노인의료서비스의 개선방안에 관한 연구 : 대전지역 의료인을 중심으로 = A Study on the Improvement of Medical Service for the Elderly: focused on the medical team of Daejeon city

This study was intended to search the ways corresponding to the necessity of medical service for the elderly rationally and effectively. The subjects were 444 doctors and nurses of A university hospital in Daejeon and surveyed from Apr. 20 through May 30, 2003. The result is as follows; 1. 270(60.8%) respondents answered that they have many experiences in treating and nursing the elderly, and 174(39.2%) respondents answered that they have no experience or little experience. 2. 431(97.05%) respondents answered that geriatrics hospitals need to be established, and 318 nurses(99.0%) and 113 doctors(91.9%) answered the same. This means that geriatrics hospitals must be established. 3. 425(95.7%)respondents answered that the geriatricians should be trained, and 19(4.3%)respondents answered that they don’t need to. 4. 335(76.1%)respondents answered that the wards specialized for the elderly should be established in the existing hospitals, and 26 respondents answered that there’s no special need to. 5. 396(89.2%)respondents answered that economical burden on nursing fee is heavy, though 48(10.8%)respondents disagreed with the idea. This means that nursing fee imposes a burden on the patients’ families. Most of the elderly have been treated and nursed at the existing general hospital without the characteristics of the infirmities of age considered because of the insufficiency of geriatrics hospitals and geriatricians. The increasing medical fee for the elderly patients’ long-term hospitalization take the opportunity to use medical service away from the elderly. Therefore, the geriatrics hospitals should be continuously extended, and geriatricians should be trained. Also, the special wards for the elderly should be established in the existing hospital. When the medical fee for the elderly is appropriated by tax and the resource of local government, the problems of the medical welfare for the elderly in an aging society can be solved.

국문 초록 (Abstract)

본 연구는 노인의료복지서비스의 필요성에 합리적이고 효율적으로 대응하기 위한 방안을 모색하기 위해 2010년 4월 20일부터 5월 30일까지 대전지역에 위치한 A대학 병원의 의사와 간호사 444명을 대상으로 설문조사를 하였다. 그 결과를 요약 정리하면 다음과 같다. 첫째, 노인환자의 치료·간호경험을 보면 노인을 치료·간호해본 경험이 있는 것으로 나타났다. 둘째, 노인전문병원의 필요성을 보면 노인전문병원이 꼭 필요하다는 것을 나타내주고 있다. 셋째, 노인전문의료인력의 필요성에 대해서는 노인전문의료인력 역시 꼭 필요한 것으로 나타났다. 넷째, 기존병원에 노인전문병동 설치의 필요성은 모두 기존병원에 노인전문병동의 설치가 필요한 것으로 나타났다. 다섯째, 유료간병인 활용시 경제적 부담정도를 보면 환자가족들이 간병료에 대해 부담을 느끼고 있는 것으로 나타났다. 노인전문병원의 지속적인 확충, 전문인력 양성의 필요성, 기존대형병원에 노인전문병동의 설치를 의무화하는 것이 필요하다 생각된다. 또한 노인의료비 부담의 최소화하는 방안을 강구하기 위하여 지방자치단체의 재원으로 충당하거나 가장 완전한 소득 재분배 기능인 세금으로 노인의료비 재원을 마련하는 방법 등도 검토되어져야 고령화 사회의 노인의료복지문제를 해결할 수 있을 것이다.
