성인 비만여성을 위한 저열량 식단 개발 및 효과에 관한 연구 = (A)study on low calorie menu development and effects for obese adults women
Abstract
The number of the obesity person increases all over the world as well as in Korea becoming an obesity society. The obese adults women have to maintain his health. Part Ⅰ analyzed the factor which survey of popular dieting regimens and eating habits of women living. Based on the results of Part Ⅰ, Part Ⅱ analyzed the factor which effected the development menu on the dieting programs of obese adults women which control most popular diet regimens.
Part Ⅰ. This survey was conducted from June 2002 to August 2002, through questionnaires given to 432 women of various ages ranging from 20 to 50, who were living in the Seoul metropolitan area. The questionnaires were designed to determine the physique, level of obesity, knowledgeability of dieting, dieting side effects, and eating habits of these women, as well as whether they were currently on a diet or not, and to uncover what “fad diets” were in fashion.
The results were as follows:
1. The age distribution of the respondents was as follows: 20s, 25.8%, 30s, 26.5%, 40s, 24.2% and 50s, 23.5%. There was a high proportion of high school graduates, 32.6%.
2. The average height among the respondents was 160.45 cm, weight 56.01 kg, desired weight 51.87 kg, and BMI, 21. 92 kg/m2. Anthromorphic variables showed significant differences among the different age groups. (p<0.05)
3. Reasons for dieting included the following: 52.3% for health reasons, 15.0% because of impaired mobility due to the excess weight, and 32.6% for aesthetic reasons. The most effective method of Dieting, according to the respondents, was exercise, dietary regimens, Alternative methods responses, In regards to the amount of exercise done for dieting purposes, 46.5% said none, 23.5% said 1~2 times a week respondents, Dieting information was found to be obtained from family or friends (31.7%), or newspapers, magazines (29.3%), Reasons for excess weight included: bad eating habits (39.6%), lack of exercise (38.3%), pregnancy and childbirth (10.6%).
4. Most popular dietary regimens, organic foods, Among exercise regimens, aerobic exercises, Among surgery and other special therapies, sauna, steam baths and high temperature half-immersion baths.
5. General knowledge on dieting among the respondents scored, on average, 9.02 points out of 10. Respondents scored highest on the correlation between obesity and disease (97.7%) and lowest on the correlation between eating speed and obesity (83.3%)
6. Side effects from dieting included dizziness and anemia (16.2%) fatigue (15.1%) indigestion/constipation (11.1%) physical weakness (10.5%), loss of concentration (7.8%), dry skin (7.4%) loss of motivation (6.7%).
7. Respondents scored highest on vegetable consumption (including kimchi). Average scores varied significantly among age groups, with those in their 20s scoring lowest on multi-grain rice consumption, those in their 30s scoring lowest on seaweed consumption and those in their 40s and 50s scoring lowest on milk and dairy product consumption.
Part Ⅱ. The SM (Standard menu, n=26) diet group was subject to nutrition education, behavior modification and an exercise regimen, with a standard menu diet. The DM (Development menu, n=26) diet group was subject to nutrition education, behavior modification, an exercise regimen, and a conventional reduced-calorie diet with the addition of organic vegetable, cooked rice with brown rice, green tea, seaweed. A 6 months dieting program was conducted, and the changes in the two groups, before and after the program, diet habit,
nutrient intakes, energy expenditure, anthropometric, biochemocal were observed.
The results of this study can be summarized as follows.
1. The average age of the patients was 44.7±7.1 years, average weight, 71.76±1.60 kg, percentage of ideal body weight (PIBW), 117.89±3.14, and body mass index (BMI), 27.75±0.66.
2. According to a survey on eating habits, improvement was shown on 17 of the 18 of the statements, and a significant difference between the two groups was not found. (p<0.05)
3. In the rate of calories burned through physical activity showing no significant difference.
The SM group, compared to the reduced calorie intake, showed a decrease in vitamin and %RDA, In contrast, the DM group, compared to the reduced calorie intake, showed a marked increased in vitamin and %RDA of micronutrients, and showed a satisfactory nutritional intake in accordance with the recommended dietary allowance for Koreans.
4. Weight, body fat, body fat percentage, BMI, waist circumference, hip circumference, WHR, triceps, subscapular the DM group showed a more noticeable difference than the SM group.
5. Biochemical measurements including the TG, TC, LDL-cholesterol decreased significantly in the DM group as well, but showed a marked increase in the “good” HDL-cholesterol. There was no difference in glucose levels for both groups. The DM group showed a more significant change than the SM group.
6. According to a survey, 77.3% of the SM group patients, and 90.9% of the DM group patients replied that the dieting program conducted in this study was “effective”. The DM group in particular had more patients who deemed the diet regimen effective than the SM group.
서울지역에 거주하는 20대부터 50대 이상에 이르는 각 연령층의 여성 432명을 대상으로 설문지를 이용하여 2002년 6월부터 8월에 걸쳐 체격 및 비만도, 다이어트 실태조사, 유행하는 다이어트 방법들, 다이어트에 관한 지식, 다이어트 후 경험한 부작용, 식습관 등을 조사하여 다음과 같은 결과를 얻었다.
1. 조사대상자의 연령 분포는 20대 25.8%, 30대 26.5%, 40대 24.2%, 50대 이상 23.5% 이었으며 학력은 고졸이 32.6%로 가장 높았다.
2. 조사대상자의 키는 160.45 cm, 체중 56.01 kg, 희망체중 51.87 kg, BMI 21.92 kg/m2 으로 나타나 조사대상자의 연령에 의해 유의한 차이 (p<0.05) 를 나타내었다.
3. 다이어트의 목적은 건강을 위해 52.3%, 몸이 무거워 거동이 불편해서 15.0%, 아름다워 보이기 위해서가 32.6% 이었으며, 가장 효과적이었던 다이 어트 방법으로는 운동요법, 식사요법, 기타요법 순서이었으며, 다이어트를 위한 운동 횟수에 있어서 46.5%가 전혀 하지 않는다, 23.5%가 주 1~2회 운동을 한다고 답하였으며, 다이어트에 관한 지식은 가족, 친구 31.7%, 신문, 잡지 29.3% 이었으며, 군살의 원인은 식습관 (39.6%), 운동부족 (38.3%), 임 신, 출산 (10.6%) 순으로 나타났다.
4. 가장 선호하는 다이어트 방법은 식이 다이어트에서는 유기농 식품, 운동 다 이어트로는 유산소 운동, 외과수술 & 스페셜 다이어트로는 사우나, 찜질방, 고온 반신욕 다이어트 방법임을 알 수 있었다.
5. 다이어트에 관한 일반지식의 평균은 10점 만점에 9.02점 이었으며 성인병과 비만과의 관계가 가장 높은 정답율 (97.7%)을, 식사속도와 비만과의 관계가 가장 낮은 정답율 (83.3%)을 보였다.
6. 체중조절 중의 부작용은 어지러움증.빈혈 16.2%, 피로 15.1%, 소화불량.변비 11.1%, 전신무력감이 10.5%, 집중력 감소 7.8%, 피부건조가 7.4%, 의욕상실 6.7% 순으로 나타났다.
8. 본 대상자들은 대부분 우유 및 유제품 섭취는 매일 하지 않으며, 김치 외에 채소류 섭취는 매일 먹는 것으로 조사되었다. 평균 점수는 연령에 따라 유 의적인 차이 (p<0.05)를 보여 20대는 잡곡밥 섭취를 하지 않으며, 30대는 해 조류 섭취 부족, 40대, 50대는 우유 및 유제품 섭취가 부족하였다.
개발식단의 체중감량효과를 알아보기 위해 SM (표준식단 다이어트, n=26) 그룹은 영양교육, 행동수정, 운동요법을 기본으로 하고 식이요법으로는 표준식단을 섭취하고 DM (개발식단, n=26) 그룹은 영양교육, 행동수정, 운동요법을 기본으로 하고 식이요법으로는 표준식단에 개발식단을 포함하여 섭취하는 그룹으로 나누어 6개월 동안 체중감량 프로그램을 실시하여 체중감량 프로그램 전후 두 그룹간의 식생활태도, 영양소 섭취량, 활동량, 체성분, 생화학적 분석 등의 변화를 관찰하였다.
본 연구의 결과를 요약하면 다음과 같다.
1. 연구대상자의 평균 연령은 44.7±7.1세였고 평균 체중, 비만도(PIBW) 및 체 질량지수(BMI, kg/m2)는 각각 71.76±1.60, 117.89±3.14, 27.75±0.66이었다.
2. 식생활 태도 조사결과 18문항 중 17문항의 개선을 보여 본 프로그램을 통 해 식생활태도가 개선되었으며 두 그룹간의 유의한 차이가 없는 것으로 나 타났다 (p<0.05).
3. 활동에 의한 열량 소비량 변화를 보면 두 그룹 모두 증가하였고 두 그룹간 의 유의한 차이가 없는 것으로 나타났다.
1일 평균 영양소 섭취량의 변화를 보면 SM 그룹은 칼로리가 감소함에 따라 비타민 무기질의 %RDA 값이 유의하게 감소하였고 DM 그룹은 유의하게 증가 하였다.
4. 체중, 체지방, 체지방율, BMI, 허리둘레, 엉덩이둘레, WHR, 삼두박근, 견갑 하골은 두 그룹간의 비교에서 DM 그룹이 SM 그룹보다 유의하게 (p<0.05) 감소하였다.
5. 혈액 중의 생화학적인 인자들의 변화를 보면 두 그룹 모두 TC와 TG, LDL-cholesterol이 각각 유의하게 감소하였고 두 그룹간의 비교에서 DM 그룹이 SM 그룹보다 유의하게 (p<0.05) 감소 하였다. SM 그룹은 HDL-cholesterol은 변화가 없었고 DM 그룹은 HDL-cholesterol 이 유의하 게 증가하였다. Glucose는 두 그룹 모두 변화가 없었다.
6. 추적조사 결과 SM 그룹은 대상자의 77.3%, DM 그룹은 90.9%가 본 프로그 램이 체중조절에 “효과가 있었다”라고 답하였고 특히 DM 그룹은 식사요법 에 효과적이었다고 답한 연구대상자들이 SM 그룹보다 유의적으로 많았다.
