말초 신경장애의 원인 및 전기 생리학적 소견에 관한 연구 = Study on the etiology and electrophysiological findings in patients with peripheral neuropathy

Recently the incidence of peripheral neuropathy has been shown increasing tendency with the increment of systemic diseases and the improvement of electrophysiologic devices that could be accompanied with peripheral neuropathy. Early diagnosis and treatment according to the underlying causes are so important with respect to the prognosis for patients with peripheral neuropathy. Therefore, we analyzed patients with peripheral neuropathy according to the etiology, the clinical features, and the characteristics of electrophysiologic studies. This study was carried out on 172 patients who visited the Department of Neurology in Chungnam National University Hospital with symptoms of peripheral neuropathy from 1991 to 1996. The results and conclusions were as follows: 1) Men were more susceptible than women to the peripheral neuropathy and aging was the important risk factor for causing neuropathy; 2) The most common cause was the immune-mediated peripheral neuropathy like Guillain-Barre´ syndrome and chronic inflammatory demyelinating polyneuropathy and the others were included diabetes mellitus, hepatic and renal diseases, chronic alcoholism, vitamin deficiency, and toxic exposure; 3) Alcohol-induced and metabolic derangement such as chronic liver disease or renal disease caused axonopathy and the immune-mediated neuropathy was classified with demyelinating neuropathy on the basis of electrophysiologic study; 4) The strict control of diabetes mellitus was the most determining factor for the preservation of normal electrophysiologic state in patients with diabetes but there was no relationship between the duration of diabetes and the abnormality of nerve conduction study; 5) The peripheral neuropathy complicated with diabetes had negative impact on axon and myelin independently or simultaneously.
