일반적으로 성병은 성교로 인해 전염되는 병으로 임질, 매독 연성하감 등이 가장 흔하게 걸리는 질병이었다. 성병이 사회문제로 대두되기 시작한 것은 일제시대로 당시 결핵과 소화기병과 함께 조선인을 괴롭힌 대표적인 질병이었다.
일제시대에는 성병이라는 용어보다는 화류병이라는 용어가 더 많이 사용되었는데, 이는 주로 불순한 성교 즉 화류계에 출입하는 자나 화류계에 종사하는 여성들에 의해 전염되는 부정한 행위의 결과로 여겨졌기 때문이다.
성병은 당사자 개인에게는 물론 유전으로 인한 문제, 성병에 대한 부정적 인식 등으로 사회악으로 까지 대두되었다. 매독이나 임질을 비롯한 대부분의 성병은 한번 감염되면 근치되기 어려운 질병으로 여겨졌으며, 성병감염환자는 해가 갈수록 늘어나 일제말기에는 51만 내지 60만명에 달할 것으로 추정되었다.
일제는 식민지 조선에서 성병 창궐에 대한 대책으로 1908년 창기단속령을 선포했다. 1916년 전국적인 통일법규를 발포하여 공창제는 전국으로 확장하였다. 성병이 만연하고 이로 인해 사회적 문제가 대두되자, 일제는 매춘부로 간주된 여성들에 대해 국가에 의한 매매춘 관리제도인 공창제를 도입하여 화류계를 각종 화류병의 온상이라고 지목하고, 주기적으로 강제적인 성병 검진을 실시하여 성병을 통제하고자 하였다. 이는 성을 사는 남성에게는 아무런 책임을 묻지 않고 성을 파는 여성만을 잠재적인 성병 감염원으로 간주한 것이기 때문에 실효를 거두기 어려웠다. 성병통제에 실패하고 성병이 날로 확산되자 일제는 화류병예방법을 공포하기도 하였다.
공창제를 통한 화류병예방과 통제의 문제점은 바로 화류병을 예방하지 못한다는 점이었다. 화류병의 폐해와 심각성은 날로 증가하였으며, 성병검사 방식도 문제점이 많았다. 경찰의 성병검진방식의 문제점은 성병을 가진 자를 제대로 치료하는 것이 아니었다. 경찰이 시행한 화류계 여성들에 대한 觸診式 성병검사는 여성들에게 수치심과 모멸감을 줄 뿐이었으며, 오히려 성병검진을 회피하게 되었다. 경찰의 강압적 성병 통제는 포주나 화류계 여성 모두에게 불안을 가져왔으며, 이는 성병 검진에 대한 저항으로 나타나기도 했다.
1930년대 공창이 몰락하고 사창이 번성하면서 일제의 매춘정책은 사창규제를 강화하는 방향으로 향하였다. 사창규제는 새로 유행하기 시작한 카페, 바와 거기서 일하는 여급들, 일반요리점과 음식점에서의 예기, 작부 고용녀들을 단속대상으로 했다. 1937년 중일전쟁이 일어나자 일제의 매춘정책은 공창제를 존속시키면서 성병위험을 어떻게 효율적으로 막을수 있는가에 두어졌다. 화류병의 예방은 평화시에도 국민보건상 중요한 문제였지만 전시에는 더욱이 중요해진다.
성병의 위협에 대처하는데 공창제가 실패함이 드러나자 일제는 화류병에 대처할 새로운 방법을 모색하였다. 1939년 일제는 식민지 조선에도 화류병예방법을 입안할 것을 계획하였으며, 아시아에서는 일본이 처음으로 1939년 국민우생법을 공포하였다. 중일전쟁 발발에 즈음하여 특히 인적자원의 보호를 위해 조선총독부에서는 1940년 8월 화류병예방법을 실시하고자 하였다가, 다음해로 연기하였다. 그러나 이 법은 실효를 거두지 못하였다. 억압적인 법률은 성병을 눈에 띄지 않는 곳으로 숨어들어가게 할 뿐이며 이렇게 되면 성병의 통제는 더욱 어려워질수밖에 없었다.

Sexual diseases are generally spread through sexual intercourse and most common were gonorrhea, syphilis, chancroid. People of Joseon suffered from sexual diseases, which did not come into social attention until Japanese Imperialism, along with tuberculosis and the disorders of their digestive system.
Under Japanese Imperialism, the term of venereal disease was more commonly used instead of sexual diseases because people believed the disease was being caused to spread by foul sexual intercourse of women in red-light districts or men frequenting there. Sexual diseases were considered even as a social ill because of the genetic problems it causes as well as the individual trouble, and the negative recognition of them. Most of the sexual diseases such as syphilis, gonorrhea, etc were thought to be hardly cured once they infect people and the number of the infected was increasing to reach 510,000 or up to 600,000 until the late of Japanese Imperialism.
In 1908, Prostitute Regulation was announced by Japanese Imperialism to control raging diseases in its colony, Joseon. A standard regulation was declared in 1916 and the registered prostitution system expanded nation-wide. In the situation that sexual diseases were so prevalent that social issues were raised from them, Japanese Imperialism, base on the idea that the origin of the diseases was the red-light districts, applied the registered prostitution system in which the government supervised the women assumed as prostitutes and tried to control sexual diseases forcing these women to have regular check-ups. However, it was not effective since men who bought prostitution were ignored and it charged only women with the possible sexual diseases. As sexual diseases spread wider and wider following the failure in controlling the diseases, Japanese Imperialism declared Venereal Disease Prevention Act.
Sexual diseases could not be prevented by the system based on registered prostitution. The hazard from the diseases was getting deeper and deeper day by day and the screening method also had many problems. The screening by the police was not for healing the sexually infected people. The police examined women of prostitution through palpation and it made them feel ashamed and disdained, and they rather tried to avoid any examination chances. On the other hand, resistance against screening sometimes arose because forceful regulation to control sexual diseases brought about a wide range of apprehension among both pimps and prostitutes.
In the 1930s, as the registered prostitution system declined and unlicensed prostitution increased, prostitution regulation of Japanese Imperialism changed to restrict private prostitution. It targeted on the waitresses and barmaids working in cafeterias or bars of new popular business trend and the gisaengs, waitresses and maids in common restaurants or eating places were overseen, too. In 1937, when the Sino-Japanese War broke out, the prostitution regulation of Japanese Imperialism turned its focus into finding the solution of how to reduce the risk of sexual infection with the existence of the registered prostitution system. Prevention of the diseases had been an important issue in peace and, in fact, had even more significance in war.
The failure in managing the risk of sexual infection through registered prostitute system urged Japanese Imperialism to find another method to cope with it. In 1939, Japanese Imperialism planned to frame the Venereal Disease Prevention Act in the colony Joseon along with the National Eugenic Act in Japan declared in 1939 first in Asia. In August, 1948, the government-general tried to put the Venereal Diseases Prevention Act in force to protect especially the human resources at the time of the Sino-Japanese War, but enforcement was postponed until the next year.
