노인 돌봄의 특징과 현실, 정책의 전환: 지역공동체 디지털 돌봄을 위하여 = A Special Features and Reality of Elderly Caring, Policy Shift:For Digital Care Model at Local Area

국문 초록 (Abstract)

이 논문의 목적은 노인 돌봄의 구조적 특징과 현실을 파악하여 이에 대한 정책적 검토와 대안을 모색하는 데에 있다. 한국은 2050년 인구의 25%가 고령 인구가 되는 초고령화 사회로 진입할 것이며, 디지털 헬스의 도입과 확대가 이루어질 것이다. 이에 높은 노인 사망률과 노인 부양 문제와 직결되는 ‘노인 돌봄’에 대하여 근본적인 성찰이 필요하다.
노인 돌봄은 장기요양 보험 제도를 기반으로 가족 돌봄과 시설 돌봄 형태가 있다. 전자는 장자 돌봄을 원칙하에 비용과 돌봄에 대한 부담이 가족관계의 갈등 요인이 되더라도 가족이 돌보는 것이고, 후자는 가족 내에서 돌봐줄 사람이 없거나 몸 상태가 일상생활을 하기 어려운 경우에 재정 상태에 따라 요양병원이나 요양원에서 돌보는 것이다. 그런데 이러한 노인 돌봄은 몇 가지 구조적 문제를 안고 있다. 첫째 가족 돌봄을 받는 노인에게 심리적 부담을 주고 둘째, 장기간 시설에서 돌봄 생활을 하는 노인의 경우 우울감을 높이고, 셋째 건강보험재정 악화를 초래하는 것이다. 이런 문제에 대한 대안으로 ‘커뮤니테 케어’를 고려했으나, 기대했던 효과를 거두지 못했다. 이유는 커뮤니티 개념과 영역에 대한 혼선과 모호성에 기인한다. 이에 본 논문에서는 노인 돌봄의 새로운 지역공동체 디지털 돌봄 모델이 필요함을 제안하였다. 이 모델은 공동체 정서를 기반으로 한 특정 지역을 중심으로 돌봄의 주체를 분명하게 하고, 디지털 기술을 활용한 디지털 헬스의 스마트 돌봄으로, 청년과 여성, 건강한 노인이 함께 하여 Ai 돌봄 일자리 체계를 통해 돌봄을 실행함으로써 노인 돌봄이 세대통합적인 효과도 가져올 것이다.

다국어 초록 (Multilingual Abstract)

The purpose of this paper is to grope a alternative policy for new elderly caring model after that is understand and diagnosing a structural features and reality of elderly caring. Korea society is going to be supper aging society which 25% of populations of that will be aging population. And it will come digital health introduction and diffusion of it. So it is to be needed that a basic and radical reflections aginst elderly caring in relations to higher death rate of old man and elderly care problems directly connected.
Elderly caring consist of family caring and facilities caring based on long-term care insurance system. the former is family are caring their parents even though the cost and burden of it can be conflict factors within family relations with the principle of care of eldest son. On the other hand the latter is to care at nursing hospital and nursing home depending on their financial situation in case of that they no one to take care of family or their body conditions are inconvenient for everyday life.
Practically elderly care have a structural problems such as. first it will be provide a psychological burden to elderly people receiving family care. the second it will be increase depression to elderly people whose are living at a caring facility in a long time. the thirdly it will be deteriorates health insurance finances.
So it was raised community care. as a alternative that this problem is to be resolve it as much as possible. But it was revealed that community care did not achieved effects that has been expected. The reasons are the conceptual confusion of community care and ambiguity of domains of it.
Therefore this paper is suggested that policy review and policy shift about elderly care which should be to consider changed social environment and to enforce ethics and publicity and accountability of caring newly is needed. And it suggested that new community digital care model has to be developed. New community digital care model for elderly care that is focus on a special area within community emotion should be make clear the subject of caring and is to be smart caring that is to digital health should be use digital technology. Also this model that is to be participate with youth and woman and work together for old man. So this model will bring to generational care effect by implementing caring though ai caring jobs.
