Ischemia and reperfusion (I/R) injury is a major cause of morbidity and mortality in liver surgery and transplantation. Alcohol intake and obesity are major causes of fatty liver. Fatty livers in human are susceptible to greater I/R injury and a higher incidence of primary graft nonfuntion after transplantation. primary graft nonfuntion of fatty liver allograft is one of the factors causing shortage of donor livers. Several hypotheses have been suggested to explain to decreased tolerance of fatty liver to I/R injury compared with normal liver. However, the reason why alcoholic fatty livers are more susceptible to I/R injury is yet to be elucidated. Therefore, this study was designed to evaluate the alteration of microsomal cytochrome P450 (CYP) and inflammatory mediators in alcoholic fatty liver exposed to hepatic I/R . Rats were subjected to 60 min of hepatic ischemia and seque5 h reperfusion. After 5 h reperfusion, liver microsomes were isolated CYP450 isozymes. The hepatic I/R significantly increased serum aminotransferase activities. The serum aminotransferase activities of the I/R feeding ethanol diet (ED) were significantly higher than that of the I/R feeding control diet (CD). Microsomal CYP content and NADPH-cytochrome P450 reductase activity were decreased exposed to I/R. however, no significant difference was detected between CD and ED group. The CYP1A1 mRNA expression of the I/R group feeding ED was significantly lower than that of I/R group feeding CD. The CYP1A2 activity of the I/R group feeding ED was significantly lower than that of I/R group feeding CD. However, CYP2E1 activity and mRNA expression of the I/R group feeding ED were significantly higher than that of I/R group feeding CD. This study showed that the TNF-αand IL-6 mRNA expression was increased 5 h reperfusion. In addition, this increase in I/R group feeding ED was significantly higher than in I/R feeding ED. The mRNA expression of iNOS was increased 5 h after reperfusion. And the mRNA expression of iNOS in I/R group feeding ED showed the tendency of increase. Our findings suggest that drug metabolizing dysfunction in alcoholic fatty liver are more severe and the expression of pro-inflammatory mediator are more apparent exposed to I/R.
