Influenza has been one of major viruses to cause respiratory diseases for centuries in the world. That has triggered a worldwide pandemic several times. The H1N1 flu raged throughout the world in 2009, and WHO declared the H1N1 flu pandemic in June. There appeared the H1N1 flu pandemic in our country as well, and the government raised the alert of public health to the caution level in July.
The purpose of this study was to examine the awareness of dental hygienists about the H1N1 flu and their performance of infection control in an effort to provide some information on the efficient prevention and management of the H1N1 flu among medical personnels, since dental hygienists are highly likely to be infected with respiratory diseases due to their close interval with patients during treatment.
The subjects in this study were dental hygienists who worked in general hospitals, dental hospitals, dental clinics and public dental clinics in Seoul and Gyeonggi Province. After 200 questionnaires were handed out for convenience' sake, the answer sheets from 191 respondents were analyzed except for some incomplete ones. A SPSS(Statistical Package for the Social Science) WIN 15.0 program was utilized to obtain statistical data on frequency, percentage, mean and standard deviation, and t-test, one-way ANOVA, x2(Chi-square) test and correlation analysis were conducted.
The findings of the study were as follows:
The dental hygienists who were better aware of the H1N1 flu performed more infection control(p<0.05).
They got a mean of 9.56 out of possible 14 points in the H1N1 flu awareness, which was not that a high score. The largest group gave a correct answer to a question whether the flu was mainly infected through the cough or sneeze of infected people, and the smallest group gave the right answer to a question whether the virus of the swine flu was influenza A(H1N1). In terms of awareness, the dental hygienists who were older, who received college or higher education, whose length of career was between six and 10 years and who had three or less colleagues at work were better aware of the flu.
As for the performance of infection control against the H1N1 flu, the dental hygienists got a mean of 38.54 out of possible 61 points, which indicated that they didn't perform infection control well. The largest group washed their hands after treatment(3.32), and the smallest group was vaccinated against the flu(0.38). Not many provided medical masks to patients with fever or respiratory symptoms(a runny nose, cough or soar throat)(2.21). The dental hygienists who worked in general hospitals, whose length of career was 11 years or more, who had 10 to 14 colleagues at work and who dealt with 16 to 25 patients on a daily basis were the best performers of infection control.
Concerning the route of information acquisition about the flu, the greatest group acquired that information through the Internet and massmedia(87.4%). They obtained the most information on personal hygiene(32.1%), followed by the state of the prevalence of the flu(26.2%), treatment methods(24.8%) and problems with the outbreak of the flu (16.4%). Thus, the information that the dental hygienists acquired was most related to personal hygiene. Regarding concern for the possible infection of the flu, those whose length of career was between six and 10 years and who worked in general hospitals were most apprehensive of the possible infection of the flu during treatment, and the dental hygienists who received college or higher education, who worked in general hospitals and whose length of career was longer believed that their hospitals took successful actions to prevent the flu. Being vaccinated against the flu(32.6%) was considered the best way to cope with and prevent the flu, followed by a supply of detailed guidelines(26.7%) and aid for education(13.6%).
The findings of the study made it clear that the H1N1 flu awareness of the dental hygienists and their infection control performance weren't at a high level. Influenza, which is a variation of an antigen, has been widespread every year, and there has been an influenza pandemic every 10 to 40 years. Therefore the development of systematic training, hospital manuals and effective education programs is required to improve the H1N1 flu knowledge and infection control of dental hygienists who work in medical institutions.
인플루엔자는 세계 여러 지역에서 수세기동안 호흡기 질환의 주요한 원인 바이러스로 여러 차례 대유행을 일으켜 왔다. 2009년 대유행을 일으킨 신종인플루엔자A(H1N1)바이러스는 세계보건기구(WHO)가 6월 대유행 상태로 선언하고 국내에서도 7월 공중보건 위기단계를 경계 단계로 격상시키는 등 대유행이 매우 빠르게 진행되었다.
이에 본 연구는 시술 시 환자와 술자 사이의 거리가 가까워 호흡기 감염에 전염성이 높은 치과위생사를 대상으로 신종인플루엔자에 대한 인식도 및 감염관리 실천도를 파악하여 의료인에서 신종인플루엔자A(H1N1)를 효율적으로 예방하고 관리하기 위한 기초 자료를 제공하고자 한다.
서울, 경기 지역의 종합병원, 치과병원, 치과의원, 보건소에 근무하는 치과위생사를 대상으로, 2010년 4월 19일부터 5월 14일까지 연구자 편의에 따라 총 200부의 설문지를 배부하여 불충분한 설문 응답지를 제외하고 총 191부를 최종 분석에 이용하였다. 수집된 자료는 SPSS(Statistical Package for the Social Science) WIN 15.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test(검증)와 One-way ANOVA(일원변량분석), χ2(Chi-square) 검증Correlation(상관관계분석)을 실시하였다.
구체적인 연구결과는 다음과 같다.
치과위생사에서 신종인플루엔자에 대한 인식도가 높을수록 실천도가 높은 것으로 나타났다(p<0.05).
치과위생사의 신종인플루엔자에 대한 인식도 수준은 14점 만점 중 전체 평균 9.56으로, 그리 높지 않은 것으로 나타났는데, '신종인플루엔자는 주로 감염된 사람의 기침이나 재채기를 통해 감염 된다'라는 전파기전에 대한 문항의 정답률 94.8%로 가장 높았으며, '신종인플루엔자 바이러스는 A(H1N1)이다'라는 정의 문항이 36.6%로 정답률이 가장 낮았다. 인식도는 연령이 많은 치과위생사 일수록, 대졸 이상, 근무경력이 6~10년, 근무처의 치과위생사의 수가 3명 이하인 치과위생사가 신종인플루엔자에 대한 인식도가 높았다.
치과위생사의 신종인플루엔자에 대한 실천도의 수준은 61점 만점 중 전체 평균 38.54로 실천도 또한 높지 않게 나타났고, '진료 행위를 한 후에 손을 씻는다'가 3.32로 가장 높았고, '신종인플루엔자에 대한 예방접종을 실시했다'가 평균 0.38로 가장 낮았다. 또한 '발열 및 호흡기 증상 (콧물, 기침, 인후통 등)을 가진 환자는 의료용 마스크 등을 제공 한다'가 2,21로 역시 낮았다. 종합병원에 근무하는 치과위생사와 근무경력이 11년 이상이며, 치과위생사가 10~14명이 근무하고 하루 평균 환자 수가 16~25명인 곳이 실천도가 가장 높았다.
신종인플루엔자에 대한 정보 습득 경로는 인터넷 및 대중매체가 87.4%로 가장 높았으며, 개인위생에 대한 정보는 32,1%, 신종인플루엔자 발생현황은 26.2%, 치료방법 24.8%, 신종인플루엔자 발생에 따른 문제점 16.4%로 치과위생사들은 신종인플루엔자에 개인위생에 대한 정보를 가장 많이 얻었음을 알 수 있다. 신종인플루엔자에 대유행으로 감염 가능성의 우려를 조사한 결과 근무경력이 6~10년, 종합병원에 근무하는 치과위생사가 신종인플루엔자 대유행으로 환자 진료로 인한 감염 가능성에 대해 우려하고 있었고, 대졸이상, 종합병원에 근무, 근무경력이 많은 치과위생사일수록 병원이 자체적으로 신종인플루엔자 예방을 위한 대비책이 잘 수립되어 있다고 인식하였다. 신종인플루엔자의 대응 및 대비를 위해 신종인플루엔자의 예방접종 32.6%, 세부지침 제시 26.7%, 교육지원 13.6%가 필요하다고 하였다.
본 연구를 통해 치과위생사의 신종인플루엔자에 대한 인식도와 실천도가 높지 않을 것을 확인할 수 있었다. 인플루엔자는 빈번한 항원변이로 매년 유행하며, 10년~40년 주기로 대유행을 일으키므로 의료기관에 근무하는 치과위생사들이 신종인플루엔자에 대한 지식을 정확히 알고 이에 따른 감염관리 실천도를 높일 수 있도록 체계적인 훈련과정과 병원 매뉴얼 관리 및 효과적인 교육프로그램 개발이 필요하다고 생각한다.
