정신질환자는 사회의 한 구성원과 인간으로서 누려야 할 권리가 있지만 정신과적 증상으로 인해 사회적으로 고립되어 있고 정신질환에 대한 잘못된 인식이나 태도 때문에 삶의 질이 저하되어 있다.
1995년 12월에 정신보건법이 제정되면서 우리나라에서도 점차적으로 정신 보건 분야에 대한 인식이 변화되고 있으나 아직도 부정적인 견해는 사회 전반에 편만 되어있어 정신질환자의 예방, 치료, 재활 모든 측면에 영향을 끼치고 있다.
본 연구는 정신과 간호사, 비정신과 간호사간의 정신질환 및 정신질환자에 대한 태도를 Taylor와 Dear가 개발한 Community Attitudes Toward the Mentally Ill(CAMI) scale을 이용하여 비교 분석하였다. CAMI의 4가지 하위척도는 권위주의, 자비심, 사회생활제한, 지역정신보건개념으로서 각 척도별로 10문항씩 총 40문항으로 구성되어 있다.
간호사는 가장 가까이에서 정신질환자들을 간호하고 있기 때문에 정신질환자들의 적극적인 치료와 효율적인 사회 복귀에 지대한 영향력을 미친다. 또한 간호사의 정신질환 및 정신질환자에 대한 긍정적인 태도는 환자 자신의 권리를 주장하고 삶의 질을 향상시키므로 중요한 의미를 갖는다.
비정신과 간호사는 간호대학 졸업 후 정신 간호 제공의 기회가 거의 없고 정신간호에 대한 지식이 부족하기 때문에 실제 정신질환자를 간호하고 있는 정신과 간호사와 전문가로서 한 직종이지만 차이를 보인다. 이에 정신과 간호사, 비정신과 간호사의 정신질환에 대한 태도를 파악하여 비교함으로써 궁극적으로 정신질환자의 삶의 질을 향상시키는 간호를 제공하기 위한 기초 자료로 삼고자 이 연구를 시행하였다.
본 연구를 위한 설문 조사는 2004년 4월 13일부터 2004년 5월 5일까지 시행하였으며, 서울과 경기도에 소재하는 대학병원 3곳, 종합병원 1곳, 정신병원 1곳의 총 5개소에서 근무하는 정신과 간호사 125명, 비정신과 간호사 128명으로 총 253명이 대상이었다.
조사된 자료는 SPSS programe을 사용하여 빈도, 백분율, t-test, One-way ANOVA를 이용하여 분석하였다.
본 연구의 조사 결과를 요약하면 다음과 같다.
1. 정신과 간호사, 비정신과 간호사간의 정신질환 및 정신질환자에 대한 전반적인 태도는 권위주의(t=.4817, p=.00), 사회생활제한(t=6.750, p=.00), 지역정신보건개념(t=-7.012, p=.00)에서 유의했다. 즉, 정신과 간호사가 비정신과 간호사보다 덜 권위적이고, 사회생활 측면에서 덜 제한적이었으며 지역정신보건개념도 더 긍정적인 것으로 나타났다.
2. 연령별 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 높은 연령층에서 덜 온정적(F=4.795, p.001)이고, 지역정신보건개념은 더 긍정적(F=2.586, P=040)이었다. 이에 비해 비정신과 간호사는 높은 연령층이 사회생활 측면에서 덜 제한적(F=3.368, p=.021)이었고, 지역정신 보건개념도 더 긍정적(F=5.166, p=.002)인 것으로 나타났다.
교육 배경에 따르는 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 대학원 재학 중이거나 대학원 졸업 간호사들이 사회생활 측면에서 덜 제한적(F=4.891, p=.003)이고, 지역정신보건개념이 더 긍정적 (F=2.890, p=.038)이었으나 비정신과 간호사의 경우 교육 배경에 따라서 차이가 없었다.
임상경력의 경우 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 임상경력이 길수록 덜 온정적(F=3.028, p=.013)이었고, 비정신과 간호사는 1년 미만의 신규간호사와 10년 이상의 경력 간호사가 사회생활 측면에서 덜 제한적인(F=2.479, p=.034) 것으로 나타났다.
자녀 여부의 경우 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 자녀가 있을 때 지역정신보건개념(t=126, p=.048)이 더긍정적이 었으나 비정신과 간호사는 별 차이가 없었다.
정신과 근무 희망 여부의 경우 비정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 정신과 근무를 희망하였을 때 덜 권위적(t=2.730, p=0.07)이었으나 정신과 간호사들에서는 별 차이가 없었다.
근무지에 따르는 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 권위주의(F=3.828, p=.024), 자비심(F=3.540, p=.032), 사회생활제한(F=3.613, p=.030)에서 유의하여 대학병원과 종합병원에 근무하는 간호사보다 정신병원에 근무하는 간호사가 더 권위적이고, 덜 온정적이 었으며 사회생활 측면에서도 더 제한적인 것으로 나타났다.
정신보건간호사과정 수료 여부에 따르는 정신과 간호사의 정신질환 및 정신질환자에 대한 태도는 수료하였을 때 지역정신보건개념이 더 긍정적(t=-1.604, p=.020)인 것으로 나타났다.
위와 같은 연구 결과를 종합하여 볼 때 정신과 간호사가 비정신과 간호사보다 정신질환 및 정신질환자에 대한 태도에서 덜 권위적이고 덜 사회생활 제한적이며 지역정신보건개념에 대해 더 긍정적이었다.
정신과 간호사의 경우 학력이 높을 때 덜 사회 제한적이며, 자녀가 있고 정신보건간호사과정을 수료한 간호사가 지역정신보건개념이 더 긍정적이었다. 그러나 연령이 높고 임상경력이 길수록 덜 온정적이었으며 대학병원과 종합병원보다 정신병원에서 근무하는 간호사가 더 권위적이었다. 따라서 정신병원에 근무하는 정신과 간호사들에게 정신질환 및 정신질환자에 대한 주기적인 교육을 실시함으로써 바람직한 태도의 변화를 유도해야 될 것이다.
비정신과 간호사의 경우 높은 연령층이 덜 사회 제한적이었고, 더 긍정적인 지역정신보건개념을 가지고 있었다. 또한 정신과 근무를 희망한 비정신과 간호사가 덜 권위적인 것으로 나타났다. 따라서 비정신과 간호사 역시 병원 내 교육 및 간호사 보수교육 프로그램과 홍보를 통해 정신질환 및 정신질환자에 대해 올바르게 이해하고 긍정적인 태도를 가지도록 노력해야한다.
본 연구의 결과를 근거로 하여 다음과 같이 제언하고자 한다.
첫째, 전반적인 간호사의 정신질환 및 정신질환자에 대한 태도를 파악하기 위해서 확대된 조사연구가 필요하다.
둘째, 정신과 간호사의 정신질환 및 정신질환자에 대한 올바른 이해와 긍정적인 태도로 변화하기 위해서 지속적인 정신질환 및 정신질환자에 대한 태도 평가와 주기적인 재교육이 필요하다.
셋째, 비정신과 간호사의 정신질환 및 정신질환자에 대한 올바른 이해와 긍정적인 태도를 갖기 위해 병원 내 교육 및 간호사 보수교육 프로그램과 홍보가 필요하다.
Even though mentally ill people have human rights to live as social members in society, they are alienated from society on account of psychiatric symptoms and are in bad quality of life because people misunderstand the illness. In December 1995, the mental health act was enacted in our country the recognition on the mental health has been changing, but still the negative viewpoints are around the nation. And they affect the prevention, treatment, rehabilitation, and every aspect of the mentally ill people.
Community Attitudes Toward the Mentally Ill(CAMI) scale developed by Taylor & Dear was used to compare and analyze psychiatric nurses and non-psychiatric nurses' attitudes toward mental illness and the mentally ill. CAMI consists of 4 sub-scales which are Authoritarianism, Benevolence, Social restrictiveness, Community mental health ideology. Each scale has 10 questions and It is tally 40 questions.
As nurses are the very people who take care of the ill people, they are deeply involved in the treatment of them and their returning to society. And also nurses' positive attitudes toward mental illness and the mentally ill are very important because they help the people claim their rights and improve the quality of life. As Non-psychiatric nurses scarcely have the opportunities to serve the mentally ill people after graduating from nursing college and don't have the knowledge on them, actually there must be big differences. So I would like to perform this study to provide the basic data for improving the quality of life for the mentally ill people by grasping psychiatric nurses and non-psychiatric nurses' attitudes toward mental illness and the mentally ill and comparing them.
The survey was conducted during April 13, 2004 through May 5, 2004. The subjects were 253 nurses(128 psychiatric nurses and 125 non-psychiatric nurses) who were randomly selected from 3 university hospitals, 1 general hospital, 1 psychiatric hospital located in Seoul or Kyung-gi province. The data was analyzed by SPSS statistics program with frequency, percentage, t-test and One-way ANOVA.
The result of this study was as follows.
1. General attitudes of psychiatric nurses and non-psychiatric nurses toward mental illness and the mentally ill were deeply related to Authoritarianism(t=.4817, p=.00), Social restrictiveness (t=6.750, p=.00), Community mental health ideology(t=-7.012, p=.00) So to speak, psychiatric nurses were relatively less authoritative, less social-restrictive and more positive in community mental health ideology than non-psychiatric nurses.
2. As for attitudes toward mental illness and the mentally ill by age, older psychiatric nurses were less benevolent(F=4.795, p.001) than younger psychiatric nurses, but more positive in Community mental health ideology(F=2.586, P=040). Older non-psychiatric nurses were less social-restrictive(F=3.368, p=.021) than younger non-psychiatric nurses and more positive in Community mental health ideology(F=5.166, p=.002).
As for attitudes toward mental illness and the mentally ill by education backgrounds, higher-educated psychiatric nurses were less social-restrictive(F=4.891, p=.003) than lower-educated psychiatric nurses and more positive in Community mental health ideology(F=2.890, p=.038). Non-psychiatric nurses had no differences by education backgrounds.
As for attitudes toward mental illness and the mentally ill by clinical career, psychiatric nurses who had longer clinical career were less benevolent(F=3.028, p=.013) than ones who had short clinical career. Non-psychiatric nurses who had less than 1 year clinical career and who had more than 10 year-clinical career were less social-restrictive(F=2.479, p=.034).
As for attitudes toward mental illness and the mentally ill by having children or not, psychiatric nurses who had children were more positive in Community mental health ideology(t=126, p=.048) than psychiatric nurses who didn't have children. Non-psychiatric nurses had no differences by having children or not.
As for attitudes toward mental illness and the mentally ill by requesting working in the department of psychiatry or not, non-psychiatric nurses who wanted to work in the department of psychiatry were less authoritative(t=2.730, p=0.07) than non-psychiatric nurses who didn't want to. Psychiatric nurses had no differences by requesting working in the department of psychiatry or not.
As for attitudes toward mental illness and the mentally ill by the working places, nurses working in the psychiatric hospital were more authoritative(F=3.828, p=.024), less benevolent(F=3.540, p=.032), and more social-restrictive(F=3.613, p=.030) than the nurses working in the university hospitals.
As for attitudes toward mental illness and the mentally ill by finishing the training courses of the Psychiatric mental health nurse or not, nurses who finished the courses were more positive in Community mental health ideology(t=-1.604, p=.020) than the nurses who did not.
In conclusion, psychiatric nurses were less authoritative, less social-restrictive, and more positive in Community mental health ideology than non-psychiatric nurses respecting attitudes toward mental illness and the mentally ill. Higher educated psychiatric nurses were less social-restrictive Nurses who finished the training courses of the Psychiatric mental health nurse were more positive in Community mental health ideology. But older and longer clinical career nurses were less benevolent. Nurses working in the psychiatric hospital were more authoritative than the other hospitals. Therefore it is desirable to lead the psychiatric nurses to have better attitudes by training them periodically with regards to attitudes toward mental illness and the mentally ill. Older non-psychiatric nurses were less social-restrictive and more positive in Community mental health ideology. Non-psychiatric nurses who wanted to work in the department of psychiatry were less authoritative. Consequently non-psychiatric nurses as well as psychiatric nurses should try to apprehend mental illness and the mentally ill well and to have the positive attitudes through education in the hospitals, service education programs for nurses, and the PRs.
With the result of this study, I recommend the following.
1. It is necessary to study extended research for catching general attitudes of nurses toward mental illness and the mentally ill.
2. It is suggested that it is important to check the attitudes and to retrain the nurses periodically in order to have psychiatric nurses understand mental illness and the mentally ill well.
3. There should be nurses' education in the hospitals, service education programs for nurses, and PRs to help non-psychiatric nurses understand mental illness and the mentally ill well and have the positive attitudes.
