Helicobacter pylori (H. pylori) is a gram-negative, microaerophilic, and spiral-shaped bacterium that colonizes the gastric mucosa. H. pylori contributes to chronic inflammation of gastric mucosa that can lead to chronic gastritis, peptic ulcer, gastric adenocarcinoma, and gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Nucleotide oligomerization domain (NOD)-like receptors (NLRs) are typical members of the cytoplasmic pattern recognition receptors (PRRs) expressed on immune cells and non-immune cells. NLRs play an important role in the innate immune response by recognizing pathogen-associated molecular patterns (PAMPs) and damage-associated molecular patterns (DAMPs). Nucleotide-binding oligomerization domain 2 (Nod2) is a representative member of the NLR family that recognizes the bacterial peptidoglycan-derivative, muramyl dipeptide (MDP). Among the inflammasome NLRs, NLRP3 and NLRC4 are activated by detecting various microbial molecules, inducing the production of IL-1β and IL-18. Activation of Nod2 and the NLR-subset inflammasome regulates the production of proinflammatory cytokines for host defense against pathogens. The IL-1B gene polymorphism is well known in gastric cancer, and neutrophils are recruited by H. pylori infection and are associated with the development of gastric cancer. However, the mechanism of IL-1β productionn in H. pylori-infected neutrophils is not well known.
Many studies have reported that H. pylori induced IL-1 production in immune cells such as dendritic cells and macrophages via NLRP3 inflammasome. Furthermore, recruitment of neutrophils and macrophages was regulated by IL-1β in the gastric tissues of H. pylori-infected mice. However, the mechanism of IL-1β production in neutrophils by H. pylori is still controversial. Moreover, studies on PRRs including Toll-like receptors (TLRs) and Nod1 in gastric epithelial cells during H. pylori infection have been reported. Some little is known of the role of Nod2, therefore whether host and bacterial factors involved in the role of the NLRP3 inflammasome and Nod2 on the innate immune response in neutrophils and gastric epithelial cells were investigated.
In this study, it was investigated whether host and bacterial factors involved in the regulation of H. pylori-induced IL-1β production in neutrophils. Thioglycollate-elicited neutrophils (pNph) and bone marrow-derived neutrophils (BMNs) were isolated from mice and infected with H. pylori at the indicated dose and time points. H. pylori-induced IL-1β production was abolished in NLRP3-, ASC-, and caspase-1/11-deficient neutrophils. An inhibitor assay revealed that various extracellular signals, including ATP/P2X7R, potassium efflux, reactive oxygen species production, and release of cathepsin B were involved in IL-1 production in H. pylori-infected neutrophils, suggesting essential role for NLRP3 inflammasome in IL-1β production against H. pylori infection. Host TLR2, but not TLR4 and Nod2, was also required for transcription of NLRP3 and IL-1β as well as secretion of IL-1β. H. pylori lacking cagL, a key component of the type IV secretion system (T4SS), induced less IL-1β production in neutrophils than isogenic WT strain. Moreover, T4SS was involved in caspase-1 activation and IL-1β maturation in H. pylori-infected neutrophils. These results demonstrate that host TLR2 and NLRP3 inflammasome and bacterial T4SS are essential factors for IL-1β production in neutrophils response to H. pylori.
It was investigated whether other bacterial factors contributed to IL-1 production in neutrophils in response to H. pylori. It was found that FlaA was essential for H. pylori-mediated IL-1β production in neutrophils, whereas vacA and ureA were dispensable. However, IL-1 production was not affected by FlaA in H. pylori-infected bone marrow-derived dendritic cell (BMDCs). These results suggest that H. pylori flagellin limitedly induced IL-1 production in neutrophils. Moreover, FlaA was involved in priming and inflammasome activation of NLRP3 in H. pylori-infected neutrophils. TLR5 and NLRC4, representative receptors of flagellin, were not required for H. pylori-induced IL-1β production in neutrophils. Instead, bacterial motility was essential for the production of IL-1β in response to H. pylori. These results demonstrate that the motility of H. pylori flagellin is required for regulation of both priming and activation of NLRP3 inflammasome in neutrophils.
Finally, the role of Nod2 was investigated in the innate immune responses of gastric epithelial cells. Many studies have shown that Nod1 contributes to host defense against H. pylori infection, but the precise molecular mechanism by which Nod2 regulates host defense in gastric epithelial cells is unknown. Three human gastric epithelial cells (SNU638, MKN-45, and AGS) treated with MDP and H. pylori were used to confirm the expression of Nod2 and Rip2, and downstream signaling pathway was identified. It was found that the gene and protein expression of Nod2 and Rip2 were upregulated in MDP-treated human gastric epithelial cells. In addition, MDP induced NF-κB and MAPK-mediated IL-8 and antimicrobial peptide production in human gastric epithelial cells. Moreover, H. pylori induced the expression of Nod2 and Rip2 in a T4SS-dependent manner, but the expression was cell line-specific. Our findings suggest that Nod2 can regulate innate immune response in gastric epithelial cells.
In conclusion, host TLR2 and NLRP3 inflammasome contributes to H. pylori-induced IL-1 production in neutrophils. In addition, T4SS and motility of flagellin are key bacterial factors that regulate IL-1 production in neutrophils in response to H. pylori. Moreover, Nod2 is involved in host immune response in gastric epithelial cells. MDP, a Nod2 agonist, induced Nod2-Rip2-mediated production of IL-8 and -defensin via activation of NF-κB/MAPK in human gastric epithelial cells. H. pylori also can induce IL-8 production in gastric epithelial cells. These results will be useful in exploring and preventing new treatment strategies for H. pylori infected diseases.
Helicobacter pylori (H. pylori)는 그람 음성, 미세호기성의 나선형균으로 사람의 위장 질환의 원인이 된다. 노드 유사 수용체 (Nucleotide oligomerization domain (NOD)-like receptors, NLRs)는 대표적인 세포질 패턴 인식 수용체 (pattern recognition receptors, PRRs)로 면역세포와 비 면역세포에서 발현된다. 노드 유사 수용체는 병원체연관분자패턴(pathogen-associated molecular patterns, PAMPs)과 손상연관분자패턴(damage-associated molecular patterns, DAMPs)을 인식하여 선천면역 반응에 중요한 역할을 한다. 대표적인 노드 유사 수용체 중 하나인 Nucleotide-binding oligomerization domain 2 (Nod2)는 세균의 세포벽 성분인 펩티도글리칸의 muramyl dipeptide (MDP) 부분을 인식한다. 노드 유사 수용체 중에서 NLRP3와 NLRC4는 다양한 미생물 분자를 인식함으로써 활성화된다. Nod2와 NLR 관련 수용체들의 inflammasome의 활성화는 병원균에 대한 숙주 방어를 위해 염증성 사이토카인의 생성을 조절한다.
많은 연구에서 H. pylori는 NLRP3 inflammasome을 통해 수지상 세포와 대식세포와 같은 면역 세포에서 IL-1β 생성을 유도한다고 보고했다. 그러나 H. pylori 감염 시 호중구에서의 IL-1β 생성 메커니즘은 여전히 논란의 여지가 있다. 또한, H. pylori 감염 시 위상피세포에서 톨 유사 수용체 (Toll-like receptors, TLRs)와 Nod1을 포함한 패턴 인식 수용체에 대한 연구는 보고된 바 있지만 Nod2의 역할은 아직 밝혀진 바가 없다. 따라서, 본 연구에서는 호중구와 위상피세포에서 선천 면역 반응에 대한 NLRP3 inflammasome과 Nod2의 역할에 대해 규명하고자 하였다.
본 연구에서는 호중구에서 H. pylori에 의한 IL-1β 생성에 관여하는 숙주와 박테리아 요인을 조사하였다. 이를 위해서 마우스에서 thioglycollate로 유도하여 복강 내 호중구를 분리하였고, 마우스 골수로부터 호중구를 분리하여 H. pylori를 지정된 농도와 시간 조건으로 감염시켰다. H. pylori에 의한 IL-1β 생성은 NLRP3-, ASC- 그리고 caspase-1/11-결핍 마우스로부터 얻은 호중구에서 완전히 감소하였다. 또한, NLRP3 inflammasome의 다양한 활성 인자인 ATP/P2X7R, potassium efflux, ROS 그리고 cathepsin B의 억제제 처치에 의해 H. pylori에 의해서 유도된 IL-1β의 생성이 유의적으로 낮아지는 것을 확인하였다. 이러한 결과는 호중구에서 H. pylori에 의한 IL-1β의 생성에 NLRP3 inflammasome이 중요하다는 것을 나타낸다. 호중구에서 H. pylori 감염에 의한 NLRP3와 IL-1β의 전사와 분비는 TLR2에 의해서 조절되었지만, TLR4와 Nod2의 영향은 받지 않았다. H. pylori type IV secretion system (T4SS)의 핵심 구성요소 인 cagL 결핍 균은 호중구에서 WT 균보다 더 적은 IL-1β 생성을 유도하였다. 또한 T4SS는 H. pylori가 감염된 호중구에서 caspase-1 활성과 IL-1β 성숙에 관여하였다. 이러한 결과는 숙주 TLR2와 NLRP3 inflammasome과 박테리아 T4SS가 호중구에서 H. pylori에 의한 IL-1β 생성에 필수적임을 나타낸다.
다음으로, 호중구에서 H. pylori 감염에 대한 IL-1β 생성에 다른 박테리아 요소가 관여하는지 조사하였다. 호중구에서 H. pylori에 의한 IL-1β 생성에 FlaA가 중요하다는 것을 밝혔으며, vacA와 ureA는 영향을 주지 않았다. 하지만 BMDCs에서 FlaA는 IL-1β 생성에 영향을 주지 않았다. 본 연구에서는 H. pylori flagellin이 호중구에서 제한적으로 IL-1β 생성을 유도한다는 것을 나타낸다. 또한 H. pylori가 감염된 호중구에서 FlaA는 NLRP3 inflammasome의 priming과 활성을 조절하였다. 대표적인 flalgellin의 수용체인 TLR5와 NLRC4는 호중구에서 H. pylori에 의한 IL-1β 생성에 필요하지 않았다. 대신에, flagellin의 주요한 기능인 운동성은 호중구에서 H. pylori에 의한 IL-1β 생성을 조절하였고, 이는 NLRP3 inflammasome의 priming과 활성 조절에 박테리아 운동성이 필수적임을 나타낸다.
마지막으로, 위상피세포의 선천 면역반응에서 Nod2의 역할을 조사하였다. 많은 연구에서 H. pylori 감염에 대한 숙주 방어에 Nod1이 기여한다고 보고했지만, Nod2가 위상피세포에서 숙주 방어를 조절하는 정확한 분자 기전은 아직 밝혀진 바가 없다. 이를 위해서 세 종류의 사람 위상피세포주 (SNU638, MKN-45, 그리고 AGS)를 사용하여 MDP와 H. pylori에 의한 Nod2와 Rip2의 발현을 확인했으며, 하위 신호기전을 확인하였다. 위상피세포에서 MDP 처리에 의해서 Nod2와 Rip2의 유전자와 단백질 발현이 증가하였다. MDP는 NF-κB와 MAPK를 활성화시켜서 IL-8과 antimicrobial peptides의 생성을 유도하였다. 또한, 위상피세포주 특이적으로 H. pylori는 T4SS에 의한 Nod2와 Rip2의 발현을 유도하였다. In vivo에서 Nod2의 역할을 규명하기 위해서 WT과 Nod2-결핍 마우스에 H. pylori PMSS1를 경구 감염 시켰으며, 감염 4주 후 위 용균액에서 검출되는 세균수를 측정하였다. cagPAI를 발현하는 H. pylori가 감염된 Nod2-결핍 마우스의 위 용균액에서 검출되는 세균 수는 WT 마우스에 비해서 감소하였다. 이러한 결과는 Nod2가 위상피세포에서 선천 면역반응을 조절할 수 있음을 나타낸다.
결론적으로, 숙주의 TLR2와 NLRP3 inflammasome과 박테리아의 T4SS와 flagellin은 호중구에서 H. pylori에 의한 IL-1β 생성에 기여하였다. 또한, Nod2는 위상피세포에서 H. pylori 감염에 대한 숙주 면역 반응에 기여하였다. 따라서, 본 연구 결과에서 확인된 H. pylori에 대한 면역 반응의 이해는 H. pylori 감염 질병에 대한 새로운 치료 전략을 모색하고 예방하는 데 유용한 지표로 이용될 수 있을 것으로 생각된다.
