The purpose of this study is to report if dance movement psychotherapy is effective in reducing post-traumatic stress disorder (PTSD) and in improving brain waves and ego-identity among sexually-abused adolescents.
In this study I first described problems of psychological and physical traumas, relations with the emotional brain, and actions of emotions and brain waves as a theoretical approach to post-traumatic disorder among sexually-abused adolescents and then presented ego-identity and theoretical grounds of physical and psychological effects of dance/movement psychotherapy.
I developed the nine-stage dance/movement psychotherapy program for sexually-abused adolescents on the basis of the results of Ryu researches (2002, 2006) and the theory of post-traumatic stress among sexually-abused adolescents. To test effectiveness of the research, I tried to see if dance/movement psychotherapy might affect the decrease in post-traumatic stress, improvement in ego-identity, and changes in brain waves among sexually-abused adolescents before and after the treatment with PTSD, AEIS, and brain waves measurements (the prefrontal lobe, the frontal lobe, the temporal lobe, the parietal lobe) in experimental and control groups.
To do this, 30 sexually-abused girls (14 to 19 of age) sheltered in Seoul, Gyeonggi Province, North Gyeongsang Province and South Gyeongsang Province were divided into two groups to implement the program in 15 sessions, one session per week and two hours per session, from April to August 2008; the experimental group was treated with the dance/movement psychotherapy program while the control group was asked to read in relation to stress; both groups went through three brain waves measurements to see changes in post-traumatic disorder symptoms in addition to PTSD and AEIS as pre-test and post-test.
The results of this study can be summarized as follows:
First, there were statistically significant differences in post-traumatic stress disorder symptoms between the experimental and control group; for sub-areas, A-type characteristics showing symptoms of fear, anxiety, and terror, B-type characteristics with symptoms of repetitive memory, terror, and continuous reexperience of sorrow, C-type characteristics showing symptoms of response paralyses and avoidance to reject things related to an accident, and D-type characteristics showing symptoms of the increase in continuous arousal of hypersensitive responses and insomnia from an accident scene all decreased in the experimental group treated with the program; therefore, dance/movement psychotherapy relieved post-traumatic stress disorder symptoms among sexually-abused adolescents and was effective in all of its sub-areas.
Second, there were significant differences in sub-areas of ego-identity, such as self-direction, self-acceptance, leading, and intimacy, with no significant difference in future confidence and goal orientation, between the experimental and control group. Therefore, dance/movement psychotherapy was effective in improving self-direction, self-acceptance, leading, and intimacy, but not future confidence and goal orientation, in terms of ego-identity.
Third, I measured and analyzed brain waves to see more minute changes in post-traumatic disorder symptoms; terror, helplessness, pain reflection, arousal reaction, anger, and hypersensitivity found in post-traumatic stress disorder mean the excited condition of the limbic system; as sexually-abused adolescents have a poor ability to analyze and classify arousal information of the brain and have the emotional brain activated, I tried to measure eight parts of the prefrontal lobe, the frontal lobe, the temporal lobe, and the parietal lobe of their brain to report changes in brain waves before and after program treatment for post-traumatic stress disorder symptoms.
As a result, increase and decrease patterns could be found in alpha, beta, and theta in each band, together with significant changes in certain areas. First, the experimental group showed an increase in theta, alpha, beta, and gamma waves at the prefrontal lobe but a decrease at other areas in post-test. To this contrary, the control group showed an increase at all areas, because the experimental group had emotions of post-traumatic stress connected with the cognitive area at the prefrontal lobe in the emotional brain, which is the limbic system, improving cognitive capacity to analyze and classify aroused information and making cognitive changes in emotional problems while the control group had all areas increased, arousing all areas excessively. An increase in SEF 50% means arousal of the brain at the increased area; the experimental group had it increased only at the left prefrontal lobe and decreased at the other areas, having arousal of the brain waves drop, while the control group had it rise. An increase in alpha SEF50% means an increase in convenience of the brain; it increased at all areas in the experimental group but failed to increase at all areas in the control group.
The prefrontal lobe plays a role of important functions to combine information from sensory, memory, and compensatory systems through connection of thalami and the limbic system; since it is by the limbic system that those with post-traumatic stress disorder commit an accident or make an action hard to understand because of confusion in judgement and association, it is necessary to activate functions of the prefrontal lobe connected relatively in order to control it. The experimental group showed significant changes in the prefrontal part generally.
As a result, dance/movement psychotherapy can make approaches to express and solve depression, anger, aggression, hostility, helplessness, and dissociation hidden in the inner world of sexually-abused adolescents desirably, creatively, and spontaneously through the body; it is believed that dance/movement psychotherapy works as a medium to test the body shown in emotions and reconstruct mental, emotional, and behavioral changes in order to reduce post-traumatic stress disorder and improve ego-identity among sexually-abused adolescents who need integration of mental, physical, and mental conditions.
본 연구는 무용동작 심리치료가 성폭력 피해 청소년들의 외상후 스트레스 와 뇌파 및 자아정체감에 효과가 있는지를 보고자하는데 목적이 있다.
본 연구에서는 류분순(2002, 2006)의 연구 결과와 성폭력 피해청소년의 외상후 스트레스 이론에 근거해서 성폭력 피해 청소년을 위한 9단계 무용동작 심리치료 프로그램을 개발하였다. 연구의 효과 검증을 위해서는 실험집단과 통제집단에게 처치 전후로 외상후 스트레스 장애증상 요인척도와 3차례의 뇌파 측정(전전두엽, 전두엽, 측두엽, 두정엽)및 자아정체감 척도를 실시하여 무용동작 심리치료가 성폭력 피해 청소년들의 외상후 스트레스 감소와 뇌파의 변화 및 자아 정체감 향상에 효과가 있는지를 검증 하였다.
연구 대상과 방법은 서울, 경기, 경북, 경남 지역의 기관에 보호 되어 있는 성폭력 피해 여자 청소년 30명(14-19세)을 나이, 성폭력 기간, 재학여부, 성매매 노출 유무, 등에 따라 공정하게 두 집단으로 나누어, 2008년 4월에서8월까지 매주 한회기 2시간씩 15회기의 프로그램을 실시하였으며, 실험집단은 무용동작 심리치료 프로그램을 통제집단은 스트레스 관련 독서를 하게 하였고 두 집단 모두 사전 사후로 외상후 스트레스 장애 검사, 3차례의 뇌파 측정, 자아 정체감 검사를 실시하였다.
본 연구의 결과를 요약하면 다음과 같다.
첫째, 외상후 스트레스 장애 증상은 실험집단과 통계집단에서 의미 있는 차이를 보였는데, 하위 영역별로 두려움, 불안, 공포의 증상을 보이는 A형 특징과 반복기억, 공포, 슬픔의 지속적 재경험 증상의 B형 특징, 사건과 연관된 일들에 거부하는 회피와 반응의 마비 증상을 보이는 C형 특징, 사고 장면에 대한 불면증 및 과민반응의 지속적인 각성증가의 증상을 보이는 D형 특징 모두 프로그램을 처치한 실험집단에서 증상이 완화되어 무용동작 심리치료는 성폭력피해 청소년들의 외상후 스트레스 장애 증상을 완화시키는 것으로 확인 되었다.
둘째. 자아정체감 분석결과에서는 실험집단과 통제집단에서 하위 영역별로 주체성, 자기수용성, 주도성, 친밀성에서 의미 있는 차이를 보였으며, 미래 확신성과 목표지향성에서는 차이를 보이지 않았다, 이것은 무용동작 심리치료가 성폭력피해 청소년들의 자아정체감 향상에, 미래 확신성과 목표지향성을 제외한 주체성, 자기수용성, 주도성, 친밀성을 향상시키는데 효과가 있는 것으로 나타났다.
셋째. 외상후 장애 증상의 보다 세밀한 변화를 보고자 뇌파를 측정하고 분석하였다. 외상후 스트레스 장애에서 나타나는 공포, 무력감, 고통회상, 각성반응, 분노, 과민상태 등은 뇌의 변연계가 흥분된 상태로 성폭력 피해 청소년들은 뇌의 각성 정보를 분석하고 분류하는 능력이 약하고 감정뇌가 활성화된 상태이므로 이들에게 뇌의 전전두엽, 전두엽, 측두엽, 두정엽, 8군데를 측정하여 외상후 스트레스 장애증상에 대한 프로그램 처치 전 후의 뇌파변화를 보았다.
그 결과 각 대역별 알파, 베타, 세타, 감마에서 증감 패턴을 확인할 수 있었으며, 특정 부위에서 변화를 확인할 수 있었다. 먼저 실험집단에서는 사후에 전전두엽에서 세타파, 알파파 ,베타파, 감마파 등의 증가가 있었으나 다른 부위에서는 감소가 있었다. 반면 통제집단에서는 전체 부위에서 증가하는 결과를 보였는데, 이는 실험집단의 경우 외상후 스트레스 장애의 감정이 변연계인 감정 뇌에서 전전두엽의 인지영역으로 연결되어 각성된 정보를 분석하고 분류 할 수 있는 인지적 능력이 증가되어 감정적 문제에 대해 인지적인 변화를 가져오게 된 것이라고 볼 수 있으며, 통제집단의 경우에는 전체 부위에서 증가되어, 전체 부위가 지나치게 각성된 것으로 나타났다. SEF50%의 증가는 증가 부위의 뇌의 각성을 의미하는데 실험집단은 좌측 전전두부만 증가하고 나머지 부위는 감소된 것으로 나타나 뇌파의 각성도가 떨어졌으나 통제집단의 경우 뇌파의 각성도가 올라갔음을 볼 수 있었다. 또한 알파SEF50%의 증가는 뇌의 정서적 쾌적성의 증가를 의미하는데, 실험집단의 경우 전체부위에서 증가하였지만, 통제집단의 경우는 전체부위에서 증가하지 않았다.
전전두엽은 시상과 변연계 구조의 연결부분을 통해 감각, 기억체계 및 보상체계에서 온 정보를 통합하는 중요한 기능의 역할을 하는데, 외상후 스트레스 장애자는 판단, 연상 등에 혼란이 일어나 사고를 일으키거나 이해 할 수 없는 행동을 하는 것은 모두 변연계(limbic system)에 의해 일어나므로 여기에 대한 조절은 상대적으로 연결된 전전두엽의 기능이 활성화되어야한다. 그런데 본 연구에서 실험집단은 전체적으로 전전두부에서 의미 있는 변화를 보여주었다.
이상의 결과로 볼 때, 무용동작 심리치료는 성폭력 피해 청소년들의 내면에 잠재된 우울, 분노, 공격성, 적대감, 무력감, 해리 등등의 외상후 장애 스트레스 증상을 신체를 통해 바람직하게 창의적, 자발적으로 다양하게 표출하고 풀어 나갈 수 있게 접근 할 수 있는 것으로 판단되며, 정신과 몸, 마음 상태의 통합을 필요로 하는 성폭력 피해 청소년들에게 무용동작 심리치료는 이들의 외상후 스트레스장애의 감소와 자아정체성의 향상을 위해 감정으로 나타나는 신체를 검토하고 정신과 감정, 행동의 변화를 재구성하는 매개체의 역할을 하는 것으로 확인 되었다.
