세계 각국의 응급의료시스템 도입시기의 인구·보건지표 비교분석 : Comparison of demographic and epidemiologic information at the initial stage of emergency medicine around the world

응급의료체계의 도입은 지역사회를 대상으로 신속한 응급의료를 제공할 뿐만 아니라 한 국가의 건강수준을 향상시키고 질병부담을 감축하는 데 매우 중요한 역할을 한다. 산업화와 도시화가 진행되면서 고소득 국가와 유사하게 중·저소득 국가에서도 비감염성 질환이나 외상으로 인한 사망률이 증가하는 질병의 천이를 보이고 있다. 2000년대 후반 까지도 응급의료체계는 도입과정에 많은 비용이 필요하다는 인식으로 인해 고소득 국가에서 주로 실효성이 있는 의료 분야로 여겨져 왔으나 외상이나 응급질환에 대해 신속한 의료를 제공할 수 있을 뿐만 아니라 보건학적 통계조사가 가능하고 질병의 조기발견과 같은 이차예방이 이루어질 수 있어 중·저소득 국가에서도 응급의학 전문 과목에 대한 교육과 시스템 도입의 필요성이 부각되고 있다. 그러나 응급의료체계는 복잡하고 다양한 요인들로 구성되며 효과를 정량화하는데 어려움이 있어 아직까지 전문가 의견에 한정된 논문 외에는 세계적으로 응급의료체계와 인구·보건지표 수치의 정량적인 변화와 영향에 대해서 연구된 바가 거의 없다.
본 연구의 목적은 응급의학회 혹은 응급의학과 전문의제도가 도입된 나라들을 문헌고찰 및 웹 검색을 통해 조사하여, 각 나라별 응급의학회의 설립 및 응급의학과 전문의제도 도입 시의 인구·보건지표를 도식화 하고, 응급의학회 설립 및 응급의학과 전문의제도 도입 유무 또는 도입 전후의 보건지표 변화를 정량적으로 비교 분석하는 것이다.
WHO, World bank의 데이터베이스에 있는 214개 나라를 기준으로 Pubmed와 각 나라의 응급의학회 홈페이지 검색을 통해 응급의학회 설립연도가 명시된 58개국과 응급의학과 전문의제도 도입연도가 명시된 70개국을 선정하였다. 응급의학회 유무에 따른 인구·보건지표의 차이를 확인하기 위하여 WHO, World bank의 데이터베이스를 이용하여 설립연도가 명시된 58개국과 명시되지 않은 156개국의 1960년부터 2014년 까지 GDP, 인구수, 1000명당 의사 수, 사망원인별 표준화사망률, 사망원인별 사망자수와 같은 인구·보건지표에 t 검정을 시행하였다. 응급의학회 설립 전후에 변화되는 보건지표를 알아보기 위하여 사망률데이터가 있는 44개국에서 사망원인별 사망자수 변화 기울기에 짝지은 t검정(p<0.05)을 시행하였다. 응급의학과 전문의제도 유무에 따른 인구·보건지표의 차이를 확인하기 위하여 도입연도가 명시된 70개국과 명시되지 않는 144개국의 인구·보건지표에 t 검정을 시행하였다. 응급의학과 전문의제도 도입 전후에 변화되는 보건지표를 알아보기 위하여 사망률데이터가 있는 27개국에서 사망원인별 사망자수 변화 기울기에 짝지은 t검정(p<0.05)을 시행하였다. 응급의학회 설립, 응급의학과 전문의제도 도입과 배출시기의 차이가 있을 수 있어 도입 전후 4년에서 8년까지, 5년간의 사망자수 변화에 대한 기울기를 구하고 이를 변수로 사용했다.
응급의학회가 설립되지 않는 나라들에 비해 설립된 나라들에서 GDP (4,795±10,228 vs. 10,228±11,072, p=0.01), 인구수 (14.12x10⁶±32.89x10⁶ vs. 70.62x10⁶±191.86x10⁶ p=0.03), 1000명당 의사수 (1.29±1.43 vs. 2.18±1.44 p<0.01), 심혈관질환으로 인한 표준화사망률 (61.62±128.54 vs. 134.97±142.50 p<0.01), 외상으로 인한 표준화사망률 (13.59±27.17 vs. 34.49±34.63 p<0.01)이 통계적으로 유의하게 높았다. 응급의학회가 설립되기 전에 비하여 설립된 후 5년간의 사망자수 변화 기울기가 감염성질환(145.61 vs. -145.26 p<0.01), 외상(-5.16 vs. -41.34 p<0.01) 및 교통사고(-2.95 vs. -24.35 p<0.01)로 인한 사망자수에서 통계적으로 유의한 감소를 보였다.
응급의학과 전문의제도가 도입되지 않는 나라들에 비해 도입된 나라들에서GDP (4,931±4,907 vs. 8,834±12,116 p=0.05), 인구수 (13.97x10⁶±32.58x10⁶ vs. 61.24x10⁶±176.27x10⁶ p=0.03), 1000명당 의사수 (1.31±1.48 vs. 1.81±1.42 p=0.02), 심혈관질환으로 인한 연령표준화사망률 (47.10±126.71 vs. 107.63±154.68 p=0.03), 외상으로 인한 연령표준화사망률(10.24±27.10 vs. 25.36±32.75 p=0.02)이 통계적으로 유의하게 높았다. 응급의학 전문과목이 도입되기 전에 비하여 도입된 후 5년간의 사망자수 변화 기울기가 감염성질환 (19.95 vs. -28.13 p<0.01) 및 교통사고 (18.27 vs. -27.16 p<0.01)로 인한 사망자수에서 통계적으로 유의한 감소를 보였다. 하위군 분석에서 외상으로 인한 사망자수 변화 기울기는 고소득 국가뿐만 아니라 중·저소득 국가에서도 유의하게 감소하였다.
본 연구에서는 응급의학회 또는 응급의학과 전문의제도가 도입된 나라들에서 응급의학회 또는 응급의학과 전문의 제도가 도입된 시기의 GDP, 인구수, 1,000명당 의사수, 심혈관 질환 및 외상으로 인한 사망률이 응급의학회 또는 응급의학과 전문의제도가 도입된 나라들에서의 최근 수치에 비하여 유의하게 높았다. 응급의학과의 발생시기에는 도시화, 산업화가 진행되면서 급속한 도시개발과 도시환경정비의 미비로 인하여 외상환자가 증가한 것으로 보인다. 더불어 예방접종과 항생제의 보급으로 인해 감염성질환은 감소하는 반면 비감염성질환은 증가하는 질병의 천이를 보였다. 외상이나 심혈관질환과 같은 응급질환의 증가는 응급의료의 필요성에 대한 정책적, 전문가적, 대중적인 수요를 촉발시키는 것으로 보인다. 응급의학회 및 응급의학과 전문의제도가 도입되기 전에 비하여 도입된 후 감염질환과 외상으로 인한 5년간의 사망자수 변화 기울기가 유의하게 감소하였다. 감염성질환으로 인한 사망자수는 세계적으로 감소 추세에 있으며 본 연구에서도 유사한 결과를 보였다. 반면, 외상으로 인한 사망자수는 세계적으로 증가추세에 있으며 특히 중 저소득 국가에서 이러한 경향이 두드러지나 본 연구에서는 응급의학회 및 응급의학과 전문의제도 도입 후 감소추세를 보였다. 한 연구에 따르면 59개 중 저소득 국가의 응급센터에서 전문적인 응급의학 교육을 받은 의사가 24시간 상주하는 곳은 4%에 불과하였다. 그러나 외상으로 인한 사망자수가 세계적으로 증가 추세에 있는 것을 고려 하였을 때 중 저소득 국가에서도 응급 환자의 분류, 초기 처치, 소생술 등 전문적인 응급의학 교육 프로그램의 도입은 보건의료에 중요한 역할을 할 것으로 기대된다.
본 연구는 각국의 응급의료체계 도입시기의 배경을 밝혀 응급의료 도입 당시의 수요와 응급의학 도입 전 후의 보건지표를 정량화 하였다. 이는 응급의료시스템의 도입시기에 있는 중·저소득 국가들에서 참고자료가 될 수 있을 것이다.
The emergency medical services (EMS) system is devoted to various time-sensitive diseases. It has not taken priority over primary prevention in low- and middle-income countries (LMICs) because of the belief that emergency care is innately expensive and profits relatively few patients when communicable diseases have been conventionally considered the major burden of disease. From the beginning of the 2000s, non-communicable diseases and injuries appeared to be the major burden of disease even in LMICs with rapid industrialization and urbanization. It is likely that 24/7 available clinical staff trained in emergency care could decrease the mortality rate of emergency medical conditions. Considering this disease transition, development in LMICs is in urgent need. However, there are few reports containing diverse medical, epidemiologic, and demographic information on the initial stage of implementation of emergency care in individual countries. Even though it is difficult to compare EMS systems and patient outcomes among countries due to their heterogeneities, it can be hypothesized that proper emergency care of acute illnesses and injuries might affect mortality.
The objectives of this study are as follows: (1) to describe the status of emergency medicine (EM) around the world with baseline demographic and epidemiologic information, (2) to compare individual countries at the time of initiation of either an EM society or EM specialty, and (3) to quantify the effect of establishing an EM society or adopting EM as a medical specialty on total number of deaths by cause.
For the literature review, PubMed, Cochrane Review, and the Bulletin of the World Health Organization (WHO) were searched. For quantitative evidence, 214 countries from the latest online estimates in the databases from the WHO and World Bank were used for assessing demographic and epidemiologic information. Webpages of societies, associations, and organizations active in areas relevant to EM were also screened. Demographic and epidemiologic information on the year of beginning were described of 58 countries with the year of EM society and 70 countries with that of EM as a specialty established or first recognized. First, a total of 214 countries were divided into two groups based on their EM society status. A t-test was performed to compare the differences in the demographic and epidemiologic information between countries with and without an EM society. A paired t-test was performed (p<0.05) for the statistical analysis to compare the differences in the demographic, epidemiologic, and health transition information between the periods before and after the adoption of an EM society. The gradient of the total number of deaths due to injury in the five-year period from four to eight years before and after the adoption of an EM society was used as a variable. Second, a total of 214 countries were divided into two groups based on their EM as a specialty status and then a t-test to compare the differences in the demographic and epidemiologic information between countries with and without an EM as a specialty and a paired t-test to compare the differences in the demographic, epidemiologic, and health transition information between the periods before and after the adoption of EM as a specialty was followed. The gradient of the total number of deaths due to injury in the five-year period from four to eight years before and after the adoption of an EM as a specialty was used as a variable.
Gross Domestic Product (GDP) (10,228±11,072 vs. 4,795±10,228 p=0.01), population (70.62x10⁶±191.86x10⁶ vs. 14.12x10⁶±32.89x10⁶ p=0.03), number of physicians per 1,000 capita (2.18±1.44 vs. 1.29±1.43 p<0.01), as well as age standardized death rate (ASDR) due to cardiovascular disease (134.97±142.50 vs. 61.62±128.54 p<0.01) and injury (34.49±34.63 vs. 13.59±27.17 p<0.01) at the time of EM society establishment in countries with EM societies were significantly higher than those of the last available in countries without EM societies. The gradients of the total number of deaths due to infectious diseases (145.61 vs. -145.26 p<0.01), injuries (-5.16 vs. -41.34 p<0.01), and road traffic accidents (-2.95 vs. -24.35 p<0.01) in the five-year period from four to eight years before and after the adoption of an EM society showed a statistically significant difference. GDP (8,834±12,116 vs. 4,931±4,907 p=0.05), population (61.24x10⁶±176.27x10⁶ vs. 13.97x10⁶±32.58x10⁶ p=0.03), number of physicians per 1,000 capita (1.81±1.42 vs. 1.31±1.48 p=0.02), as well as ASDR due to cardiovascular disease (107.63±154.68 vs. 47.10±126.71 p=0.03) and injury (25.36±32.75 vs. 10.24±27.10 p=0.02) at the time of EM being recognized as a specialty in countries with EM as a formal specialty were significantly higher than those of last available in countries without EM as a formal specialty. The gradients of the total number of deaths due to infectious diseases (19.95 vs. -28.13 p<0.01) and road traffic accidents (18.27 vs. -27.16 p<0.01) in the five-year period from four to eight years before and after the recognition of EM as a specialty were significantly different. The positive gradient of the total number of deaths due to injury in high-income countries (HICs) (58.37 vs. -53.94 p<0.01) became less steep, and the negative gradient of total number of deaths due to injury in LMICs (206.22 vs. -139.28 p<0.01) became steeper after establishing EM societies with statistical significance.
This study showed that GDP, population density, and number of physicians per 1,000 capita were higher in the countries that established EM societies or EM as a formal specialty than the latest values of non-established countries. At the time of the establishment of EM societies or EM as a formal specialty, the rate of ASDR due to injury and cardiovascular disease was higher than the latest values of non-established countries. In the total number of deaths due to injury, the degree of yearly decline increased significantly after establishing EM societies or EM as a formal specialty compared to the same slope for the period before the establishing year.
Considering that the effect of the global burden of disease on the total number of deaths due to injury has increased globally over the past 20 years, the implementation of EM in LMICs with rapid industrialization and urbanization might be an important part of the healthcare system. This study may help guide EM development as a reference point in LMICs where there is a lack of firm emergency medical care infrastructures but increased demand.
