진폐증환자의 혈액학적변화에 관한 연구 = An Evaluation of Hematological Changes in Patients with Pneumoconiosis

Various hematological studies were performed on 88 patients with pneumoconiosis admitted in Labour and Welfare Corporation, Dong Hae Hospital. The patients were classified according to the degree of the disease by ILO's International Classification, category 0, 1, 2, 3. 23 were category 1, 45 were category 2 and 20 were category 3.
The results of hematological changes were summarized as follows:
1. The mean values of serum eryghropoietin as well as the reticulocyte counts were significantly higher in patients with pneumoconiosis than normal control(p<0.01), which indicated an existence of compensatory erythropoietic mechanism to the hypoxia induced by a decrease of pulmonary function.
2. The levels of serum ferritin were higher than the upper limit of normal in 27% of patients with pneumoconiosis. In severe cases (category 3), 52% of the patients showed high ferritin values.
3. The mean values of hemoglobin and hematocrit of patient with category 3 were significantly higher than those of category 2 (p<0.05).
4. The mean platelet count of category 2 was significantly higher than those of category 1 and 3 (p<0.05).
5. The mean value of erythrocyte sedimentation rates in patients with pneumoconiosis was 15.6 mm/h, which exceeded the upper normal limit of Korean males.
6. The mean value of total iron binding capacity in category 2 was significantly lower than those of category 1 and 3 (p<0.05).
7. A case of iron deficiency anemia, two cases of secondary polycythemia and a case of macrocytic anemia were found among the 88 patients with pneumoconiosis.
8. No remarkable findings were noted on mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, red cell distribution width, mean platelet volume, plateletcrit, platelet distribution width, white blood cell count and differential count, bone marrow aspiration and biopsy, and serum iron levels in patients with pneumoconiosis.
