小兒白癬症의 臨床 및 病原菌相 = Clinical and Mycological Observation on Dermatophytosis among Children

The clinical and mycological study was performed on dermatophytosis in children under the age of 18 years. The number of patients with dermatophytosis diagnosed on clinical findings and KOH examination at Chilgok Catholic Dermatological Clinic in Taegu city was 3,510 during past 6 years from January 1976 to December 1981.
Tinea pedis was the most common one (1,098 cases), followed by tinea cruris (886), tinea corporis(802), tinea capitis (431), tinea unguium (154) and tinea manus (139), in order of decreasing frequency. There was no remarkable annual change in the order of frequency between the clinical types during the observation period, but tinea capitis and tinea corporis showed an increasing tendency since 1978.
Of the 3,510 patients, males numbered 1,913 and females 1,597, giving male to female ratio 1.2:1. Males were infected predominantly by tinea cruris, while females by tinea pedis and tinea unguium.
The majority of cases (2,164) were found to be in the pubertal age (≥13 years). Number of patients belonging in preschool age (≤6 years) and school age (7∼12 years), were 885 and 461, respectively. Tinea capitis was noted predominantly in the group of preschool age, which decreasing rate of infection with advancing age, while tinea cruris, tinea pedis, tinea manus and tinea unguium predominantly in the group of puberty. Most types of dermatophytosis were found predominantly in the summer season when the climate is hot and humid, while tinea capitis was prevalent in the winter, autumn and spring.
From the 3,510 patients 2,265 organisms were isolated and indentified. The species isolated were, in order of decreasing frequency, Trichophyton rubrum (1,224 strains, 54% of total organisms isolated), Microsporum canis (433, 19.1%), T. mentagrophytes (316, 14.0%), Epidermophyton floccosum (176, 7.8%), T. interdigitale (53, 2.3%), T ferrugineum (45, 2.0%) and M. gypseum(18, 0.8%).
The annual distribution of most of the organisms including T. rubrum remained relatively constant, except M. canis showing an increasing tendency and T.ferrgineum a decreasing tendency in recent years.
Three main offenders of dermatophytosis constituting more than 87% of the total organisms and its seasonal distribution was as follows; M. canis was most frequently isolated in the autumn and winter, and T. mentagrophytes chiefly in winter and spring. However, total organisms had their peak prevalnce in the summer. It was because T. rubrum was the most common causative organism of dermatophytosis prevailed in hot and humid summer season.
