비골 골절의 비관혈적 치료 결과에 대한 분석 = THE POSTOPERATIVE ANALYSIS OF CLOSED REDUCTED NASAL BONE FRACTURE

The nose is the most prominent feature of the face and as such is the most likely to be traumatized. Any slight displacement of the bones or cartilages is easily seen, and so a high degree of accuracy is required in the reduction of nasal fracture. But as a consequence, critical reviews of the long-term results of the treatment of these injuries have indicated that unsatisfactory cosmetic and functional problems are all too common. In May 1995, a prospective study was set up to attempt to determine the correlation between the etiological factors involved in nasal trauma and satisfaction of doctors and patients. In the 28 month we analysed 64 cases. We assessed the result of treatment at least 6 months later. Results were as follows.
1. In cases with septal fracture, the rate of doctor's satisfaction was significantly low(p=0.000) and in cases with severe septal deviation, the rate of doctor's satisfaction was significantly low(p=0.0010).
2. In cases with septal fracture under the same degree of septal deviation, the rate of doctor's satisfaction was significant low(p=0.004) but in cases with severe septal deviation under no septal fracture group and septal fracture group, the rate of doctor's satisfaction was not significant low (p=0.113)
3. In cases of severe deviated nose, the rate of septal deviation severity and the incidence or septal fracture were significantly high respectively(p=0.013, p=0.023).
4. After 6 months, doctor's dissatisfaction was caused by undercorrection
[87.9%(29/33)] and hump like projection [12.1%(4/33)]. Patient's dissatisfaction was caused by hump like projection [50%(4/8)], undercorrection [25%(2/8)], and nasal obstruction [(25%(2/8)].
The only significant factor is septal fracture in determining the success rate of the operation in 64 cases who had been treated with closed reduction. In view of high dissatisfied rate, alternative method of treatment is suggested for patients with septal facture or with severe nasal deviation or with severe septal devation.
