMT program based on Rhythmic Group Activity increase the self-control ability of ADHD children? Compared with control group significant change appeared through pretest and posttest. It means that the DMT program was effective for the increase of self-control ability.
Thirdly, did the change of problematic behaviors in the experiment group exist during the DMT program? There was a significant decrease between pretest and posttest when the experiment group was rated by ACRS. It means that the DMT program was effective for the decrease of problematic behaviors.
Finally this study shows that DMT program based on Rhythmic Group Activity is effective on ADHD children. That is, DMT is also an effective psychotherapy like behavior therapy and cognitive-behavior therapy that supplement medicine treatment for ADHD children and it can be an alternative psychotherapy for ADHD children as one of expressive arts therapy.
